Abstract

A surgical stapler, or fastening instrument, may generally comprise a layer, such as a tissue thickness compensator, for example, releasably attached to a fastener cartridge and/or anvil by a flowable attachment portion. The flowable attachment portion may be indefinitely flowable. The flowable attachment portion may be flowable from the time that layer is installed to the fastener cartridge to the time in which the layer is implanted to patient tissue. The flowable attachment portion may comprise a pressure sensitive adhesive. The flowable attachment portion may comprise an adhesive laminate comprising a base layer comprising the tissue thickness compensator and an adhesive layer on at least a portion of a surface of the base layer comprising the pressure sensitive adhesive. Articles of manufacture comprising flowable attachment portion and methods of making and using the flowable attachment portion are also described.

Claims

1. A surgical stapler, comprising:

a staple cartridge, comprising;

a longitudinal slot configured to receive a cutting member, wherein the cutting member is translatable between a proximal position and a distal position within said longitudinal slot;

a cartridge body comprising a deck surface, wherein said deck surface comprises a first deck portion and a second deck portion extending above said first deck portion, wherein said first deck portion and said second deck portion are positioned on a first side of said longitudinal slot, and wherein said second deck portion is positioned closer to said longitudinal slot than said first deck portion;

a first row of staple cavities defined in said first deck portion;

a second row of staple cavities defined in said second deck portion; and

a plurality of staples removably stored in said first row of staple cavities and said second row of staple cavities;

an anvil comprising a plurality of staple forming pockets;

a buttress comprising a cartridge facing surface and an anvil facing surface, wherein said buttress is shaped such that said cartridge facing surface substantially mimics said deck surface when said buttress is attached to said cartridge body, and wherein said anvil facing surface comprises an arcuate surface;

a proximal attachment portion releasably attaching said buttress to said anvil, wherein said proximal attachment portion is configured to be incised by the cutting member;

a flowable portion located distal to said proximal attachment portion, wherein said flowable portion is configured to releasably attach said buttress to said anvil; and

a distal attachment portion located distal to said proximal attachment portion, wherein said distal attachment portion releasably attaches said buttress to said anvil.

2. The surgical stapler of claim 1, wherein said distal attachment portion is not incised by the cutting member as said cutting member translates toward said distal position from said proximal position.

3. The surgical stapler of claim 1, wherein said buttress comprises a cavity, and wherein said cavity comprises a medicament.

4. The surgical stapler of claim 3, wherein said buttress comprises a raised region comprising said cavity.

5. The surgical stapler of claim 4, wherein said raised region mimics the shape of said staple forming pockets of said anvil.

6. The surgical stapler of claim 1, wherein said buttress comprises a first cavity positioned adjacent to said longitudinal slot and a second cavity positioned adjacent to said longitudinal slot.

7. The surgical stapler of claim 1, wherein one of said anvil and said staple cartridge is rotatable relative to the other of said anvil and said staple cartridge to place said surgical stapler in a closed configuration.

8. The surgical stapler of claim 7, wherein said flowable portion comprises a pressure sensitive portion, and wherein said pressure sensitive portion is configured to releasably attach said buttress to said anvil when said surgical stapler is in said closed configuration.

9. A surgical stapler, comprising:

a staple cartridge, comprising;

a longitudinal slot configured to receive a cutting member, wherein the cutting member is translatable between a proximal position and a distal position within said longitudinal slot;

a plurality of staple cavities; and

a plurality of staples removably stored in said plurality of staple cavities;

an anvil comprising a plurality of staple forming pockets;

a buttress;

a proximal attachment portion releasably attaching said buttress to said anvil, wherein said proximal attachment portion is configured to be incised by the cutting member;

a flowable portion located distal to said proximal attachment portion, wherein said flowable portion is configured to releasably attach said buttress to said anvil; and

a distal attachment portion located distal to said proximal attachment portion, wherein said distal attachment portion releasably attaches said buttress to said anvil, wherein said buttress comprises a first cavity positioned adjacent to said longitudinal slot and a second cavity positioned adjacent to said longitudinal slot, wherein said first cavity comprises a first medicament and said second cavity comprises a second medicament, and wherein said first medicament and said second medicament are different.

10. A surgical stapler, comprising:

a staple cartridge, comprising;

a longitudinal slot configured to receive a cutting member, wherein the cutting member is translatable between a proximal position and a distal position within said longitudinal slot;

a plurality of staple cavities; and

a plurality of staples removably stored in said plurality of staple cavities;

an anvil comprising a plurality of staple forming pockets;

a buttress;

a proximal attachment portion releasably attaching said buttress to said anvil, wherein said proximal attachment portion is configured to be incised by the cutting member;

a flowable portion located distal to said proximal attachment portion, wherein said flowable portion is configured to releasably attach said buttress to said anvil; and

a distal attachment portion located distal to said proximal attachment portion, wherein said distal attachment portion releasably attaches said buttress to said anvil, wherein said buttress comprises a first cavity comprising a first volume and a second cavity comprising a second volume, and wherein said first volume and said second volume are different.

