Abstract

Apparatuses and devices usable for annuloplasty are provided. These can include a plurality of tissue anchors and an annuloplasty structure for placement on a heart valve annulus. The annuloplasty structure can include a plurality of compressible or adjustable subunits, and a plurality of anchor mounts alternately disposed with respect to the plurality of compressible/adjustable subunits. The anchor mounts are coupleable to the tissue anchors and can define a path for passage therethrough of a respective tissue anchor. The apparatuses and devices can be independently adjustable and can be adjustable for treating various sizes of patient annuli. Other embodiments are also described.

Claims

1. An apparatus, comprising:

a plurality of helical tissue anchors; and

an annuloplasty structure for placement on an annulus of a heart valve, the annuloplasty structure comprising;

a plurality of compressible subunits, and

a plurality of anchor mounts alternately disposed with respect to the plurality of compressible subunits, each anchor mount defining a path for passage of a respective one of the plurality of helical tissue anchors;

wherein the annuloplasty structure can be transitioned from a first configuration having a first diameter to a second configuration having a second diameter different from the first diameter.

2. The apparatus according to claim 1, further comprising a structural element in the path for corkscrewing a distal portion of the respective one of the plurality of helical tissue anchors therearound.

3. The apparatus according to claim 2, wherein the structural element comprises a bar that is disposed in parallel with a plane of the annuloplasty structure, the plane including a portion of each of the plurality of anchor mounts.

4. The apparatus according to claim 2, wherein the structural element is configured to restrict continued corkscrewing of the respective one of the plurality of helical tissue anchors into tissue beyond a limit.

5. The apparatus according to claim 1, wherein the path of a first anchor mount of the plurality of anchor mounts is non-parallel relative to the path of a second anchor mount of the plurality of anchor mounts.

6. The apparatus according to claim 1, further comprising a rotatable ring coupled to the annuloplasty structure, such that rotation of the rotatable ring contracts the annuloplasty structure.

7. The apparatus according to claim 6, further comprising a lock ring arranged to lock the rotatable ring.

8. The apparatus according to claim 1, wherein the annuloplasty structure comprises a contracting wire configured to control transitioning from the first configuration to the second configuration.

9. The apparatus according to claim 1, wherein the second diameter is smaller than the first diameter.

10. The apparatus according to claim 1, wherein the plurality of compressible subunits comprise struts.

The apparatus according to claim 1, further comprising first and second rotatable rings coupled to the annuloplasty structure, arranged such that (a) the first rotatable ring is rotatable about its axis of rotation to contract the annuloplasty structure while the second rotatable ring remains rotationally stationary about its axis of rotation, and (b) the second rotatable ring is rotatable about its axis of rotation to contract the annuloplasty structure while the first rotatable ring remains rotationally stationary about its axis of rotation.

The apparatus according to claim 1, further comprising a ratchet coupled to the annuloplasty structure, wherein the ratchet is configured to maintain a perimeter of the annuloplasty structure after transitioning the annuloplasty structure from the first configuration to the second configuration.

The apparatus according to claim 1, further comprising one or more longitudinal members reversibly coupled to the annuloplasty structure at respective locations of the annuloplasty structure, wherein the one or more longitudinal members are arranged such that the one or more longitudinal members are moveable to adjust a configuration of respective parts of the annuloplasty structure at the respective locations of the annuloplasty structure.

The apparatus according to claim 13, wherein the one or more longitudinal members comprise at least first and second longitudinal members coupled to the annuloplasty structure at respective first and second locations of the annuloplasty structure, and are configured such that movement of the first one of the longitudinal members moves a first part of the annuloplasty structure at the first location with respect to a second part of the annuloplasty structure at the second location.

15. The apparatus according to claim 1, wherein each anchor mount comprises first and second openings, and wherein each respective one of the plurality of helical tissue anchors is moveable through the first and second openings of each anchor mount.

16. The apparatus according to claim 15, wherein the distal portion of one of the plurality of helical tissue anchors is arranged to corkscrew around a structural element before moving through the second opening.

17. An system, comprising:

a plurality of helical tissue anchors; and

an annuloplasty structure for placement on an annulus of a heart valve, the annuloplasty structure comprising;

a plurality of compressible subunits comprising struts, and

a plurality of anchor mounts alternately disposed with respect to the plurality of compressible subunits, each anchor mount defining a path for passage of a respective helical tissue anchor of the plurality of helical tissue anchors;

wherein the annuloplasty structure can be transitioned from a larger configuration to a smaller configuration to reduce a size of an annulus of a heart valve.

18. The system according to claim 17, wherein each of the plurality of compressible subunits is independently adjustable from an expanded configuration to a contracted configuration.

19. An annuloplasty system for reducing a size dimension of an annulus of a heart valve, comprising:

an annuloplasty structure comprising a plurality of adjustable subunits and a plurality of anchor mounts alternately disposed with respect to the plurality of adjustable subunits;

a first ring; and

a second ring,

wherein the first ring is rotatable about its axis of rotation to contract the annuloplasty structure while the second ring remains rotationally stationary about its axis of rotation, and the second ring is rotatable about its of rotation to contract the annuloplasty structure while the first ring remains rotationally stationary about its axis of rotation.

20. An annuloplasty system for reshaping an annulus of a native heart valve, comprising:

an annuloplasty structure comprising a plurality of adjustable segments and a plurality of anchor mounts connected to the plurality of adjustable segments;

a plurality of tissue anchors;

wherein each anchor mount of the plurality of anchor mounts is coupleable to a respective tissue anchor of the plurality of tissue anchors, such that the plurality of tissue anchors can be implanted in the annulus of the native heart valve to secure the annuloplasty structure to the annulus of the native heart valve at the plurality of anchor mounts; and

wherein the annuloplasty structure is adjustable such that it can be used to treat a variety of different sizes of annuli of different patients, and a first segment of the annuloplasty structure is adjustable independently of a second segment of the annuloplasty structure.
