Abstract

A health information gathering system for gathering information from at least one user. The health information gathering system includes a database storing information relating to a user and pertaining to at least two of the areas of information selected from a group consisting of nutritional information, psychological information, occupational information, physical information and personal history information, access to the information stored in the database being controlled by the user. The user can input information pertaining to the at least two areas of information selected from the group consisting of nutritional information, psychological information, occupational information, physical information and personal history information into the database, and the user can retrieve information from the database.

Claims

1. A health information gathering system for gathering information from at least one user, comprising:

a database storing information relating to a user and pertaining to at least two of the areas of information selected from a group consisting of nutritional information, psychological information, occupational information, physical information and personal history information, access to the information stored in the database being controlled by the user;

means for permitting the user to input information pertaining to the at least two areas of information selected from the group consisting of nutritional information, psychological information, occupational information, physical information and personal history information into the database; and

means for permitting the user to retrieve information from the database.

2. The health information gathering system of claim 1, wherein the database is accessible over the Internet.

3. The health information gathering system of claim 1, further comprising means for charging a subscription fee.

4. The health information gathering system of claim 1, further comprising means for preparing a personal health status profile based on the information contained in the database and related to the user.

5. The health information gathering system of claim 4, further comprising means for printing the personal health status profile whereby the personal health status profile can be presented to a healthcare provider.

6. A method for permitting a user to capture and retain history and symptom information, comprising the steps of:

outputting, electronically, a plurality of assessments pertaining to at least one area of information selected from a group consisting of nutritional information, psychological information, occupational information, physical information and personal history information, each of the assessments including at least one question related to the health of a user;

receiving answers from a user based on the assessments;

storing the answers in a database;

receiving a selection from the user regarding particular ones of the answers to include in a personal health status profile;

generating the personal health status profile based on the selection from the user; and

outputting the personal health status profile.

7. The method of claim 6, wherein in the step of generating the personal health status profile includes suggested changes to the user'"'"'s lifestyle to improve the user'"'"'s health.
