Abstract

A method and system for generating a characterization of one beat of a patient'"'"'s supraventricular rhythm (SVR) involves performing such characterization while the heart is being paced. During SVR characterization, various pacing parameters are modified and the patient'"'"'s supraventricular rhythm is characterized while the pacing parameters are modified. The SVR characterization process is effective in single and multiple chamber pacing modes.

Claims

1. A method of characterizing a patient'"'"'s supraventricular rhythm while the heart is being paced, comprising:

pacing the patient'"'"'s heart;

modifying one or more pacing parameters; and

characterizing the patient'"'"'s supraventricular rhythm during a time in which the pacing parameters are modified.

2. The method of claim 1, further comprising resuming a pacing regimen following characterization of the patient'"'"'s supraventricular rhythm or if the characterization of the patient'"'"'s supraventricular rhythm is unsuccessful.

3. The method of claim 1, wherein the characterization of the patient'"'"'s supraventricular rhythm is performed automatically.

4. The method of claim 1, wherein the characterization of the patient'"'"'s supraventricular rhythm is performed upon command.

5. The method of claim 1, wherein modifying one or more pacing parameters includes one or more of modifying a pacemaker mode, modifying an AV delay, modifying a pacing rate, modifying a pacing interval, and suspending one or more pacemaker functions.

6. The method of claim 5, wherein modifying the pacemaker mode includes changing the pacemaker mode from a tracking, rate responsive mode to a non-tracking, non-rate responsive mode.

7. The method of claim 5, wherein modifying the AV delay includes lengthening the AV delay by a predetermined delay.

8. The method of claim 7, wherein the predetermined delay is a longest safe AV delay for tachycardia sensing.

9. The method of claim 8, wherein the longest safe AV delay for tachycardia sensing is calculated as:

longest safe AV delay=lower rate interval−

slowest VT interval−

safety window.

10. The method of claim 9, wherein the safety window is about 20 ms.

11. The method of claim 5, wherein modifying the pacing rate includes decreasing the pacing rate to a lower rate limit.

12. The method of claim 5, wherein suspending one or more pacemaker functions includes suspending one or more of rate smoothing up, rate smoothing down, dynamic AV delay, dynamic postventricular atrial refractory period, AV search interval, rate hysteresis offset, sensed AV delay offset, postventricular atrial refractory period after premature ventricular complex, dynamic ventricular refractory period, and ventricular rate regularization.

13. The method of claim 3, wherein the patient'"'"'s supraventricular rhythm is characterized automatically at a time when the patient'"'"'s sensed physiologic heart rate demand indicates the patient is at rest.

14. The method of claim 13, wherein the patient'"'"'s sensed physiologic heart rate demand is indicated by an output of a rate responsive subsystem.

15. The method of claim 14, wherein the output of the rate responsive subsystem is derived using signals from one or more rate response sensors that sense patient conditions.

16. The method of claim 15, wherein the patient conditions sensed include physical activity and respiration demand.

17. The method of claim 15, wherein the output of the rate responsive subsystem is derived using a combination of the signals from one or more types of the rate response sensors.

18. The method of claim 14, wherein the patient is at rest when the output of the rate responsive subsystem is less than a predetermined value.

19. The method of claim 18, wherein the predetermined value is 5 bpm.

20. The method of claim 13, wherein the patient is at rest when the patient'"'"'s V rate is in a lowest quartile of a preceding 24 hour period.

21. The method of claim 3, wherein characterization of the patient'"'"'s supraventricular rhythm is automatically attempted with pacing modification and if a first predetermined time interval has elapsed since a last unsuccessful SVR characterization attempt with pacing modification.

22. The method of claim 21, wherein the first predetermined time interval is selected so that SVR characterization attempts occur at different times of day.

23. The method of claim 22, wherein the first predetermined time interval is about 27 hours.

24. The method of claim 3, wherein characterization of the patient'"'"'s supraventricular rhythm is automatically attempted if a second predetermined time interval has elapsed since a last successful SVR characterization.

