- an implantable medical device having a sensor for sensing one or more patient parameters and a transmitter for transmitting data related to the one or more patient parameters;
an appliance external to the implantable medical device and having a receiver to receive the data transmitted from the implantable medical device and a communication interface to transmit information over a communication network; and
an information manager remote from the appliance and configured to receive the information transmitted from the appliance over the communication network, the information manager further including a processor and a context module configured to process the information received and generate context specific reporting information.

An intelligent patient management system collects data from a variety of sources, processes that information, and provides relevant information to an appropriate caregiver in context at the proper time. The system has multiple information sources available from which to gather additional information based upon conclusions drawn or issues identified from the patient data.

18. A method of managing a disease utilizing a patient management system comprising:

collecting physiological information from one or more sensors that sense physiological parameters for a given patient;
transferring the collected information to an information manager remote from the patient;
acquiring available patient data at the information manager;
evaluating the collected information and the acquired patient data and identifying medical issues;
determining if additional information is required to address the identified medical issue;
determining if the additional information is available;
determining whether to acquire the additonal information;
identifying a medical caregiver most relevant to address the medical issue; and
generating context relevant to the identified medical caregiver, formatting and selecting the medical issues and acquired information according to the generated context and making the contextually formatted and selected medical issues and acquired information available to the identified medical caregiver. - View Dependent Claims (19, 20)

A method of utilizing a patient management system for disease management comprising collecting and processing patient state information, determining from the patient state information a relevant caregiver, determining a context relevant to the caregiver, and presenting to the caregiver relevant processed medical information in the determined context.
