Abstract

Acute care method and apparatus for treating a patient experiencing thrombotic vascular occlusion includes introducing a selected dose of an active agent proximate a vascular occlusion in the patient in order to lyse the vascular occlusion and radiating the vascular occlusion and active agent.

Claims

1. An acute care method for treating a patient experiencing a thrombotic vascular occlusion, the method comprising:

introducing a selected dose of an agent for acting on a vascular occlusion in the patient in order to lyse said vascular occlusion; and

ultrasonically radiating said vascular occlusion and active agent in order to shorten onset and enhancing effectiveness of lysing action of said active agent.

2. The method according to claim 1 wherein the ultrasound is applied transcutaneously.

3. The method according to claim 1 wherein the ultrasound is applied intravascularly.

4. The method according to claim 2 wherein the active agent is introduced intravenously.

5. The method according to claim 4 wherein the active agent is introduced at an intravascular site of ultrasound application.

6. The method according to claim 1 wherein said vascular occlusion is a coronary occlusion.

7. The method according to claim 1 wherein said vascular occlusion is a cerebral occlusion.

8. The method of claim 1 wherein said vascular occlusion is a peripheral venous or arterial occlusion.

9. The method according to claim 1 further comprising the step of selecting the active agent on a basis of bleeding indication of the patient.

10. The method according to claim 1 wherein said active agent comprises a microbubble medium.

11. The method according to claim 10 wherein the microbubble medium is comprised of a medium selected from the group comprising free gas bubbles, stabilized gas bubbles, colloidal suspension, emulsion and aqueous solutions.

12. The method according to claim 1 wherein said active agent comprises a thrombolytic agent.

13. The method according to claim 1 wherein said active agent comprises an antiplatelet agent.

14. The method according to claim 1 wherein said active agent is an anticoagulant.

15. The method according to claim 1 wherein said active agent comprises a blood composition altering agent.

16. The method according to claim 15 wherein said blood composition altering agents are selected from a group consisting of a blood volume expansion agent, a blood replacement agent, a Hetastarch, Pentastarch and Dextran.

17. The method according to claim 1 wherein said active agent is an intravenous fat emulsion.

18. The method according to claim 17 wherein said intravenous fat emulsion is selected from a group comprising intralipid and liposyn.

19. The method according to claim 1 wherein said active agent is a GP IIb-IIIa platelet inhibitor.

20. The method according to claim 19 wherein the GP IIb-IIIa platelet inhibitor is Reopro.

21. The method according to claim 1 wherein said active agent is a GP IIb-IIIa platelet blocker.

22. The method according to claim 21 wherein the GP IIb-IIIa platelet blocker is selected from a group comprising Aggrastat and Integrillin.

23. The method according to claim 1 wherein the introduction of active agent and ultrasound radiation is performed as an emergency procedure.

25. The method according to claim 24 wherein the ultrasound is applied transcutaneously.

26. The method according to claim 24 wherein the ultrasound is applied intravascularly.

27. The method according to claim 25 wherein the active agent in introduced intravenously.

28. The method according to claim 27 wherein the active agent is introduced at an intravascular site of ultrasound application.

29. The method according to claim 24 wherein said vascular occlusion is a coronary occlusion.

30. The method according to claim 24 wherein said vascular occlusion is a cerebral occlusion.

31. The method according to claim 24 wherein said vascular occlusion is a peripheral venous or arterial occlusion.

32. The method according to claim 24 herein said active agent comprises a microbubble medium.

33. The method according to claim 32 wherein the microbubble medium is comprised of a medium selected from the group comprising free gas bubbles, stabilized gas bubbles, colloidal suspensions, emulsion and aqueous solutions.

34. The method of claim 24 wherein said active agent comprises a thrombolytic agent.

35. The method according to claim 24 wherein said active agent comprises an antiplatelet agent.

36. The method according to claim 24 wherein said active agent is an anticoagulant.

37. The method according to claim 24 wherein said active agent comprises a blood composition altering agent.

38. The method according to claim 37 wherein said blood composition altering agent is selected from a group comprising a blood volume expansion agent, a blood replacement agent, a Hetastarch, a Pentastarch and Dextran.

39. The method according to claim 24 wherein said active agent is an intravenous fat emulsion.

40. The method according to claim 39 wherein said intravenous fat emulsion is selected from a group consisting of intralipid and liposyn.

41. The method according to claim 24 wherein said active agent is a GP IIb-IIIa platelet inhibitor.

42. The method according to claim 41 wherein the GP IIb-IIIa platelet inhibitor is Reopro.

43. The method according to claim 24 wherein said active agent is a GP IIb-IIIa platelet blocker.

44. The method according to claim 42 wherein the GP IIb-IIIa platelet blocker is selected from a group comprising Aggrastat and Integrillin.

45. The method according to claim 24 wherein the introduction of active agents and ultrasound radiation is performed as an emergency procedure.

48. The apparatus according to claim 47 wherein said plurality of active agents include at least one agent for use with a patient having no indicated bleeding, at least one agent for use with a patient having possible indicated bleeding and at least one agent for use with a patient having indicated bleeding.

50. The apparatus according to claim 49 wherein said plurality of active agents includes at least one agent for use with a patient having no indicated bleeding, at least one agent for use with a patient having possible indicated bleeding and at least one agent for use with a patient having indicated bleeding.

24. An acute care method for treating a patient experiencing a thrombotic vascular occlusion, the method comprising:

selecting an active agent on a basis of bleeding indication of the patient;

introducing the selected dose of active agent into a patient experiencing a thrombotic vascular occlusion in order to lyse said vascular occlusion; and

ultrasonically radiating said vascular occlusion in order to enhance an effectiveness of the active agent lysing said vascular occlusion.

46. A method for treating a patient with a thrombotic vascular occlusion, or thrombosis, the method comprising:

introducing a selected dose of an agent into a patient with a thrombotic vascular occlusion, or thrombosis; and

ultrasonically radiating the agent in order to activate thrombus lysing properties of the agent.

47. Apparatus for effecting emergency treatment of a patient experiencing a thrombotic vascular occlusion, said apparatus comprising:

a plurality of active agents for dissolution of vascular occlusions, use of at least one of the active agents being selected on a basis of bleeding indication of the patient;

means for intravenously introducing a select agent into the patient; and

portable ultrasonic means for transcutaneously radiating the vascular occlusion.

49. A self-contained mobile unit for emergency treatment of a patient experiencing a thrombotic vascular occlusion, the unit comprising:

a plurality of active agents for dissolution of vascular occlusion, use of at least one of the active agents being selected on a basis of bleeding indication of the patient;

intravenous means for introducing a selected agent into the patient;

portable ultrasonic means for transcutaneously radiating the vascular occlusion; and

module means for storing the active agents, intravenous means and ultrasonic means.