25. The method of claim 24, wherein the second predetermined time interval is about one week.

26. The method of claim 24, wherein the second predetermined time interval is a multiple of 24 hours.

27. The method of claim 3, wherein automatic characterization of the patient'"'"'s supraventricular rhythm is terminated upon sensing at least one predetermined patient condition.

28. The method of claim 27, wherein patient conditions that terminate characterization of the patient'"'"'s supraventricular rhythm include an increase in patient physiologic heart rate demand beyond an acceptable limit, a predetermined number of beats with modified pacing parameters is exceeded, the patient'"'"'s average heart rate becomes elevated or a predetermined number of paced beats are seen after pacing parameter modifications are made.

29. The method of claim 1, wherein characterizing a patient'"'"'s supraventricular rhythm further comprises:

providing a plurality of templates;

selectively updating the plurality of templates using a plurality of qualified beats; and

characterizing the patient'"'"'s supraventricular rhythm using a particular template of the plurality of updated templates.

30. The method of claim 29, wherein providing a plurality of templates further comprises retrieving at least a first template from memory and forming one or more other templates using the qualified beats.

31. The method of claim 29, further comprising detecting qualified beats by:

sensing rate channel signals;

sensing shock channel signals; and

determining if the rate channel signals and the shock channel signals meet predetermined criteria.

32. The method of claim 29, wherein updating the plurality of templates further comprises:

determining if a qualified beat is correlated to a selected template of the plurality of templates; and

updating the selected template with the qualified beat if the qualified beat is correlated to the selected template.

33. The method of claim 32, wherein determining if the qualified beat is correlated to the selected template comprises:

determining a fiducial point of the qualified beat and a fiducial point of the selected template, respectively;

aligning shock channel signals of the qualified beat and the selected template using the fiducial point of the qualified beat and the fiducial point of the selected template;

identifying a plurality of features of shock channel signals of the qualified beat and the selected template; and

using the plurality of selected features to determine if the qualified beat is correlated to the selected template.

34. The method of claim 1, wherein modifying one or more pacing parameters includes increasing the AV delay incrementally by a predetermined interval each beat until an AV delay limit is reached during dual chamber pacing.

35. The method of claim 34, wherein the predetermined interval is about 400 ms.

36. The method of claim 34, wherein the AV delay limit is a longest safe AV delay for tachycardia sensing.

37. The method of claim 36, wherein the longest safe AV delay is characterized by:

longest safe AV delay=lower rate interval−

slowest VT interval−

safety window.

38. The method of claim 34, further comprising modifying a pacemaker mode if a predetermined number intrinsic beats is not detected after increasing the AV delay until the AV delay limit is reached.

39. The method of claim 38, wherein modifying the pacemaker mode includes modifying the pacemaker mode from a tracking pacing mode to a non-tracking pacing mode.

40. The method of claim 39, further comprising decreasing the pacing rate to a predetermined lower rate limit after modifying the pacemaker mode from the tracking pacing mode to the non-tracking pacing mode.

41. The method of claim 1, wherein modifying the one or more pacing parameters includes increasing a V-V interval by a predetermined time interval each beat until a lower rate limit is reached during single chamber pacing.

42. The method of claim 41, wherein the predetermined time interval is about 100 ms.

43. The method of claim 1, wherein modifying the one or more pacing parameters further comprises suspending left side pacing when both ventricles of the patient'"'"'s heart are being paced prior to modification.

44. The method of claim 43, further comprising increasing an AV delay incrementally by a predetermined interval each beat until an AV delay limit is reached during dual chamber pacing.

45. The method of claim 44, wherein the AV delay limit is a safe AV delay for tachycardia sensing.

46. The method of claim 44, further comprising modifying a pacemaker mode if a predetermined number intrinsic beats is not detected after increasing the AV delay until the AV delay limit is reached.

47. The method of claim 46, wherein modifying the pacemaker mode includes modifying the pacemaker mode from a dual chamber pacing mode to a single chamber pacing mode.

