1. A device comprising:
- a) a T-piece connected to b) an aerosol generator configured for breath actuation, said aerosol generator comprising i) a reservoir comprising fluid, said fluid comprising an antibiotic, ii) a vibrational element, and iii) a dome-shaped aperture plate with tapered holes, wherein said T-piece is attached to a junction of a ventilation circuit, wherein said junction comprises a Y-piece comprising a first end attached to an inspiratory line of said ventilator circuit, a second end attached to an expiratory line of said ventilator circuit, and a stem attached to an endotracheal tube, wherein said T-piece is attached to said stem.

Abstract
The present invention provides an improved means of treating tracheobronchitis, bronchiectasis and pneumonia in the nosocomial patient, preferably with aerosolized anti gram-positive and anti-gram negative antibiotics administered in combination or in seriatim in reliably sufficient amounts for therapeutic effect. In one aspect, the invention assures this result when aerosol is delivered into the ventilator circuit. In one embodiment the result is achieved mechanically. In another embodiment, the result is achieved by aerosol formulation. In another aspect, the invention assures the result when aerosol is delivered directly to the airways distal of the ventilator circuit. The treatment means eliminates the dosage variability that ventilator systems engender when aerosols are introduced via the ventilator circuit. The treatment means also concentrates the therapeutic agent specifically at affected sites in the lung such that therapeutic levels of administrated drug are achieved without significant systemic exposure of the patient to the drug. The invention further provides a dose control device to govern this specialized regimen.

3. A method of attaching and activating an aerosol generator, comprising:

A) providing a) a ventilation circuit comprising a Y-piece, wherein said Y-piece comprises a first end attached to an inspiratory line of said ventilator circuit, a second end attached to an expiratory line of said ventilator circuit, and a stem attached to an endotracheal tube, said endotracheal tube connected to a patient, and b) a device comprising a T-piece connected to an aerosol generator, said aerosol generator comprising i) a reservoir comprising fluid, said fluid comprising antibiotic, ii) a vibrational element, and iii) a dome-shaped aperture plate with tapered holes; B) attaching said T-piece to the stem of said Y-piece, thereby attaching said aerosol generator to said ventilation circuit; and C) activating said aerosol generator under conditions of breath actuation wherein the plate vibrates, causing fluid in said reservoir to be drawn through the tapered holes and into said Y-piece, creating an aerosol with a defined range of droplet sizes. - View Dependent Claims (4, 5, 6, 7, 8)

9. A method of attaching and activating an aerosol generator, comprising:

A) providing a) a ventilation circuit comprising a Y-piece, wherein said Y-piece comprises a first end attached to an inspiratory line of said ventilator circuit, a second end attached to an expiratory line of said ventilator circuit, and a stem attached to an endotracheal tube, said endotracheal tube connected to a patient, and b) a device comprising a T-piece connected to an aerosol generator, said aerosol generator comprising a reservoir comprising fluid, said fluid comprising antibiotic; B) attaching said T-piece to the stem of said Y-piece, thereby attaching said aerosol generator to said ventilation circuit; and C) activating said aerosol generator under conditions of breath actuation, causing aerosolized antibiotic to be drawn into said Y-piece and said patient during inspiration by said patient such that a more reproducible dose is delivered as compared to the dose delivered using the arrangement where the aerosol generator is in the inspiratory line. - View Dependent Claims (10, 11, 12, 13, 14)
