Abstract

The present invention involves a health care patient document system and method which enable patients to enter clinical, administrative, and financial information normally completed on a clipboard in a medical office into a computerized system that stores the information. The patient is then enabled to direct the stored information to any health care provider whether the provider is enabled to receive the data electronically or not. For those provider clinicians and organizations not enabled to receive the information electronically, the system completes the paper-based forms already in use within the organization and sends the completed form by facsimile or mail delivery.

Claims

1. A computer server for providing access to patient data records to a plurality of health care providers, said computer server comprising:

a provider database having a plurality of provider patient documents, at least one of the plurality of providers having a patient document distinct from the patient documents of the other of the plurality of providers;

a patient interaction module adapted to obtain patient specific information from a patient relating to information requested by the plurality of provider patient documents; and

a provider interaction module adapted to transmit provider patient documents that include patient specific information relating to a specific patient in a format equivalent to each of said plurality of providers.

2. The computer server of claim 1 wherein said patient interaction module is adapted to obtain at least one of the following types of information:

patient demographic, responsible party, patient insurance, patient allergies, patient medications, patient current medical problem list, patient family history, patient social history, patient vaccination history, patient chief complaint, patient history of present illness, and patient surgical history.

3. The computer server of claim 1 wherein said patient interaction module is adapted to store patient specific information in a database file.

4. The computer server of claim 3 wherein said database file includes fields for at least one of the following types of information:

record identifying, personal identifying, patient insurance/financial, advance directives, patient health status, care documentation, care plan recommendation, and practitioners.

5. The computer server of claim 4 wherein said database file further includes extension fields for containing further information.

6. The computer server of claim 1 wherein said provider interaction module is further adapted to transmit patient scheduling messages with a provider.

7. The computer server of claim 1 wherein said provider interaction module is further adapted to transmit said patient documents by one of facsimile transmission, electronic data interchange transmission, electronic mail transmission, or regular mail delivery.

8. In computer, a method of creating a patient and health care provider document database, said method comprising the steps of:

obtaining a plurality of patient information documents from a plurality of providers;

scanning the plurality of patient information documents and determining the patient information which is identified in the patient information documents;

creating a plurality of patient information template documents adapted to include patient specific information from a patient database; and

creating a patient database including data fields for patient information common to the plurality of patient information documents.

9. The method of claim 8 wherein said step of creating a patient database includes providing fields for at least one of the following types of information:

patient demographic, responsible party, patient insurance, patient allergies, patient medications, patient current medical problem list, patient family history, patient social history, patient vaccination history, patient chief complaint, patient history of present illness, and patient surgical history.

10. The method of claim 8 wherein said step of creating a patient database includes providing fields for at least one of the following types of information:

record identifying, personal identifying, patient insurance/financial, advance directives, patient health status, care documentation, care plan recommendation, and practitioners.

11. The method of claim 10 wherein said step of creating a patient database further includes extension fields for containing further information.

12. The method of claim 8 wherein said step of obtaining a plurality of patient information documents further includes obtaining access to patient scheduling procedures with a provider.

13. The method of claim 8 wherein said step of obtaining a plurality of patient information documents further includes obtaining access to transmit patient documents by one of facsimile transmission, electronic data interchange transmission, electronic mail transmission, or mail delivery.

14. A machine-readable program storage device for storing encoded instructions for a method of providing a patient information document to a health care provider, said method comprising the steps of:

obtaining patient specific information;

identifying the provider requesting the patient information document; and

creating a patient information document corresponding to the identified provider that includes the patient specific information.

15. The machine-readable program storage device of claim 14 wherein the step of creating a patient database includes providing fields for at least one of the following types of information:

patient demographic, responsible party, patient insurance, patient allergies, patient medications, patient current medical problem list, patient family history, patient social history, patient vaccination history, patient chief complaint, patient history of present illness, and patient surgical history.

16. The machine-readable program storage device of claim 14 wherein the step of creating a patient database includes providing fields for at least one of the following types of information:

record identifying, personal identifying, patient insurance/financial, advance directives, patient health status, care documentation, care plan recommendation, and practitioners.

17. The machine-readable program storage device of claim 16 wherein the step of creating a patient database further includes extension fields for containing further information.

18. The machine-readable program storage device of claim 14 wherein the step of obtaining a plurality of patient information documents further includes obtaining access to patient scheduling procedures with a provider.

19. The machine-readable program storage device of claim 14 wherein the step of obtaining a plurality of patient information documents further includes obtaining access to transmit patient documents by one of facsimile transmission, electronic data interchange transmission, electronic mail transmission, or mail delivery.