48. The method of claim 47, further comprising decreasing the pacing rate to a predetermined lower rate limit after modifying the pacemaker mode from a dual chamber pacing mode to a single chamber pacing mode.

49. A method of characterizing a patient'"'"'s supraventricular rhythm while the heart is being paced, comprising:

pacing the patient'"'"'s heart;

modifying a pacing mode;

decreasing a pacing rate;

increasing an AV delay interval; and

characterizing the patient'"'"'s supraventricular rhythm during a time in which pacing is modified.

50. The method of claim 49, wherein modifying the pacing mode comprises modifying the pacing mode from a tracking, rate responsive mode to a non-tracking, non-rate responsive mode.

51. The method of claim 49, wherein the pacing rate is gradually decreased to a lower rate limit.

52. The method of claim 49, wherein the AV delay interval is abruptly extended to a longest safe AV delay up to a predetermined AV delay limit.

53. The method of claim 52, wherein the predetermined AV delay limit is about 400 ms.

54. The method of claim 52, wherein the longest safe AV delay is characterized by:

longest safe AV delay=lower rate interval−

slowest VT interval−

safety window.

55. The method of claim 49, wherein the patient'"'"'s supraventricular rhythm is characterized if a predetermined number of intrinsic beats is detected.

56. The method of claim 49, wherein a normal pacing regimen is restored after characterization of the patient'"'"'s supraventricular rhythm or if characterization of the patient'"'"'s supraventricular rhythm is unsuccessful.

57. The method of claim 56, wherein tracking, rate responsiveness, and AV delay are abruptly restored to the normal pacing regimen.

58. The method of claim 56, wherein the pacing rate is gradually restored to the normal pacing regimen if rate smoothing is on and the pacing rate is abruptly restored to the normal pacing regimen if rate smoothing is off.

59. The method of claim 56, further comprising extending the postventricular atrial refractory period for a predetermined number of beats after the normal pacing regimen is restored.

60. The method of claim 59, wherein the predetermined number of beats is one beat.

61. A body implantable system for characterizing a patient'"'"'s supraventricular rhythm while the heart is being paced, comprising:

a lead system, the lead system comprising electrodes extending into a heart;

a detector system, coupled to the lead system, that detects rate channel signals and shock channel signals sensed by the lead system electrodes;

a control system coupled to the detector system, wherein the control system controls pacing of the patient'"'"'s heart, modifies one or more pacing parameters, and characterizes the patient'"'"'s supraventricular rhythm during a time in which the pacing parameters are modified.

62. The system of claim 61, wherein the detector system comprises at least one rate channel amplifier coupled to the control system, the control system modifying a gain of the at least one rate channel amplifier during characterization of the patient'"'"'s normal cardiac rhythm.

63. The system of claim 62, wherein the gain of the at least one rate channel amplifier is not modified in response to a ventricular paced beat unless N consecutive pacing pulses have occurred since a last intrinsic ventricular beat.

64. The system of claim 63, wherein N=1, 2 or 3.

65. The system of claim 61, wherein the control system controls resuming a pacing regimen following characterization of the patient'"'"'s supraventricular rhythm.

66. The system of claim 61, further comprising a rate responsive subsystem coupled to the control system for providing an output indicating a patient'"'"'s physiologic heart rate demand.

67. The system of claim 66, wherein the rate responsive subsystem further comprises one or more rate response sensors for sensing the patient'"'"'s physiologic heart rate demand.

68. The system of claim 66, wherein the rate responsive subsystem further comprises one or more types of rate response sensors for sensing the patient'"'"'s physiologic heart rate demand.

69. The system of claim 66, wherein the control system further comprises a rate responsive control system coupled to the rate responsive subsystem and a pacemaker for providing rate responsive pacing.

70. The system of claim 61, wherein the control system characterizes the patient'"'"'s supraventricular rhythm upon command.

71. The system of claim 61, wherein the control system characterizes the patient'"'"'s supraventricular rhythm automatically.

72. The system of claim 61, wherein the control system modifies one or more pacing parameters including pacemaker mode, AV delay, pacing rate, modifying a pacing interval, and suspending one or more pacemaker functions.

