1. A system, comprising:
- a signal sensor circuit, including a sense amplifier circuit to sense a heart sound (HS) signal from a patient;
a memory circuit;
a HS component detector circuit, coupled to the signal sensor circuit and the memory circuit to detect at least first and different second HS components from the received HS signal;
a HS metric generator circuit, coupled to the HS component detector circuit to use the detected first and second HS components to respectively generate first and second HS metrics stored in the memory circuit;
a blending circuit, coupled to the memory circuit or the HS metric generator circuit to generate one or more composite HS metrics using a ratio of the second HS metric to the first HS metric or a product of the first and second HS metrics;
a heart failure (HF) event detector circuit, including a comparator circuit coupled to the blending circuit to detect a worsening HF status using a combination of (1) at least one of the first or second HS metric and (2) at least one of the one or more composite HS metrics; and
an output circuit configured to output an indication of the worsening HF status.

Abstract
Systems and methods for detecting events indicative of worsening using heart sounds are disclosed. A system can include a signal sensor circuit to sense a heart sound (HS) signal. The system can detect at least first and different second HS components using the HS signal, and generate respective first and second HS metrics. The system can determine a trend indicator for the first or second HS metric, and selectively generate one or more composite HS metrics using the first and second HS metrics, according to the trend indicator indicating a growing or decay trend. The system can include a heart failure (HF) event detector to produce a HF status using the composite HS metrics, and output an indication of the HF status, or deliver therapy according to the HF status.

sensing a heart sound (HS) signal from a patient using a HS sensor; detecting at least a first and a different second HS components from the sensed HS signal; generating first and second HS metrics respectively from the detected first and second HS components; generating one or more composite HS metrics using a ratio of the second HS metric to the first HS metric or a product of the first and second HS metrics; detecting a worsening HF status using a combination of (1) at least one of the first or second HS metric and (2) at least one of the one or more composite HS metrics; and outputting an indication of the detected worsening HF status. - View Dependent Claims (15, 16, 17, 18, 19, 20)
