Abstract

A system for treatment of a vessel bifurcation and its method of use comprises a catheter having a catheter shaft, a portion of which is characterized as an axle about which a rotatable assembly is disposed and rotatable thereabout. The system employs a series of lumens, openings and/or ports to provide a balloon mounted on the rotatable assembly with fluid communication to the catheter shaft.

Claims

1. A system for treatment of a vessel bifurcation, the system comprising:

a catheter, the catheter having a catheter shaft, the catheter shaft defining an inflation lumen therethrough, a portion of the catheter shaft characterized as an axle portion, the axle portion defining a fluid opening, the fluid opening being in fluid communication with the inflation lumen; and

a rotatable assembly, the rotatable assembly comprising a barrel, the barrel defining a substantially hollow longitudinal passage through which the axle portion extends, the rotatable assembly further comprising an expandable medical balloon sealingly disposed about the barrel, the balloon being expandable between a reduced state and an expanded state, wherein in the expanded state the balloon has a diameter greater than that in the reduced state, the barrel further defining at least one fluid port, the at least one fluid port being in fluid communication with the substantially hollow passage and an interior of the balloon.

The system of claim 1 wherein the catheter shaft further comprises a proximal region and a distal region, the axle portion positioned between the proximal region and the distal region, at least a portion of the proximal region immediately adjacent to the axle portion having a diameter greater than that of the substantially hollow longitudinal passage of the barrel and at least a portion of the distal region immediately adjacent to the axle portion having a diameter greater than that of the substantially hollow longitudinal passage of the barrel.

3. The system of claim 2 wherein the proximal region of the catheter shaft and the barrel define a proximal gap therebetween and wherein the distal region of the catheter shaft and the barrel define a distal gap therebetween.

The system of claim 3 further comprising a first sealing mechanism and a second sealing mechanism, the first sealing mechanism positioned within the proximal gap between the proximal region and the barrel, the second sealing mechanism positioned within the distal gap between the distal region and the barrel.

5. The system of claim 3 wherein the balloon comprises a proximal overlap region and a distal overlap region:

the proximal overlap region extending from the rotatable assembly to radially overlap a portion of the proximal region of the shaft, during expansion of the balloon the proximal overlap region is sealingly engaged to the portion of the proximal region of the shaft thereby forming a fluid tight seal over the proximal gap between the barrel and the proximal region of the shaft;

the distal overlap region extending from the rotatable assembly to radially overlap a portion of the distal region of the shaft, during expansion of the balloon the distal overlap region is sealingly engaged to the portion of the distal region of the shaft thereby forming a fluid tight seal over the distal gap between the barrel and the distal region of the shaft.

6. The system of claim 5 wherein the balloon further comprises a body region extending between the proximal overlap region and the distal overlap region, at least a portion of the body region defining a stent receiving region.

The system of claim 6 further comprising a stent, the stent having a reduced configuration and an expanded configuration, in the expanded configuration the stent having a diameter greater than that in the reduced configuration, in the reduced configuration the stent being disposed about the stent receiving region.

8. The system of claim 6 wherein the rotatable assembly further comprises a secondary guidewire housing for passage of a secondary guidewire therethrough, the secondary guidewire housing engaged to a portion of the balloon.

9. The system of claim 8 wherein at least a portion of the secondary guidewire housing is bio-absorbable.

10. The system of claim 8 wherein at least a portion of the secondary guidewire housing is frangible.

11. The system of claim 8 wherein at least a portion of the secondary guidewire housing is defined by at least a portion of the sheath or balloon.

The system of claim 8 further comprising a stent, the stent having a reduced configuration and an expanded configuration, in the expanded configuration the stent having a diameter greater than that in the reduced configuration, in the reduced configuration the stent being disposed about the stent receiving region, the stent having a plurality of interconnected stent members, wherein adjacent members define a plurality of stent openings, at least a portion of the secondary guidewire housing underlying at least a portion of the stent, the secondary guidewire extending under the at least a portion of the stent and passing outward from the stent through one of the stent openings.

13. The system of claim 12 wherein the catheter shaft further defines a primary guidewire lumen for passage of a primary guidewire therethrough.

14. A method for treating a vessel bifurcation comprising the steps of:

providing a treatment system the treatment system comprising;

a catheter, the catheter having a catheter shaft, the catheter shaft defining an inflation lumen, a portion of the catheter shaft being characterized as an axle portion, the axle portion defining a fluid opening in fluid communication with the inflation lumen;

a rotatable assembly, the rotatable assembly disposed about the axle portion of the catheter shaft and being substantially freely rotatable thereabout, the rotatable assembly comprising an expandable balloon which has an interior in fluid communication with the inflation lumen, the rotatable assembly further comprising a secondary guidewire housing for passage of a secondary guidewire therethrough; and

a stent, the stent having an unexpanded configuration and an expanded configuration wherein in the expanded configuration the stent has a diameter greater than that in the reduced configuration, in the reduced configuration the stent being disposed about a portion of the balloon, at least a portion of the secondary guidewire housing underlying at least a portion of the stent, the secondary guidewire extending under the at least a portion of the stent and passing outward from the stent through one of the stent openings;

advancing the secondary guidewire into through a primary vessel and into a branch vessel at the bifurcation of the primary vessel and branch vessel;

advancing the system along the secondary guidewire to the bifurcation;

rotating the rotatable assembly such that the one of the stent openings is aligned with the ostium of the branch vessel; and

deploying the stent into the bifurcation.
