Abstract

Methods of using a guide catheter employ a retractable inner guide movably disposed within an open lumen of a splittable outer guide. The outer guide includes a longitudinal pre-stress line extending from distal to proximal ends. The inner guide includes an open lumen adapted to receive a payload. The inner guide is translatable and rotatable relative to the outer guide. The guide catheter further includes a proximally attached guide handle which is separable into at least two sections. Separation of the guide handle splits the proximal end of the outer guide. Proximal outer guide retraction continues the splitting of the outer guide along the longitudinal pre-stress line.

Claims

1. A method of inserting a payload into a coronary sinus of a patient'"'"'s heart, comprising:

providing a guide catheter comprising;

an outer guide having an open lumen and a longitudinal pre-stress line extending between a distal end and a proximal end of the outer guide;

an inner guide having an open lumen, the inner guide movably disposed within the open lumen of the outer guide such that the inner guide can rotate axially and translate longitudinally relative to the outer guide; and

a guide handle having a distal end connected to the proximal end of the outer guide, the guide handle separable into at least two sections such that guide handle separation splits the outer guide along the longitudinal pre-stress line at the proximal end of the outer guide, the outer guide further splitting along the longitudinal pre-stress line upon outer guide retraction in a proximal direction relative to the inner guide;

inserting a distal end of the guide catheter into the patient'"'"'s right atrium via an access vessel;

axially rotating and longitudinally translating the inner guide relative to the outer guide to direct a distal end of the inner guide for locating and cannulating the patient'"'"'s coronary sinus;

inserting the payload through the proximal end of the inner guide to insert the payload into the patient'"'"'s coronary sinus;

removing the outer guide by separating the guide handle into at least two sections to split the outer guide along the longitudinal pre-stress line; and

further splitting the outer guide along the longitudinal pre-stress line by retracting the outer guide in a proximal direction relative to the inner guide, the inner guide securing the payload against dislodgment during outer guide retraction.

A method according to claim 1, wherein the outer guide further comprises one or more electrodes disposed at the distal end of the outer guide, and at least one conductor connected to the one or more electrodes and extending along the outer guide to a location accessible at a proximal end of the guide catheter, the method further comprising performing one or both of sensing cardiac signals and transferring energy to cardiac tissue using the one or more electrodes.

A method according to claim 1, wherein the inner guide further comprises one or more electrodes disposed at the distal end of the inner guide, and at least one conductor connected to the one or more electrodes and extending along the inner guide to a location accessible at a proximal end of the guide catheter, the method further comprising performing one or both of sensing cardiac signals and transferring energy to cardiac tissue using the one or more electrodes.

A method according to claim 1, wherein the guide catheter further comprises an occlusion balloon disposed at the distal end of the outer guide and an inflation lumen accessible from a proximal end of the guide catheter, the method further comprising selectively pressurizing and depressurizing the inflation lumen to respectively inflate and deflate the occlusion balloon.

A method according to claim 1, wherein the guide catheter further comprises an occlusion balloon disposed at the distal end of the inner guide and an inflation lumen accessible from a proximal end of the guide catheter, the method further comprising selectively pressurizing and depressurizing the inflation lumen to respectively inflate and deflate the occlusion balloon.

A method according to claim 1, wherein the guide catheter further comprises a steering tendon disposed within the outer guide, a distal end of the steering tendon connected to the distal end of the outer guide, such that the steering tendon deflects the distal end of the outer guide upon application of a tensile force to a proximal end of the steering tendon, and wherein locating and cannulating the patient'"'"'s coronary sinus further comprises applying a tensile force to the proximal end of the steering tendon to direct the distal end of the guide catheter.

