Abstract
A medical needle set for visualized tissue ablation within a subject'"'"'s body includes a cannula and components configured for inclusion in the cannula, including a trocar for occlusion of the cannula lumen during needle placement, and a visualizing ablation probe used for simultaneous endoscopic viewing and ablation of tissue sites with a laser beam. The cannula can include a tissue-gripping surface for stabilization of the needle set on the target tissue. A surgical system for tissue ablation includes a visualizing ablation needle set operably connected to an endoscope and a laser. A surgical procedure using this system permits simultaneous visualization and ablation of tissues, including those of the facet joints of the spine.

12. A tissue ablation method comprising the steps of:

(a) providing a visualizing ablation needle set, said needle set comprising a cannula comprising a hollow shaft defining a longitudinal lumen, said shaft having a proximal segment and a distal segment, a trocar configured for inclusion in said longitudinal lumen, and a visualizing ablation probe configured for inclusion in said longitudinal lumen, said probe comprising an obturator configured to be operably connected both to a laser energy delivery device and also to an endoscope. (b) inserting said trocar into said longitudinal lumen of said cannula and positioning said cannula with trocar proximate to a tissue site;
(c) removing said trocar from said longitudinal lumen of said cannula and inserting into said longitudinal lumen of said cannula said visualizing ablation probe operably connected both to a laser energy delivery device and also to an endoscope;
(d) viewing said tissue site with said endoscope both to verify said positioning of said cannula and also to visualize said tissue site; and
,(e) ablating tissue at said tissue site with said laser energy delivery device while viewing said ablation with said endoscope.

13. A facet joint syndrome treatment method comprising the steps of:

(a) viewing at least one facet joint by fluoroscopy and positioning a cannula with inserted trocar proximate to said facet joint;
(b) removing said trocar from said cannula and inserting into said cannula a visualizing ablation probe operably connected both to a laser energy delivery device and also to an endoscope;
(c) viewing said facet joint with said endoscope to verify said positioning and to visualize said facet joint; and
,(d) ablating tissue at said facet joint with said laser energy source while viewing said ablation with said endoscope.
