Abstract

An improved method for efficiently and accurately entering detailed data by yes/no entries so that the data is automatically recorded, optionally automatically distributed and optionally transformed into a readable prose report of the data, which is entered by yes/no markings.

Claims

1. A system for entering data by yes/no entries and producing at least one of a retrievable data base and a language text report of the entered data, the system comprising:

a) a workstation comprising a computer, including a screen, capable of displaying a form capable of receiving entries of yes/no data, capable of accessing a computer and programmed to permit access by an authorized user;

b) a plurality of templates, each of the templates showing a plurality of relevant inquiries and capable of accepting data entry as yes/no entries by a user, said templates being accessible on the workstation or on a computer accessible by the workstation;

c) a system access display programmed on the workstation or accessible by the workstation and providing the capability for a user to access a selected database or a selected template;

d) a plurality of modifiers related to and associated with designated inquiries on at least a portion of the templates and at least a portion of the specific sub-templates showing more detailed inquiries related to the inquiries on the templates and sub-templates for the entry of additional data;

e) a retrievable database for storing and retrieving entered data from at least one of the templates, sub-templates and modifiers; and

,

f) a language program accessible by the workstation and capable of producing a language text report of the entered data.

2. The system of claim 1 wherein the workstation comprises a terminal in communication with a computer programmed with the templates and modifiers.

3. The system of claim 1 wherein the workstation is programmed to display a database access display providing the capacity to select a template for a selected section of the database.

4. The system of claim 1 wherein the system is a system for entering medical data.

5. The system of claim 1 wherein the database access display enables the selection of a template selector for medical history, medical examination, medical course and medical discharge and related matters.

6. The system of claim 1 wherein the templates are capable of data entry by a yes/no entry on selected inquiries in each template inquiry.

7. The system of claim 1 wherein the system includes programming to enable a user to select a sub-template for each of at least a portion of the template inquiries, the sub-template including more detailed inquiries for the selected template inquiry to enable the user to enter more detailed data by yes/no entries.

8. The system of claim 7 wherein at least a portion of the sub-templates enable the selection of modifiers for at least a portion of the inquiries on the sub-templates, the modifiers including more detailed inquiries for the sub-template inquiry to enable the user to enter more detailed data by yes/no entries.

9. The system of claim 1 wherein at least a portion of the templates and modifiers enable the entry of data in addition to the yes/no responses.

10. The system of claim 1 wherein the language program produces an English language text report of the entered data.

11. The system of claim 1 wherein the workstation is programmed to access additional databases.

12. The system of claim 1 wherein the system further includes a computer programmed to receive, distribute or direct distribution of all or selected portions of the entered data to at least one identified recipient or database.

13. The system of claim 12 wherein the computer is programmed to store the entered data and maintain a record of distribution of the entered data.

14. The system of claim 12 wherein the selected portions of the data are distributable automatically to the identified recipients or databases according to programmed instructions.

15. The system of claim 1 wherein the language program comprises a program capable of producing a language text report of the entered data as a single sentence report for each data entry.

16. The system of claim 15 wherein the single sentences optionally include data entry modifying adjectives from the modifiers.

17. The system of claim 15 wherein the language program is further capable of organizing the single sentences into plural object sentences, accurately reporting a plurality of data entries in a single plural object sentence.

18. A method for distributing copies of medical records, the method comprising:

a) entering the medical records into a medical record database;

b) entering a plurality of distribution options into a distribution database;

c) selecting a distribution option from the distribution database;

d) distributing medical records according to the selected distribution option; and

,

e) retaining in a database a record of the distribution of the medical records for retrieval as required.

19. The method of claim 18 wherein the medical records are identified for a selected distribution option and automatically electronically distributed according to the selected distribution option.

20. The method of claim 18 wherein the medical records are identified for a selected distribution option and copies are automatically produced for distribution according to the selected distribution option.

21. A system for distributing copies of medical records, the system comprising:

a) a database containing the medical records;

b) a computer programmed to access the database and to distribute the medical records according to at least one selected distribution option via at least one communication means; and

,

c) a database programmed to retain a record of the medical records sent the address to which the medical records were sent for retrieval upon request.

22. The system of claim 21 wherein the communication means comprises a system for electronically transmitting the medical records to pre-selected addresses.

23. The system of claim 21 wherein the communication means comprises a system for producing copies of the medical records for physical delivery to selected addresses.

24. A method for generating easily readable English text from simple sentences, each of the sentences reporting a single data entry and optionally containing modifiers of one or more of the data entries comprising:

a) selecting the words or phrases, which correspond to the reported data entries;

b) selecting which words or phrases can be combined into a single sentence and selecting the order of the selected words or phrases;

c) limiting the number of words or phrases, which can be combined in a single sentence;

d) arranging the words or phrases so that modifiers modify only the designated word or phrase; and

,

e) supplying the punctuation and conjuctives to create the English text.

25. The method of claim 24 wherein the number of words or phrases as objects of a sentence is limited to five.

26. The method of claim 24 wherein repetitive words are eliminated.
