Abstract

Adjacent tissue layers can be accessed using a catheter device with a distal tip having a conductive portion including a first cutting feature and one or more projections extending from the first cutting feature towards an outer diameter of the distal tip. Electrical energy can be supplied to the conductive portion of the device to cut tissue. A stent can be delivered to form a fluid communication between the adjacent tissue layers.

Claims

1. A catheter assembly, comprising:

a catheter body having a proximal end, a distal end, and a passage therethrough;

a tip positioned near the distal end of the catheter body, the tip being conically shaped and having a conductive portion including a distal ring-shaped cutting feature and a projection outwardly extending from the ring-shaped cutting feature towards an outer diameter of the tip, the tip further comprising an insulated portion extending radially further outward than the ring-shaped cutting feature, the conductive portion configured to be electrically coupled to an electrosurgical generator; and

a stent carried on the catheter body proximal to the tip.

2. The catheter assembly of claim 1, wherein the tip does not have a guidewire lumen.

3. The catheter assembly of claim 1, the catheter body further comprising a guidewire lumen, wherein the ring-shaped cutting feature is configured to be disposed about and insulated from the guidewire lumen by the insulated portion.

4. The catheter assembly of claim 1, wherein the conductive portion includes an exterior portion made of stainless steel and an interior portion comprising copper electrically coupled to the exterior portion and configured to be electrically coupled to the electrosurgical generator.

5. The catheter assembly of claim 1, wherein the electrosurgical generator is configured to supply about 50 watts to about 100 watts of power to the conductive portion of the tip.

6. The catheter assembly of claim 1, wherein the stent is a self-expanding stent and the self-expanding stent is radially restrained by a movable sheath.

7. The catheter assembly of claim 6, wherein the stent is configured to expand to form distal and proximal flanges when deployed.

8. The catheter assembly of claim 1, wherein the catheter body is configured to be attached to an endoscope.

9. The catheter assembly of claim 1, further comprising a control handle configured to control the energy provided by the electrosurgical generator to the conductive portion.

10. A catheter assembly, comprising:

a catheter body having a proximal end, a distal end, and a passage therethrough;

a tip positioned near the distal end of the catheter body, the tip being conically shaped and having a conductive portion including a distal ring-shaped cutting feature and a projection extending from the ring-shaped cutting feature proximally towards an outer diameter of the tip, the tip further comprising an insulating portion extending proximally from the ring-shaped cutting feature and radially further outward than the ring-shaped cutting feature, the conductive portion configured to be electrically coupled to an electrosurgical generator; and

a stent carried on the catheter body proximal to the tip.

11. The catheter assembly of claim 10, wherein the stent is a self-expanding stent and the self-expanding stent is radially restrained by a movable sheath.

12. The catheter assembly of claim 11, wherein the stent is configured to expand to form distal and proximal flanges when deployed.

13. A catheter assembly, comprising:

a catheter body having a proximal end, a distal end, and a central passage therethrough;

a tip positioned near the distal end of the catheter body, the tip being conically shaped and having a conductive portion including a distal ring-shaped cutting feature and a projection extending proximally and radially outward from the ring-shaped cutting feature, the tip further comprising an insulating portion extending radially further outward than the ring-shaped cutting feature, the conductive portion configured to be electrically coupled to an electrosurgical generator; and

a stent carried on the catheter body proximal to the tip.

14. The catheter assembly of claim 13, wherein the stent is a self-expanding stent and the self-expanding stent is radially restrained by a movable sheath.

15. The catheter assembly of claim 14, wherein the stent is configured to expand to form distal and proximal flanges when deployed.
