Abstract

A system and method to deliver a therapeutic quantity of energy to a patient. The system includes a capacitor having a rated energy storage capacity substantially equal to the therapeutic quantity of energy, a boost converter coupled with the capacitor and constructed to release energy from the capacitor at a substantially constant current for a time interval, and an H-bridge circuit coupled with the boost converter and constructed to apply the substantially constant current in a biphasic voltage waveform to the patient. The method includes storing a quantity of energy substantially equal to the therapeutic quantity of energy in a capacitor, releasing the quantity of energy at a relatively constant current during a time interval using a boost converter coupled with the capacitor, and delivering a portion of the quantity energy in a direction to the patient using an H-bridge circuit coupled with the boost converter.

Claims

1. A system to deliver therapeutic energy to a patient, the system comprising:

a capacitor configured to store and release the therapeutic energy;

a boost converter circuit coupled to the capacitor and constructed to control a shape of a therapeutic waveform that arises upon release of the therapeutic energy from the capacitor, the boost converter circuit including;

an inductor,

a current sensing network, coupled to the inductor, to sense a magnitude of current in the inductor,

a switch coupled between the inductor and the current sensing network, and

an operational amplifier configured to compare the magnitude of current in the inductor with a current profile,

wherein the boost converter circuit is constructed to control a state of the switch based on the comparison; and

a current flow control circuit coupled to the boost converter circuit and comprising a plurality of control circuits configured to linearly control a current output from the current flow control circuit upon release of the therapeutic energy from the capacitor.

2. The system of claim 1, wherein the current flow control circuit comprises a plurality of transistors arranged as an H-bridge circuit.

3. The system of claim 2 wherein each control circuit of the plurality of control circuits is configured to control a gate voltage of a respective transistor of the plurality of transistors to linearly control the current output from the current flow control circuit.

4. The system of claim 2 wherein the plurality of transistors comprises one of insulated-gate bipolar transistors, metal-oxide semiconductor field-effect transistors, and silicon-controlled rectifiers.

5. The system of claim 2 wherein the plurality of control circuits is configured to control each transistor of the plurality of transistors such that each transistor provides a voltage controlled current source.

6. The system of claim 1 wherein the plurality of control circuits is configured to linearly control the current output from the current flow control circuit such that the current flow control circuit is configured to provide both of a defibrillation waveform and a pacing waveform.

7. The system of claim 1 wherein the therapeutic waveform is a ramped biphasic waveform that increases to a desired voltage level over an initial portion of each phase.

8. The system of claim 7 wherein the initial portion of each phase has a duration of one to six milliseconds and a voltage of the therapeutic waveform gradually increases from a zero voltage to a target voltage over the duration of the initial portion of each phase.

9. The system of claim 1 wherein the current flow control circuit is configured to control the therapeutic energy delivered to the patient without providing a current path through dissipation resistors.

10. The system of claim 1 wherein the capacitor is a single capacitor having a minimum energy rating of approximately 305 Joules.

11. The system of claim 1 wherein, for a patient impedance of 200 ohms, a utilization of a capacitor energy storage capability is at least 69%.

12. The system of claim 11 wherein, for the patient impedance of 200 ohms, the utilization of the capacitor energy storage capability is less than 100%.

13. The system of claim 1 wherein, for a patient impedance of 150 ohms, a utilization of a capacitor energy storage capability is at least 72%.

14. The system of claim 13 wherein, for the patient impedance of 150 ohms, the utilization of the capacitor energy storage capability is less than 100%.

15. The system of claim 1 wherein a utilization of a capacitor energy storage capability is approximately 65%-75% for a patient impedance of approximately 150-200 ohms.

16. The system of claim 1 wherein the shape of the therapeutic waveform controlled by the boost converter circuit is represented by a patient voltage as a function of therapeutic energy delivery time and wherein an absolute value of the patient voltage exhibits a flatter shape for a higher impedance patient load than for a lower impedance patient load.

17. The system of claim 1 wherein the current output from the current flow control circuit cyclically varies as a function of therapeutic energy delivery time and wherein a shape of an envelope profile of the cyclically varying current output is a function of patient impedance and determines the shape of the therapeutic waveform.
