Abstract

The invention consists in a device that concerns the optimization of the cardiac geometry in patients with heart failure. The device comprises one or more elastic elements in the radial direction towards the inside of the ventricle and plastic deformation in a direction that is transversal to the said ventricle, the element being equipped with means for attaching it to the internal wall of the ventricle.

Claims

1. Endoventricular device for treating dilating cardiomyopathy by correcting the geometry of the left ventricular cavity (1) without the removal of cardiac mass, characterised by the fact that the device (6, 7 and 8) consists of at least one element whose shape is substantially similar to that of the perimeter of the portion of endocardium to which it is applicable, and which is respectively susceptible to elastic deformation in a radial direction towards the inside of the ventricle and plastic deformation in a direction that is transversal to the said ventricle, the element being equipped with means (12) for attaching it to the internal wall of the ventricle and/or the mitral valve annulus.

Device according to claim 1 characterised by the fact that the said element is covered by a woven covering of biological or biocompatible material, the said covering presenting a flat part and having external parts (12) extending and projecting from both sides of the said element, the said parts being suitable for being attached by the flat part of the covering to the internal wall (2) of the ventricle (1) and/or the mitral valve annulus (3) by means of suturing.

3. Device according to claim 2 characterised by the fact that the material constituting the said covering is dacron, teflon or goretex.

4. Device according to claim 2 characterised by the fact that the biological material constituting the said covering is human or animal pericardium.

Device according to claim 1 characterised by the fact that the said element is equipped with means for attaching it to the internal wall of the ventricle (1) and/or the mitral valve annulus (3), the said means consisting of at least one area of narrowing of the transversal section along its surface, the said area of narrowing being susceptible to being directly sutured to the said internal wall of the ventricle and/or the mitral valve annulus.

6. Device according to the previous claims characterised by the fact that the said element consists of closed or open rings.

7. Device according to the previous claims characterised by the fact that the transversal section of the said element is rectangular, or circular. or elliptical, or semi-circular, or semi-elliptical.

8. Device according to the previous claims characterised by the fact that the said element is constituted of closed or open bands.

9. Device according to the claims from 1 to 7 characterised by the fact that it is constituted of linear strips.

10. Device according to the claims from 1 to 5 characterised by the fact that it is constituted of closed or open helicoid springs.

11. Device according to the claims from 1 to 5 characterised by the fact that it is constituted of closed or open planar springs.

12. Device according to claim 1 characterised by the fact that the said elastic deformation is in a non-linear relationship with endoventricular pressure.

13. Device according to claim 12 characterised by the fact that the said elastic deformation decreases as endoventricular pressure increases.

14. Device according to claim 13 characterised by the fact that the said elastic deformation continuously limits diastolic dilatation.

15. Device according to the claims from 2 to 5 characterised by the fact that the suturing stitches are reinforced with pladgets of biological or biocompatible material.

16. Device according to claim 1 characterised by the fact that the said device consists of more than one element radially connected by elastic members.

17. Device according to claim 1 characterised by the fact that the said element includes diametrically connected elastic members (10).

18. Device according to claim 17 characterised by the fact that it consists of more than one element axially connected by elastic members (10).

19. Device according to claim 1 characterised by the fact that the said element is equipped with means running through the myocardial wall for attaching it.

20. Device according to claim 1 characterised by the fact that the device (6, 7 and 8) is applicated with a mattress suture accomplished with the open ring/band itself running through the thickness of the muscular wall and the junction of the tips at a predetermined measure.

21. Device according to claim 1 characterised by the fact that any component of the device must be characterized by radioopacity by means of intrinsic radioopacity of the material used (metal spring) or the addition of radioopaque elements inside the elastic material or the coverage material.

22. Device according to claim 1 characterised by the fact that the said device is capable of ceding elastic energy during the systolic phase.

23. Device according to claim 1 characterised by the fact that the said device is provided with one or more radially elastic elements positioned along different planes of the ventricular axis and transversally deformable in such a way as to adjust to the geometry of the said ventricle;

the said elements expanding under the action of endoventricular pressure in accordance with claims 12, 13 and 14.

24. Device according to claim 1 characterised by the fact that the said device is provided of more than one radially elastic element. connected radially and/or axially by linear elastic members;

the said elements being positioned along different planes of the ventricular axis and axially deformable in such a way as to adjust to the geometry of the said ventricle;

the said elements expanding under the action of endoventricular pressure in accordance with claims 12, 13 and 14 and allowing a distribution of wall stress in the volume of the ventricle by means of the said linear elastic members.
