Abstract

One aspect of the present subject matter relates to an implantable medical system. An embodiment of the system includes a baroreflex stimulator and a cardiac stimulator. The baroreflex stimulator applies a baroreflex stimulation signal through an electrode and provides a signal indicating that the baroreflex signal is being applied. The cardiac stimulator applies a pacing therapy with a pacing rate, receives the signal from the baroreflex stimulator, and increases the pacing rate of the pacing therapy to compensate for cardiac output compromised by applying the baroreflex stimulation signal. Other aspects are provided herein.

Claims

1. An implantable medical system, including:

a baroreflex stimulator to apply a baroreflex stimulation signal through an electrode and provide a signal indicating that the baroreflex signal is being applied; and

a cardiac stimulator to apply a pacing therapy with a pacing rate, receive the signal from the baroreflex stimulator, and increase the pacing rate of the pacing therapy to compensate for cardiac output compromised by applying the baroreflex stimulation signal.

The system of claim 1, further comprising an implantable neural stimulator (NS) device and an implantable cardiac stimulator device, the implantable NS device including the baroreflex stimulator, the implantable cardiac stimulator device including the cardiac stimulator, each of the implantable NS device and the implantable cardiac stimulator device including a transceiver for wirelessly communicating with each other.

3. The system of claim 1, further comprising a single implantable housing including both the baroreflex stimulator and the cardiac stimulator.

The system of claim 1, wherein the baroreflex stimulator further comprises a lead adapted to be electrically connected to the pulse generator and to be fed through a right ventricle and into a pulmonary artery, the lead including the electrode to be positioned in the pulmonary artery and to deliver the baroreflex stimulation signal to a baroreceptor site in the pulmonary artery.

The system of claim 1, wherein the baroreflex stimulator further comprises a lead adapted to be electrically connected to the pulse generator and to be fed into a left ventricle, the lead including the electrode to be positioned to deliver the baroreflex stimulation signal to a baroreceptor site in the left ventricle.

The system of claim 1, wherein the baroreflex stimulator further comprises a lead adapted to be electrically connected to the pulse generator and to be fed into a right ventricle, the lead including the electrode to be positioned to deliver the baroreflex stimulation signal to a baroreceptor site in the right ventricle.

The system of claim 1, wherein the baroreflex stimulator further comprises a left ventricle lead adapted to be electrically connected to the pulse generator and a right ventricle lead adapted to be electrically connected to the pulse generator, the left ventricle lead being adapted to be fed into a left ventricle and the right ventricle lead being adapted to be fed into a right ventricle, the left ventricle lead including a first electrode to be positioned to deliver the baroreflex stimulation signal to a baroreceptor site in the left ventricle, and the right ventricle lead including a second electrode to be positioned to deliver the baroreflex stimulation signal to a baroreceptor site in the right ventricle.

8. The system of claim 1, wherein the baroreflex stimulator further comprises a lead adapted to be electrically connected to the pulse generator and to deliver the baroreflex stimulation to an aortic nerve.

9. The system of claim 1, wherein the baroreflex stimulator further comprises a lead adapted to be electrically connected to the pulse generator and to deliver the baroreflex stimulation to a right atrium.

10. The system of claim 1, wherein the baroreflex stimulator further comprises at least one lead adapted to be electrically connected to the pulse generator and to deliver the baroreflex stimulation to a combination of two or more baroreceptor sites selected from a group of baroreceptor sites consisting of a baroreceptor site in a right atrium, a baroreceptor site in a left ventricle, and a baroreceptor site in a right ventricle.

11. The system of claim 1, wherein the baroreflex stimulator further comprises a lead adapted to be electrically connected to the pulse generator and to deliver the baroreflex stimulation to a carotid sinus nerve.

12. The system of claim 1, wherein the baroreflex stimulator further comprises a lead adapted to be electrically connected to the pulse generator and to deliver the baroreflex stimulation to a vagus nerve.

35. The system of claim 1, wherein the baroreflex signal is adapted to transvascularly stimulate an afferent nerve through an intravascularly-fed electrode positioned proximate to the afferent nerve.

36. The system of claim 1, wherein the baroreflex signal is adapted to stimulate an afferent nerve using a nerve cuff electrode.

