1. An implantable cardiac rhythm management device, comprising:
- an exciter for generating an excitation current between excitation electrodes;
voltage sense electrodes for generating an impedance signal corresponding to cardiac volume when the excitation current is applied; and
circuitry for processing the impedance signal to detect oscillations in cardiac volume during successive heart beats and to detect volumetric alternans when the measured oscillations exceed a specified magnitude for a specified number of beats.

means for detecting an oscillation in cardiac rhythm when alternations in a measured cardiac parameter during successive heart beats exceed a specified magnitude for a specified number of beats;
first and second pacing channels;
a controller programmed to deliver pacing therapy; and
wherein the controller is programmed to initiate pre-excitation pacing therapy when an oscillation in cardiac rhythm is detected.