7. A method according to claim 1, wherein the payload comprises a pacing lead.

8. A method of inserting a payload into a coronary sinus of a patient'"'"'s heart, comprising:

providing a guide catheter comprising;

an outer guide having an open lumen and a longitudinal pre-stress line extending between a distal end and a proximal end of the outer guide;

an inner guide having an open lumen, the inner guide movably disposed within the open lumen of the outer guide such that the inner guide can rotate axially and translate longitudinally relative to the outer guide; and

a guide handle having a distal end connected to the proximal end of the outer guide, the guide handle separable into at least two sections such that guide handle separation splits the outer guide along the longitudinal pre-stress line at the proximal end of the outer guide, the outer guide further splitting along the longitudinal pre-stress line upon outer guide retraction in a proximal direction relative to the inner guide;

inserting a distal end of the guide catheter into the patient'"'"'s right atrium via an access vessel;

axially rotating and longitudinally translating the inner guide relative to the outer guide to direct a distal end of the inner guide for locating and cannulating the patient'"'"'s coronary sinus;

distally advancing the outer guide over the inner guide to seat the outer guide in the patient'"'"'s coronary sinus;

proximally retracting the inner guide to remove the inner guide from the outer guide;

inserting the payload through the proximal end of the outer guide to insert the payload into the patient'"'"'s coronary sinus;

removing the outer guide by separating the guide handle into at least two sections to split the outer guide along the longitudinal pre-stress line; and

further splitting the outer guide along the longitudinal pre-stress line by retracting the outer guide in a proximal direction relative to the payload.

9. A method according to claim 8, wherein the outer guide further comprises one or more electrodes disposed at the distal end of the outer guide, and at least one conductor connected to the one or more electrodes and extending along the outer guide to a location accessible at a proximal end of the guide catheter, the method further comprising performing one or both of sensing cardiac signals and transferring energy to cardiac tissue using the one or more electrodes.

10. A method according to claim 8, wherein the inner guide further comprises one or more electrodes disposed at the distal end of the inner guide, and at least one conductor connected to the one or more electrodes and extending along the inner guide to a location accessible at a proximal end of the guide catheter, the method further comprising performing one or both of sensing cardiac signals and transferring energy to cardiac tissue using the one or more electrodes.

A method according to claim 8, wherein the guide catheter further comprises an occlusion balloon disposed at the distal end of the outer guide and an inflation lumen accessible from a proximal end of the guide catheter, the method further comprising selectively pressurizing and depressurizing the inflation lumen to respectively inflate and deflate the occlusion balloon.

12. A method according to claim 8, wherein the guide catheter further comprises an occlusion balloon disposed at the distal end of the inner guide and an inflation lumen accessible from a proximal end of the guide catheter, the method further comprising selectively pressurizing and depressurizing the inflation lumen to respectively inflate and deflate the occlusion balloon.

13. A method according to claim 8, wherein the guide catheter further comprises a steering tendon disposed within the outer guide, a distal end of the steering tendon connected to the distal end of the outer guide, such that the steering tendon deflects the distal end of the outer guide upon application of a tensile force to a proximal end of the steering tendon, and wherein locating and cannulating the patient'"'"'s coronary sinus further comprises applying a tensile force to the proximal end of the steering tendon to direct the distal end of the guide catheter.

14. A method according to claim 8, wherein the payload comprises a pacing lead.

15. A method of inserting a payload into a coronary sinus of a patient'"'"'s heart, comprising:

providing a guide catheter comprising;

an outer guide having an open lumen and a longitudinal pre-stress line extending between a distal end and a proximal end of the outer guide;

an inner guide having an open lumen, the inner guide movably disposed within the open lumen of the outer guide such that the inner guide is displaceable relative to the outer guide, the inner guide having a stiffness less than that of the outer guide;

a guide handle having a distal end connected to the proximal end of the outer guide, the guide handle separable into at least two sections; and

a steering member disposed along the outer guide, a distal end of the steering member connected at a distal end of the outer guide, and a proximal end of the steering member accessible at the proximal end of the outer guide, whereby application of a tensile force to the proximal end of the steering member effects deflection of the distal end of the outer guide;

inserting a distal end of the guide catheter into the patient'"'"'s right atrium via an access vessel;

manipulating the inner guide relative to the outer guide and applying tensile force to the proximal end of the steering member to direct a distal end of the inner guide into the patient'"'"'s coronary sinus;

inserting the payload through the proximal end of the inner guide to insert the payload into the patient'"'"'s coronary sinus;

removing the outer guide by separating the guide handle into at least two sections to split the outer guide along the longitudinal pre-stress line; and

further splitting the outer guide along the longitudinal pre-stress line by retracting the outer guide in a proximal direction relative to the inner guide, the inner guide securing the payload against dislodgment during outer guide retraction.

