1. A method to determine the integrity of an implantable cardiac lead implanted in a patient, comprising:
- determining a length of the implantable cardiac lead;
calculating a test frequency based on the length;
using a frequency signal source to apply a set of test signals to the implantable cardiac lead at frequencies within a predetermined range of the test frequency, each test signal including a phase shifted signal;
measuring an instantaneous imaginary component of a transmission line impedance of the implantable cardiac lead in response to the each test signal;
determining an imaginary impedance for each test signal based on the imaginary component of the transmission line impedance;
comparing changes in the imaginary impedance for each test signal to a predetermined threshold to determine whether the implantable cardiac lead has a potential lead integrity issue; and
communicating an indication of the potential lead integrity issue in response to determining that the implantable cardiac lead has a potential lead integrity issue by reporting the potential lead integrity issue for the patient to a medical professional.

Scientific and medical system circuitry for diagnosis of implantable cardioverter defibrillator (ICD) lead conductor anomalies, in particular conductor migration and externalization within an ICD implantable cardiac lead. The system determines an “imaginary” component of the high frequency transmission line impedance having certain spectral changes that correspond to radially outward movements or local externalization of a conductor within a lead body allowing for the detection of conductor migration and small insulation failures.