73. The system of claim 72, wherein the control system changes a pacemaker mode from a tracking, rate responsive mode to a non-tracking, non-rate responsive mode.

74. The system of claim 72, wherein the control system lengthens the AV delay by a predetermined delay interval.

75. The system of claim 74, wherein the predetermined delay interval is a safe AV delay for tachycardia sensing.

76. The system of claim 75, wherein the longest safe AV delay for tachycardia sensing is characterized by:

longest safe AV delay=lower rate interval−

slowest VT interval−

safety window.

77. The system of claim 72, wherein the control system decreases the pacing rate to a lower rate limit.

78. The system of claim 72, wherein the control system suspends one or more of rate smoothing up, rate smoothing down, dynamic AV delay, dynamic postventricular atrial refractory period, AV search interval, rate hysteresis offset, sensed AV delay offset, postventricular atrial refractory period after premature ventricular complex, dynamic ventricular refractory period, and ventricular rate regularization.

79. The system of claim 61, wherein the control system characterizes the patient'"'"'s supraventricular rhythm when a V rate of the patient is in a lowest quartile of a preceding 24 hour period.

80. The system of claim 61, wherein the control system terminates characterization of the patient'"'"'s supraventricular rhythm upon sensing patient physiologic heart rate demand.

81. The system of claim 61, wherein the control system:

provides a plurality of templates;

selectively updates the plurality of templates using a plurality of qualified beats; and

characterizes the patient'"'"'s supraventricular rhythm using a particular template of the plurality of updated templates.

82. The system of claim 81, wherein the control system retrieves at least a first template from memory and forms one or more other templates using the qualified beats.

83. The system of claim 81, wherein the control system determines if the rate channel signals and the shock channel signals detected by the detector system meet predetermined criteria for qualified beats.

84. The system of claim 81, wherein the control system:

determines if a qualified beat is correlated to a selected template of the plurality of templates; and

updates the selected template with the qualified beat if the qualified beat is correlated to the selected template.

85. The system of claim 84, wherein the control system:

determines a fiducial point of the qualified beat and a fiducial point of the selected template, respectively;

aligns shock channel signals of the qualified beat and the selected template using the fiducial point of the qualified beat and the fiducial point of the selected template;

identifies a plurality of features of shock channel signals of the qualified beat and the selected template; and

uses the plurality of selected features to determine if the qualified beat is correlated to the selected template.

86. A system for characterizing a patient'"'"'s supraventricular rhythm while the heart is being paced, comprising:

means for pacing the patient'"'"'s heart;

means for modifying one or more pacing parameters; and

means for characterizing the patient'"'"'s supraventricular rhythm during a time in which the pacing parameters are modified.

87. The system of claim 86, wherein means for modifying one or more pacing parameters includes one or more of means for modifying a pacemaker mode, means for modifying an AV delay, means for modifying a pacing rate, means for modifying a pacing interval, and means for suspending one or more pacemaker functions.

88. The system of claim 87, wherein means for modifying the AV delay includes means for lengthening the AV delay by a predetermined interval.

89. The system of claim 87, wherein means for suspending one or more pacemaker functions includes means for suspending one or more of rate smoothing up, rate smoothing down, dynamic AV delay, dynamic postventricular atrial refractory period, AV search interval, rate hysteresis offset, sensed AV delay offset, postventricular atrial refractory period after premature ventricular complex, dynamic ventricular refractory period, and ventricular rate regularization.

90. A system for characterizing a patient'"'"'s supraventricular rhythm while the heart is being paced, comprising:

pacing the patient'"'"'s heart;

means for modifying a pacing mode;

means for decreasing a pacing rate;

means for increasing an AV delay interval; and

means for characterizing the patient'"'"'s supraventricular rhythm during a time in which pacing is modified.

91. The system of claim 90, wherein means for modifying the pacing mode further comprises means for modifying the pacing mode from a tracking, rate responsive pacing mode to a non-tracking, non-rate responsive mode.
