Abstract

Disclosed is a method for improving cognitive function or for improving coordination of function across a patient'"'"'s cortical regions. The method includes applying electrical stimulation to at least a portion of the patient'"'"'s subcortical structures involved in the generation and control of generalized efference copy signals. Internally generated movement of the patient is then detected and, in response to such internally generated movement, application of electrical stimulation is controlled. The method of the present invention has a number of benefits, including increasing flexibility in identifying targets for stimulation, improving the probability of successfully treating brain injury, and permitting patient biofeedback and self-regulation.

Claims

1. A method for improving cognitive function of a conscious patient, said method comprising:

selecting a conscious patient;

applying electrical stimulation to at least a portion of the patient'"'"'s subcortical structures involved in the generation and control of generalized efference copy signals under conditions effective to improve the patient'"'"'s cognitive function;

detecting internally generated movement of the patient; and

controlling said applying electrical stimulation in response to detected internally generated movement of the patient.

2. A method according to claim 1, wherein the patient does not suffer chronic pain.

3. A method according to claim 1, wherein the patient does not suffer from generalized seizures.

4. A method according to claim 1, wherein the patient does not suffer chronic pain and does not suffer from generalized seizures.

5. A method according to claim 1, wherein the subcortical structure is the patient'"'"'s intralaminar nuclei.

6. A method according to claim 1, wherein the patient has impaired cognitive function which can be modulated by external or internally generated stimulation.

7. A method according to claim 1, wherein the patient exhibits spontaneous fluctuations in cognitive function impairment.

8. A method according to claim 5, wherein the at least a portion of the patient'"'"'s intralaminar nuclei comprises paralamellar regions.

9. A method according to claim 5, wherein electrical stimulation is applied only to the at least a portion of the patient'"'"'s intralaminar nuclei.

10. A method according to claim 1, wherein the patient exhibits deficits of attention, intention, working memory, or awareness.

11. A method according to claim 1, wherein the patient has impaired cognitive function which is, at least partially, the result of a brain injury produced by stroke, blunt head trauma, missile penetration, toxicological agents, anoxia, ischemia, nutritional deficiencies, developmental diseases, degenerative diseases, infectious diseases, neoplastic diseases, autoimmune diseases, or complications thereof.

12. A method according to claim 5, wherein the patient has a specific cognitive function impairment, wherein said method further comprises:

selecting a subdivision of the patient'"'"'s intralaminar nuclei which modulates the specific cognitive function, and wherein said electrical stimulation is applied to at least the selected subdivision.

13. A method according to claim 12, wherein electrical stimulation is applied only to the selected subdivision.

14. A method according to claim 12, wherein said selecting comprises:

determining an area of the brain having reduced baseline function; and

correlating the area of the brain having reduced baseline function with an intralaminar nuclei subdivision which projects thereto.

15. A method according to claim 14, wherein the area of the brain having reduced baseline function exhibits increased function during periods of patient activity.

16. A method according to claim 14, wherein electrical stimulation is applied only to the selected subdivision.

17. A method according to claim 15, wherein said determining is carried out metabolically, physiologically, or anatomically.

18. A method according to claim 1, wherein said applying electrical stimulation comprises:

contacting the at least a portion of the patient'"'"'s subcortical structure with an electrode under conditions effective to deliver an electrical signal to the at least a portion of the patient'"'"'s subcortical structure.

19. A method according to claim 11, further comprising:

contacting at least a portion of the patient'"'"'s subcortical structure with one or more pharmacological agents.

20. A method according to claim 1, wherein electrical stimulation is applied continuously.

21. A method according to claim 1, wherein electrical stimulation is applied intermittently.

22. A method according to claim 1, wherein electrical stimulation is applied periodically.

23. A method according to claim 5, wherein electrical stimulation is applied to two or more subdivisions of the intralaminar nuclei which modulate separate cortical regions.

29. A method according to claim 1, wherein said method identifies regional or intrahemispheric changes in brain waves.

30. A method according to claim 1 further comprising:

contacting the at least a portion of the patient'"'"'s subcortical structure with a pharmacological agent.

31. A method according to claim 1, wherein the internally generated movement of the patient is selected from the group consisting of head movement, neck movement, torso movement, and saccadic eye movement.

33. A method according to claim 1, wherein the subcortical structure is median dorsalis or pulvinar nuclei of one or both thalami.

34. A method according to claim 1, wherein the subcortical structure is the globus pallidus interna, superior colliculus, or mesencephalic reticular formation of one or both thalami.

35. A method according to claim 1, wherein said controlling is carried out to adjust intensity of said applying in response to detected internally generated movement of the patient.

36. A method according to claim 1, wherein said controlling is carried out to adjust frequency of said applying in response to detected internally generated movement of the patient.

37. A method according to claim 1, wherein said controlling is carried out to adjust pulse width of said applying in response to detected internally generated movement of the patient.

38. A method according to claim 1, wherein said controlling is carried out to adjust time on and time off of said applying in response to detected internally generated movement of the patient.

39. A method according to claim 1 further comprising:

augmenting said applying electrical stimulation by applying a non-electrical stimulus.

40. A method according to claim 39, wherein the non-electrical stimulus is auditory.

24. A method according to claim 24, wherein electrical stimulation is applied periodically.

25. A method according to claim 24, wherein electrical stimulation applied to the two or more subdivisions of the intralaminar nuclei is synchronized.

26. A method according to claim 24, wherein at least one of the two or more subdivisions of the intralaminar nuclei lies in the right thalamus and at least one of the two or more subdivisions of the intralaminar nuclei lies in the left thalamus.

27. A method according to claim 24, wherein the patient has a specific cognitive function impairment and wherein at least one of the two or more subdivisions of the intralaminar nuclei modulates the specific cognitive function.

28. A method according to claim 24, wherein the patient has a specific cognitive function impairment and wherein at least two of the two or more subdivisions of the intralaminar nuclei modulates the specific cognitive function.

32. A method according to claim 32, wherein the internally generated movement of the patient is saccadic movement.

41. A method for improving coordination of function across cortical regions in a patient comprising:

applying electrical stimulation to two or more subdivisions of the patient'"'"'s subcortical structures involved in the generation and control of generalized efference copy signals, wherein the two or more subdivisions of the patient'"'"'s subcortical structures modulate separate cortical regions;

detecting internally generated movement of the patient; and

controlling said applying electrical stimulation in response to detected internally generated movement of the patient.

42. A method according to claim 41, wherein the subcortical structure is the patient'"'"'s intralaminar nuclei.

43. A method according to claim 41, wherein electrical stimulation applied to the two or more subdivisions of the intralaminar nuclei is synchronized.

44. A method according to claim 41, wherein electrical stimulation applied to the two or more subdivisions of the intralaminar nuclei is periodic.
