1. At least one non-transitory computer-readable medium comprising computer-readable instructions for causing a computer comprising at least one processor to perform a method of automatically editing an automatically produced medical transcription of dictation from medical personnel concerning a patient, the method comprising:
- analyzing, using the at least one processor, the medical transcription for presence of a first trigger phrase spoken by the medical personnel specifying a template to be used for the medical transcription, the template having a plurality of fields, each corresponding to content of a particular type;
determining, using the at least one processor, that the first trigger phrase is present in the medical transcription if an actual phrase in the medical transcription matches a grammar for the first trigger phrase;
selecting, using the at least one processor, the template for the medical transcription from amongst a plurality of templates when it is determined that the first trigger phrase is present, wherein the template comprises one or more tags that identify a respective one of the plurality of fields;
identifying, using the at least one processor, at least one sub-trigger indicating the presence of content of a type corresponding to the respective one of the plurality of fields; and
inserting, using the at least one processor, the content in association with the respective one of the plurality of fields in the template, wherein inserting the content in association with the respective one of the plurality of fields in the template comprises replacing the one or more tags with the content.

A computer program product, for automatically editing a medical record transcription, resides on a computer-readable medium and includes computer-readable instructions for causing a computer to obtain a first medical transcription of a dictation, the dictation being from medical personnel and concerning a patient, analyze the first medical transcription for presence of a first trigger phrase associated with a first standard text block, determine that the first trigger phrase is present in the first medical transcription if an actual phrase in the first medical transcription corresponds with the first trigger phrase, and insert the first standard text block into the first medical transcription.