13. An implantable medical system, comprising:

means for applying baroreflex stimulation to a pressoreceptive region to lower systemic blood pressure;

means for applying cardiac stimulation with a pacing rate to a heart; and

means for increasing the pacing rate of the cardiac stimulation to compensate for the applied baroreflex stimulation.

14. The system of claim 13, wherein the means for increasing the pacing rate of the cardiac stimulation when baroreflex stimulation is applied includes means for beginning pacing.

15. The system of claim 13, further comprising means for communicating between an implantable neural stimulator (NS) device and an implantable cardiac stimulator.

16. The system of claim 15, wherein the means for communicating between the implantable NS device and the implantable cardiac stimulator includes means for wirelessly communicating between the implantable NS device and the implantable cardiac stimulator.

17. A method for operating an implantable medical system having a cardiac stimulator and a baroreflex stimulator, comprising:

implementing a pacing therapy with a programmed pacing rate using the cardiac stimulator when the baroreflex stimulator does not apply baroreflex stimulation; and

implementing the pacing therapy with a higher pacing rate using the cardiac stimulator to maintain a desired cardiac output when the baroreflex stimulator applies baroreflex stimulation.

18. The method of claim 17, further comprising transmitting a communication signal between the baroreflex stimulator and the cardiac stimulator to indicate when the baroreflex stimulator is applying the baroreflex stimulation.

19. The method of claim 17, wherein the cardiac stimulator and the baroreflex stimulator are incorporated in one implantable medical device, and transmitting the communication signal includes transmitting the communication signal within the implantable medical device.

20. The method of claim 17, wherein the cardiac stimulator is incorporated in an implantable cardiac stimulator and the baroreflex stimulator is incorporated in an implantable neural stimulator (NS) device, and transmitting the communication signal includes transmitting the communication signal between the implantable NS device and the implantable cardiac stimulator.

21. The method of claim 17, further comprising delivering baroreflex stimulation through a lead adapted to be fed through a right ventricle and pulmonary valve into a pulmonary artery.

22. A method for operating an implantable medical system having a cardiac stimulator and a baroreflex stimulator, comprising:

applying baroreflex stimulation using the baroreflex stimulator;

determining if the cardiac output is adequate; and

if the cardiac output is not adequate, increasing a pacing rate using the cardiac stimulator to improve the cardiac output.

23. The method of claim 22, further comprising delivering baroreflex stimulation through a lead adapted to be fed through a right ventricle and pulmonary valve into a pulmonary artery.

24. A method, comprising:

applying baroreflex stimulation; and

applying cardiac stimulation with an increased pacing rate to compensate for a lower cardiac output when the baroreflex stimulation is applied.

25. The method of claim 24, wherein applying baroreflex stimulation includes applying baroreflex stimulation to a baroreceptor site in a pulmonary artery.

26. The method of claim 24, wherein applying baroreflex stimulation includes applying baroreflex stimulation to a baroreceptor site in a right atrium.

27. The method of claim 24, wherein applying baroreflex stimulation includes applying baroreflex stimulation to a baroreceptor site in a left ventricle.

28. The method of claim 24, wherein applying baroreflex stimulation includes applying baroreflex stimulation to a baroreceptor site in a right ventricle.

29. The method of claim 24, wherein applying baroreflex stimulation includes applying baroreflex stimulation to an afferent nerve.

30. The method of claim 24, wherein applying baroreflex stimulation includes applying baroreflex stimulation to a baroreflex neural target in a cardiac fat pad.

31. The method of claim 24, wherein applying baroreflex stimulation includes applying transvascular stimulation to stimulate a baroreflex.

32. The method of claim 31, wherein applying transvascular stimulation to stimulate a baroreflex includes applying stimulation to an afferent nerve using an electrode position in a vessel proximate to the afferent nerve.

33. The method of claim 31, wherein applying transvascular stimulation to stimulate a baroreflex includes applying stimulation to a baroreflex neural target in a cardiac fat pad using an electrode position in a vessel proximate to the cardiac fat pad.

34. The method of claim 24, wherein applying baroreflex stimulation includes applying baroreflex stimulation to an afferent nerve using a cuff electrode.