11. A surgical stapler, comprising:

a staple cartridge, comprising;

a longitudinal slot configured to receive a cutting member, wherein said cutting member is translatable between a proximal position and a distal position within said longitudinal slot;

a cartridge body comprising a first deck surface and a second deck surface extending above said first deck surface, wherein said first deck surface and said second deck surface are positioned on a first side of said longitudinal slot, and wherein said second deck surface is positioned closer to said longitudinal slot than said first deck surface;

a first row of staple cavities defined in said cartridge body, wherein each staple cavity of said first row of staple cavities defines an opening in said first deck surface;

a second row of staple cavities defined in said cartridge body, wherein each staple cavity of said second row of staple cavities defines an opening in said second deck surface; and

a plurality of staples removably stored in said first row of staple cavities and said second row of staple cavities;

an anvil comprising a plurality of staple forming pockets;

a buttress comprising a cartridge facing surface and a tissue facing surface, wherein said cartridge facing surface is shaped such that it substantially mimics said first deck surface and said second deck surface when said buttress is attached to said cartridge body, and wherein said tissue facing surface is arcuate;

a proximal attachment portion releasably attaching said buttress to said surgical stapler, wherein said proximal attachment portion is configured to be incised by said cutting member;

a flowable portion located distal to said proximal attachment portion, and wherein said flowable portion is configured to releasably attach said buttress to said surgical stapler; and

a distal attachment portion located distal to said proximal attachment portion, wherein said distal attachment portion releasably attaches said buttress to said surgical stapler.

12. The surgical stapler of claim 11, wherein said distal attachment portion is not incised by said cutting member as said cutting member translates toward said distal position from said proximal position.

13. The surgical stapler of claim 11, wherein said buttress comprises a cavity, and wherein said cavity comprises a healing agent.

14. The surgical stapler of claim 13, wherein said buttress comprises a raised region comprising said cavity.

15. The surgical stapler of claim 14, wherein said raised region mimics the shape of said staple forming pockets of said anvil.

16. The surgical stapler of claim 11, wherein said buttress comprises a first cavity positioned adjacent to said longitudinal slot and a second cavity positioned adjacent to said longitudinal slot.

17. The surgical stapler of claim 11, wherein one of said anvil and said staple cartridge is rotatable relative to the other of said anvil and said staple cartridge to place said surgical stapler in a closed configuration.

18. The surgical stapler of claim 17, wherein said flowable portion comprises a pressure sensitive attachment portion, and wherein said pressure sensitive attachment portion is configured to releasably attach said buttress to said surgical stapler when said surgical stapler is in said closed configuration.

19. A surgical stapler, comprising:

a staple cartridge, comprising;

a longitudinal slot configured to receive a cutting member, wherein said cutting member is translatable between a proximal position and a distal position within said longitudinal slot;

a plurality of staple cavities; and

a plurality of staples removably stored in said plurality of staple cavities;

an anvil comprising a plurality of staple forming pockets;

a buttress;

a proximal attachment portion releasably attaching said buttress to said surgical stapler, wherein said proximal attachment portion is configured to be incised by said cutting member;

a central portion located distal to said proximal attachment portion, and wherein said central portion is configured to releasably attach said buttress to said surgical stapler; and

a distal attachment portion located distal to said proximal attachment portion, wherein said distal attachment portion releasably attaches said buttress to said surgical stapler, wherein said buttress comprises a first cavity positioned adjacent to said longitudinal slot and a second cavity positioned adjacent to said longitudinal slot, wherein said first cavity comprises a first healing agent and said second cavity comprises a second healing agent, and wherein said first healing agent and said second healing agent are different.

20. A surgical stapler, comprising:

a staple cartridge, comprising;

a longitudinal slot configured to receive a cutting member, wherein said cutting member is translatable between a proximal position and a distal position within said longitudinal slot;

a plurality of staple cavities; and

a plurality of staples removably stored in said plurality of staple cavities;

an anvil comprising a plurality of staple forming pockets;

a buttress;

a proximal attachment portion releasably attaching said buttress to said surgical stapler, wherein said proximal attachment portion is configured to be incised by said cutting member;

a central portion located distal to said proximal attachment portion, and wherein said central portion is configured to releasably attach said buttress to said surgical stapler; and

a distal attachment portion located distal to said proximal attachment portion, wherein said distal attachment portion releasably attaches said buttress to said surgical stapler, wherein said buttress comprises a first cavity comprising a first volume and a second cavity comprising a second volume, and wherein said first volume and said second volume are different.

21. A surgical stapler, comprising:

a staple cartridge, comprising;

a longitudinal slot configured to receive a cutting member, wherein the cutting member is translatable between a proximal position and a distal position within said longitudinal slot;

a stepped deck surface, comprising;

a first deck surface; and

a second deck surface extending above said first deck surface, wherein said second deck surface is positioned closer to said longitudinal slot than said first deck surface;

a plurality of staple cavities defined in said staple cartridge, wherein each said staple cavity defines an opening in said stepped deck surface; and

a plurality of staples removably stored in said plurality of staple cavities;

an anvil comprising a plurality of staple forming pockets;

a buttress comprising a first surface facing said deck surface and a second surface facing said anvil when said buttress is attached to said staple cartridge, wherein said first surface is shaped such that it substantially mimics said stepped deck surface when said buttress is attached to said staple cartridge, and wherein said second surface comprises an arcuate surface;

a proximal attachment portion releasably attaching said buttress to said staple cartridge, wherein said proximal attachment portion is configured to be incised by the cutting member;

a flowable portion located distal to said proximal attachment portion, wherein said flowable portion is configured to releasably attach said buttress to said staple cartridge; and

a distal attachment portion located distal to said proximal attachment portion, wherein said distal attachment portion releasably attaches said buttress to said staple cartridge.
