Abstract

A pacing protocol is provided that reduces or minimizes ventricular pacing in favor of intrinsic conduction. When operating in a mode that provides ventricular pacing, a series of conduction checks are performed to determine if intrinsic conduction has returned. These conduction checks occur according to a predetermined pattern that generally includes longer intervals between subsequent attempts. The AV interval provided for dual chamber based pacing is modulated and generally moves from a larger value to a nominal value as the interval between unsuccessful conduction checks increases.

Claims

1. An implantable medical device (IMD) comprising:

a processing unit for controlling the IMD and including a ventricular pacing protocol module for selectively operating the IMD in an atrial based pacing mode or a dual chamber based pacing mode;

a pulse generator for selectively providing cardiac pacing under the control of the processing unit;

an intrinsic conduction checking module communicatively coupled with the processing unit and causing the processing unit to perform conduction checks when operating in the dual chamber based pacing mode, the conduction checks occurring according to a predetermined pattern; and

an AV interval module for varying a duration of an AV interval when operating in the dual chamber based pacing mode and based upon the predetermined pattern.

2. The IMD of claim 1, wherein the AV interval tapers from a first duration to a nominal duration, wherein the first duration is greater than the nominal duration.

3. The IMD of claim 1, wherein the AV interval decreases as a step function from a first duration to a nominal duration, wherein the first duration is greater than the nominal duration.

4. The IMD of claim 1, wherein the intrinsic conduction checking module includes a maximum interval and the AV interval is set to a nominal value when the maximum interval is reached.

5. The IMD of claim of claim 1, wherein the AV interval is inversely proportional to a duration of an interval between conduction checks in the predetermined pattern.

6. The IMD of claim 1, further comprising a comparison module that determines a percentage of time that the IMD operates in the dual chamber based pacing mode and wherein the AV interval module selects an AV interval based upon the percentage of time.

7. An implantable medical device (IMD) comprising:

means for selective providing cardiac pacing;

means for providing the cardiac pacing according to a ventricular pacing protocol;

means for performing conduction checks during operation of the ventricular pacing protocol in a dual chamber based pacing mode; and

means for varying the AV interval while operating in the dual chamber based pacing mode.

8. The IMD of claim 7, wherein the means for varying taper the duration of the AV interval with unsuccessful conduction checks.

9. The IMD of claim 7, wherein the means for varying adjust the duration of the AV interval as a step function with unsuccessful conduction check.

10. The IMD of claim 7, wherein the means for varying further include means for determining a percentage of time that the IMD operates in the dual chamber based pacing mode and determines and AV interval based at least in part upon the percentage.

11. The IMD of claims 7, wherein the means for performing conduction checks follow a predetermined pattern including a maximum conduction check interval and the means for varying the AV interval set the AV interval to a nominal value when the maximum conduction check interval is reached.

12. A method comprising:

operating an implantable medical device (IMD) having cardiac pacing capabilities in an atrial based pacing mode if intrinsic conduction is present and in a dual chamber pacing mode according to a ventricular pacing protocol if intrinsic conduction is not present;

performing a conduction check at predetermined intervals while operating in the dual chamber pacing mode, wherein the predetermined intervals vary according to a predetermined pattern provided for sequential unsuccessful conduction checks and the predetermined pattern includes an initial minimum interval and a maximum interval;

selecting a subsequent interval from the predetermined pattern with each unsuccessful conduction check, until the maximum interval is reached;

determining an AV interval for the dual chamber pacing mode, wherein the AV interval varies from a first duration to second duration as the predetermined pattern progresses with the first duration being greater than the second duration.

13. The method of claim 12, wherein the AV interval is varied by tapering the duration.

14. The method of claim 12, wherein the AV interval is varied according to a step function.

15. The method of claim 12, further comprising:

determining a percentage time that the IMD operates in the dual chamber based pacing mode and the AV interval is determined at least in part based upon the percentage.

16. The method of claim 12, wherein the AV interval is inversely proportional to the predetermined interval.

17. The method of claim 12 wherein the AV interval is set a nominal value when the conduction check interval is at the maximum.

18. The method of claim 17, further comprising increasing the AV interval from the nominal value for a predetermined number of cardiac cycles prior to a conduction check at the maximum interval.