16. A method according to claim 15, wherein the outer guide further comprises one or more electrodes disposed at the distal end of the outer guide, and at least one conductor connected to the one or more electrodes and extending along the outer guide to a location accessible at a proximal end of the guide catheter, the method further comprising performing one or both of sensing cardiac signals and transferring energy to cardiac tissue using the one or more electrodes.

17. A method according to claim 15, wherein the inner guide further comprises one or more electrodes disposed at the distal end of the inner guide, and at least one conductor connected to the one or more electrodes and extending along the inner guide to a location accessible at a proximal end of the guide catheter, the method further comprising performing one or both of sensing cardiac signals and transferring energy to cardiac tissue using the one or more electrodes.

18. A method according to claim 15, wherein the guide catheter further comprises an occlusion balloon disposed at the distal end of the outer guide and an inflation lumen accessible from a proximal end of the guide catheter, the method further comprising selectively pressurizing and depressurizing the inflation lumen to respectively inflate and deflate the occlusion balloon.

19. A method according to claim 15, wherein the guide catheter further comprises an occlusion balloon disposed at the distal end of the inner guide and an inflation lumen accessible from a proximal end of the guide catheter, the method further comprising selectively pressurizing and depressurizing the inflation lumen to respectively inflate and deflate the occlusion balloon.

20. A method according to claim 15, wherein the payload comprises a pacing lead.

21. A method of inserting a payload into a coronary sinus of a patient'"'"'s heart, comprising:

providing a guide catheter comprising;

an outer guide having an open lumen and a longitudinal pre-stress line extending between a distal end and a proximal end of the outer guide;

an inner guide having an open lumen, the inner guide movably disposed within the open lumen of the outer guide such that the inner guide can rotate axially and translate longitudinally relative to the outer guide, the inner guide having a stiffness less than that of the outer guide;

a guide handle having a distal end connected to the proximal end of the outer guide, the guide handle separable into at least two sections; and

a steering member disposed along the outer guide, a distal end of the steering member connected at a distal end of the outer guide, and a proximal end of the steering member accessible at the proximal end of the outer guide, whereby application of a tensile force to the proximal end of the steering member effects deflection of the distal end of the outer guide;

inserting a distal end of the guide catheter into the patient'"'"'s right atrium via an access vessel;

manipulating the inner guide relative to the outer guide and applying tensile force to the proximal end of the steering member to direct a distal end of the inner guide into the patient'"'"'s coronary sinus;

distally advancing the outer guide over the inner guide to seat the outer guide in the patient'"'"'s coronary sinus;

proximally retracting the inner guide to remove the inner guide from the outer guide;

inserting the payload through the proximal end of the outer guide and into the patient'"'"'s coronary sinus;

removing the outer guide by separating the guide handle into at least two sections to split the outer guide along the longitudinal pre-stress line; and

further splitting the outer guide along the longitudinal pre-stress line by retracting the outer guide in a proximal direction relative to the payload.

22. A method according to claim 21, wherein the outer guide further comprises one or more electrodes disposed at the distal end of the outer guide, and at least one conductor connected to the one or more electrodes and extending along the outer guide to a location accessible at a proximal end of the guide catheter, the method further comprising performing one or both of sensing cardiac signals and transferring energy to cardiac tissue using the one or more electrodes.

23. A method according to claim 21, wherein the inner guide further comprises one or more electrodes disposed at the distal end of the inner guide, and at least one conductor connected to the one or more electrodes and extending along the inner guide to a location accessible at a proximal end of the guide catheter, the method further comprising performing one or both of sensing cardiac signals and transferring energy to cardiac tissue using the one or more electrodes.

24. A method according to claim 21, wherein the guide catheter further comprises an occlusion balloon disposed at the distal end of the outer guide and an inflation lumen accessible from a proximal end of the guide catheter, the method further comprising selectively pressurizing and depressurizing the inflation lumen to respectively inflate and deflate the occlusion balloon.

25. A method according to claim 21, wherein the guide catheter further comprises an occlusion balloon disposed at the distal end of the inner guide and an inflation lumen accessible from a proximal end of the guide catheter, the method further comprising selectively pressurizing and depressurizing the inflation lumen to respectively inflate and deflate the occlusion balloon.

26. A method according to claim 21, wherein the payload comprises a pacing lead.
