Abstract

The invention relates generally medical devices and methods for the treatment of glaucoma in an animal eye and, more particularly, to medical devices and methods for treating tissue of the trabecular meshwork and /or Schlemm'"'"'s canal of the eye to restore or rejuvenate a portion or all of the normal physiological function of directing aqueous outflow for maintaining a normal intraocular pressure in the eye.

Claims

1. A medical device system for treating tissue of trabecular meshwork of an eye comprising aspiration means for inducing a liquid flow through the trabecular meshwork in an opposite direction of a physiological aqueous outflow pathway.

2. The medical device system of claim 1, wherein the aspiration means comprises an elongated tubular member having a proximal end, a distal end and an inflatable cup balloon mounted at said distal end, wherein the cup balloon has a balloon rim defining an isolated enclosure adapted for inducing said liquid flow through the trabecular meshwork by a suction power exerted at the proximal end.

The medical device system of claim 1 further comprising an elongated device adapted for extending through the trabecular meshwork of the eye and comprising a lumen having an inlet port and an outlet port for providing said physiological outflow pathway, wherein the outlet port is positioned in Schlemm'"'"'s canal.

4. The medical device system of claim 3, wherein a therapeutic liquid is introduced through the inlet port for providing said liquid flow through the trabecular meshwork in an opposite direction of the physiological aqueous outflow pathway.

5. The medical device system of claim 4, wherein the therapeutic liquid comprises a pharmaceutical substance, the pharmaceutical substance being a sterile saline.

6. The medical device system of claim 4, wherein the therapeutic liquid comprises a pharmaceutical substance, the pharmaceutical substance being a steroid.

7. The medical device system of claim 4, wherein the therapeutic liquid comprises a pharmaceutical substance, the pharmaceutical substance being a growth factor.

8. The medical device system of claim 4, wherein the therapeutic liquid comprises a pharmaceutical substance, the pharmaceutical substance being an angiogenic inhibitor.

9. The medical device system of claim 4, wherein the therapeutic liquid comprises a pharmaceutical substance, the pharmaceutical substance being Imidazole antiproliferative agents.

10. The medical device system of claim 4, wherein the therapeutic liquid comprises a pharmaceutical substance, the pharmaceutical substance being quinoxoalines.

11. The medical device system of claim 4, wherein the therapeutic liquid comprises a pharmaceutical substance, the pharmaceutical substance being phsophonylmethoxyalkyl nucleotide analogs and related nucleotide analogs.

12. The medical device system of claim 4, wherein the therapeutic liquid comprises a pharmaceutical substance, the pharmaceutical substance being potassium channel blockers.

13. The medical device system of claim 4, wherein the therapeutic liquid comprises a pharmaceutical substance, the pharmaceutical substance being synthetic oligonucleotides.

14. The medical device system of claim 4, wherein the therapeutic liquid comprises a pharmaceutical substance, the pharmaceutical substance being Transforming Growth Factor-beta (TGF-beta).

15. The medical device system of claim 4, wherein the therapeutic liquid comprises a pharmaceutical substance, the pharmaceutical substance being 5-[1-hydroxy-2-[2-(2-methoxyphenoxyl)ethylamino]ethyl]-2-methylbenzenesulfonamide.

16. The medical device system of claim 4, wherein the therapeutic liquid comprises a pharmaceutical substance, the pharmaceutical substance being guanylate cyclase inhibitors.

17. The medical device system of claim 4, wherein the therapeutic liquid comprises a pharmaceutical substance, the pharmaceutical substance being selected from the group consisting of methylene blue, butylated hydroxyanisole, and N-methylhydroxylamine.

18. The medical device system of claim 4, wherein the therapeutic liquid comprises a pharmaceutical substance, the pharmaceutical substance being 2-(4-methylaminobutoxy) diphenylmethane.

19. The medical device system of claim 4, wherein the therapeutic liquid comprises a pharmaceutical substance, the pharmaceutical substance being a combination of apraclonidine and timolol.

20. The medical device system of claim 4, wherein the therapeutic liquid comprises a pharmaceutical substance, the pharmaceutical substance being cloprostenol analogs or fluprostenol analogs.

21. The medical device system of claim 4, wherein the therapeutic liquid comprises a pharmaceutical substance, the pharmaceutical substance being an ophthalmic composition that provides a sustained release of a water-soluble medicament, said watersoluble medicament comprising a crosslinked carboxy-containing polymer, a sugar, and water.

22. The medical device system of claim 4, wherein the therapeutic liquid comprises a pharmaceutical substance, the pharmaceutical substance being a non-comeotoxic serine-threonine kinase inhibitor.

23. The medical device system of claim 4, wherein the therapeutic liquid comprises a pharmaceutical substance, the pharmaceutical substance being a composition of non-steroidal glucocorticoid antagonist.

24. The medical device system of claim 4, wherein the therapeutic liquid comprises a pharmaceutical substance, the pharmaceutical substance being a prostaglandin analog or a derivative thereof.

25. The medical device system of claim 1 further comprising an elongated catheter device adapted for penetrating into Schlemm'"'"'s canal in an ab extemo procedure and comprising a catheter distal section with a plurality of micropores for providing a physiological liquid flow pathway, wherein said liquid flow passes through the trabecular meshwork in an opposite direction of a physiological aqueous outflow pathway.

The medical device system of claim 1 further comprising an elongated catheter device adapted for penetrating into Schlemm'"'"'s canal in an ab extemo procedure and comprising a catheter distal section with a water-jet capability for providing a physiological liquid flow pathway, wherein said liquid flow passes through the trabecular meshwork in an opposite direction of a physiological aqueous outflow pathway.

28. The medical device of claim 27, wherein said device is positioned inside Schlemm'"'"'s canal in an ab extemo procedure.

29. The medical device of claim 27, wherein said device is positioned through the trabecular meshwork in an ab interno procedure.

30. The medical device of claim 28 further comprising an imaging ultrasound arrangement mounted on said device for guiding a deployment procedure of said medical device.

31. The medical device of claim 29 further comprising an imaging ultrasound arrangement mounted on said device for guiding a deployment procedure of said medical device.

27. A medical device for treating tissue of trabecular meshwork of an eye comprising an ultrasound arrangement on said medical device for providing ultrasonic vibrational energy to stimulate said tissue of the trabecular meshwork.

32. A medical device for treating tissue of trabecular meshwork of an eye comprising a fiber optic arrangement on said medical device for providing light imaging function for tissue characterization.

33. The medical device of claim 32, wherein the light imaging function comprises a near infrared Raman spectroscopy.

35. The method of claim 34 further comprising aspiration means for directing a liquid flow through the trabecular meshwork in an opposite direction of the physiological aqueous outflow pathway.

36. The method of claim 35, wherein the aspiration means comprises an elongated tubular member having a proximal end, a distal end and an inflatable cup balloon mounted at said distal end, wherein the cup balloon has a balloon rim defining an isolated enclosure adapted for inducing said liquid flow through the trabecular meshwork by a suction power exerted at the proximal end.

34. A method for treating tissue of trabecular meshwork of an eye comprising directing a liquid flow through the trabecular meshwork in an opposite direction of a physiological aqueous outflow pathway.
