Abstract

Embodiments disclosed herein may include systems and methods for determining a patient'"'"'s respiratory effort and blood oxygen saturation based on data acquired from a pulse oximetry sensor and analyzing the parameters in conjunction with each other. For example, the respiratory effort may be determined based on a photo-plethysmographic waveform generated from light attenuation detected by the sensor, and the blood oxygen saturation may be a pulse-based estimate of arterial blood oxygen saturation determined from the detected attenuation. Analysis of the parameters may enable detection and classification of apnea (e.g., obstructive or central) or another underlying cause for respiratory instability. Furthermore, the measured respiratory effort may be compared to respiratory effort supplied by a ventilator to ensure proper sensor placement before enabling automatic adjustment of ventilator settings.

Claims

1. A system comprising:

a ventilator configured to be attached to a patient and configured to operate based at least in part on a ventilator setting to generate a desired respiratory effort; and

a monitor configured to;

analyze a plethysmograph signal to determine information corresponding to an actual respiratory effort,

automatically adjust the ventilator setting based on blood oxygen saturation of the patient,

determine whether a desired relationship exists between the information corresponding to the actual respiratory effort and the desired respiratory effort, and

operate the ventilator such that the ventilator adjustments are suspended when the desired relationship is determined not to exist.

2. The system of claim 1 wherein the monitor is further configured to determine whether a relationship exists between a ventilation phase of the ventilator and the actual respiratory effort.

3. The system of claim 1 wherein the monitor is further configured to determine whether a relationship exists between a ventilation rate of the ventilator and the actual respiratory effort.

4. The system of claim 1 wherein the monitor is further configured to generate a sensor placement alarm signal when the desired relationship is determined not to exist.

5. The system of claim 1 wherein the blood oxygen saturation is determined based at least in part on the plethysmograph signal.

6. A processor-implemented method comprising:

operating a ventilator configured to be attached to a patient based at least in part on a ventilator setting to generate a desired respiratory effort;

analyzing a plethysmograph signal to determine information corresponding to an actual respiratory effort;

automatically adjusting the ventilator setting based on blood oxygen saturation of the patient;

determining whether a desired relationship exists between the information corresponding to the actual respiratory effort and the desired respiratory effort; and

operating the ventilator such that ventilator adjustments are suspended when the desired relationship is determined not to exist.

The method of claim 6 wherein determining whether a desired relationship exists between the information corresponding to the actual respiratory effort and the desired respiratory effort comprises determining whether a relationship exists between a ventilation phase of the ventilator and the actual respiratory effort.

The method of claim 6 wherein determining whether a desired relationship exists between the information corresponding to the actual respiratory effort and the desired respiratory effort comprises determining whether a relationship exists between a ventilation rate of the ventilator and the actual respiratory effort.

9. The method of claim 6 further comprising generating a sensor placement alarm signal when the desired relationship is determined not to exist.

10. The method of claim 6 wherein the blood oxygen saturation is determined based at least in part on the plethysmograph signal.

11. A processor-implemented method for use with a ventilator operable in accordance with a ventilator setting, the processor-implemented method comprising:

calculating a patient'"'"'s blood oxygen saturation value (Sp02) based on a received plethysmograph signal;

calculating the patient'"'"'s expected respiratory effort value (E_expected) based at least in part on the patient'"'"'s blood oxygen saturation;

determining the patient'"'"'s actual respiratory effort value (E_actual);

calculating a ventilation oximetry index (VOI) based on the patient'"'"'s actual respiratory effort value and the expected respiratory effort value;

providing an output based on the VOI;

determining whether a desired relationship exists between the expected and actual respiratory effort value; and

operating the ventilator such that ventilator adjustments relative to the ventilator setting are suspended when the desired relationship is determined not to exist.

12. The method of claim 11 further comprising:

calculating a predicted carbon dioxide saturation change value (dC02) based on the patient'"'"'s actual respiratory effort value and the expected respiratory effort value, wherein the VOI is based on the predicted carbon dioxide saturation change value.

13. The method of claim 12 wherein the dC02 is calculated according to a formula:

C02=40−

40(E_expected/E_actual).

14. The method of claim 13, wherein the VOI is calculated according to a formula:

VOI=Sp02−

0.5(dC02).

15. The method of claim 11, wherein the VOI is indicative of the patient'"'"'s blood oxygen saturation value that would have been calculated if the patient was not breathing abnormally.

16. A monitor for use with a ventilator operable in accordance with a ventilator setting, the monitor configured to:

calculate a patient'"'"'s blood oxygen saturation value (Sp02) based on a received plethysmograph signal;

calculate the patient'"'"'s expected respiratory effort value (E_expected) based at least in part on the patient'"'"'s blood oxygen saturation;

determine the patient'"'"'s actual respiratory effort value (E_actual);

calculate a ventilation oximetry index (VOI) based on the patient'"'"'s actual respiratory effort value and the expected respiratory effort value;

provide an output based on the VOI;

determine an existence of a desired relationship between the expected and actual respiratory effort value; and

operate the ventilator such that ventilator adjustments relative to the ventilator setting are suspended when the desired relationship is determined not to exist.

17. The monitor of claim 16 that is further configured to:

calculate a predicted carbon dioxide saturation change value (dC02) based on the patient'"'"'s actual respiratory effort value and the expected respiratory effort value, wherein the VOI is based on the predicted carbon dioxide saturation change value.

18. The monitor of claim 17 wherein the dC02 is calculated according to a formula:

dC02=40−

40(E_expected/E_actual).

19. The monitor of claim 18, wherein the VOI is calculated according to a formula:

VOI=Sp02−

0.5(dC02).

20. The monitor of claim 16, wherein the VOI is indicative of the patient'"'"'s blood oxygen saturation value that would have been calculated if the patient was not breathing abnormally.
