1. A method for extending a lead, comprising:

  • inserting a first filament loop and a second filament loop over a lead, wherein the first and second filament loops comprise a filament, the first and second filament loops extending outside a body, wherein the body is elongated and has a longitudinal axis and a circumference, wherein the body includes an inner surface and an outer surface, a wall between the inner surface and the outer surface, a first aperture, a second aperture, a third aperture, and a fourth aperture formed through the wall, the second aperture being distally positioned relative to the first aperture, the third aperture being distally positioned relative to the second aperture, the fourth aperture being distally positioned relative to the third aperture, wherein the first aperture and the second aperture are offset along the longitudinal axis and offset along the circumference, the first aperture and the third aperture are offset along the longitudinal axis and offset along the circumference, the second aperture and the fourth aperture are offset along the longitudinal axis and offset along the circumference, the third aperture and the fourth aperture are offset along the longitudinal axis and offset along the circumference, the filament extending along the inner surface of the body and through the first aperture and the second aperture, the first filament loop extending outside the outer surface of the body through the first aperture and the second aperture, the second filament loop extending along the inner surface of the body and through the third aperture and the fourth aperture, the second filament loop extending outside the outer surface of the body;

    tightening simultaneously the first and second filament loops about the lead to grip at least a portion of an outer surface of the lead against the body; and

    applying tension to the lead by applying tension to a tether configured to be coupled to the lead via the first and second filament loops when the first and second filament loops are tightened about the lead.
