1. A voice navigation, command and data entry input system for a decision support and electronic record system, comprising:

  • a core voice recognition engine, including a phoneme identification mechanism for identifying phonemes appearing in a voice input signal and generating corresponding phoneme identifiers, and a word identification mechanism for identifying and providing words corresponding to the phoneme identifications as an output to the decision support and electronic record system, an individual phoneme identification library associated with the phoneme identification mechanism for storing and providing phoneme identifications corresponding to phonemes characteristic of a voice input signal of a corresponding user, and word libraries associated with the word identification mechanism for identifying and providing as an output words corresponding to the phoneme identifications wherein the words include at least one of a keystroke, a combination of keystrokes, a sequence of keystrokes, a word, a phrase, a character, a letter, a number, a symbol, a command and an instruction, including a field of use library for storing and providing words specific to the field of use context of the decision support and electronic record system, a command/navigation library for storing and providing keystroke sequences pertaining to navigation through and control of operations of the decision support and electronic record system, and a common vocabulary library for storing and providing common words employed in operations of the decision support and electronic record system.
