Abstract

A system and method for receiving health data from a community of independent sources of health data related to one or more patients, the health data being selected from the set including oral data, written data and/or electronic data; formatting the health data to enable creation of a universal patient record for each of the one or more patients by tagging of the health data to enable extraction of pertinent portions of the health data for coding of health encounters with the one or more patients; and enabling controlled access to the formatted health data via a network connecting the community of independent sources of health data, the network including at least one central server including a central database storing the formatted health data and at least two on-site servers, each on-site server located at one of the independent sources of health data and each on-site server configured to hold a database replicated from the central database.

Claims

1. A method for providing community health data services, the method comprising:

receiving health data from a community of independent sources of health data related to one or more patients, the health data being selected from the set including oral data, written data and/or electronic data;

formatting the health data to enable creation of a universal patient record for each of the one or more patients by tagging of the health data to enable extraction of pertinent portions of the health data for coding of health encounters with the one or more patients; and

enabling controlled access to the formatted health data via a network connecting the community of independent sources of health data, the network including at least one central server including a central database storing the formatted health data and at least two on-site servers, each on-site server located at one of the independent sources of health data and each on-site server configured to hold a database replicated from the central database.

The method of claim 1 wherein the universal patient record is configured as an electronic data interface populated with the formatted health data, the universal patient record applying the formatted health data to include at least one of a summary of patient information, current medications, demographic data, vital data, insurance data, pharmacy data, identification of a primary care physician and current health issues.

3. The method of claim 1 further comprising:

organizing the formatted health data in the central database according to a schema separating the health data into one or more fields for each universal patient record, the schema identifying the pertinent portions of the health data for the coding of the health encounters.

The method of claim 3 wherein the organizing the formatted health data in the central database according to a schema separating the health data into one or more fields for each universal patient record, the schema identifying the pertinent portions of the health data for the coding of the health encounters includes:

providing in the schema that tagging in the health encounters includes tagging a prognosis and/or a diagnosis, the tagging providing a source for one or more history fields and/or one or more summary fields.

The method of claim 3 wherein the organizing the formatted health data in the central database according to a schema separating the health data into one or more fields for each universal patient record, the schema identifying the pertinent portions of the health data for the coding of the health encounters includes:

determining the pertinent portions of the health data according to physician-determined criteria.

The method of claim 5 wherein the organizing the formatted health data in the central database according to a schema separating the health data into one or more fields for each universal patient record, the schema identifying the pertinent portions of the health data for the coding of the health encounters includes:

processing the received health data using a table providing one or more dictionaries of normalized health terms to enable substantially automatic tagging via a nature language processing to produce a standard entry for the populating the one or more fields.

The method of claim 6 wherein the processing the received health data using a table providing one or more dictionaries of normalized health terms to enable substantially automatic tagging via a nature language processing to produce a standard entry for the populating the one or more fields includes:

applying the dictionary of normalized terms to enable a standard entry for same and/or similar received health data via parsing the health data to locate relevant health data.

The method of claim 3 wherein the organizing the formatted health data in the central database according to a schema separating the health data into one or more fields for each universal patient record, the schema identifying the pertinent portions of the health data for the coding of the health encounters includes:

populating one or more fields in the schema with the health data from the community of independent sources of health data, the community of independent sources of health data including one or more of physicians, insurance providers, home health care providers, dental providers, laboratories, pharmacies, imaging providers, and physical therapy providers.

The method of claim 8 wherein the populating one or more fields in the schema with the health data from the community of independent sources of health data, the community of independent sources of health data including one or more of physicians, insurance providers, home health care providers, dental providers, laboratories, pharmacies, imaging providers, and physical therapy providers includes:

determining the pertinent portions of the health data by locating one or more inconsistencies in the health data received from the independent sources of health data, the inconsistencies identifying potential health care issues for a patient.

10. The method of claim 9 wherein the determining the pertinent portions of the health data by locating one or more inconsistencies in the health data received from the independent sources of health data, the inconsistencies identifying potential health care issues for a patient includes:

comparing two or more of the fields in the schema;

applying a metric to the two or more fields, the metric to determine whether the compared fields meet a predetermined parameter of consistency; and

identifying each of the two or more fields according to the predetermined parameter.

11. The method of claim 10 wherein the determining the pertinent portions of the health data by locating one or more inconsistencies in the health data received from the independent sources of health data, the inconsistencies identifying potential health care issues for a patient includes:

determining a most recently altered field among the one or more inconsistencies in the health data; and

tagging the most recently altered field.

12. The method of claim 10 wherein the determining the pertinent portions of the health data by locating one or more inconsistencies in the health data received from the independent sources of health data, the inconsistencies identifying potential health care issues for a patient includes:

providing a warning to a user of a universal patient record created using the database of the inconsistency.

13. The method of claim 10 wherein the inconsistencies identifying potential health care issues for a patient include one or more of:

prescription drug inconsistencies indicative of drug interactions;

treatment inconsistencies; and

inconsistencies in demographic data;

14. The method of claim 1 wherein receiving health data from a community of independent sources of health data related to one or more patients, the health data being selected from the set including oral data, written data and/or electronic data includes:

receiving the health data by one or more of facsimile transmission, email transmission, and transmission of speech files over a network connection.

The method of claim 1 wherein the formatting the health data to enable creation of a universal patient record for each of the one or more patients by tagging of the health data to enable extraction of pertinent portions of the health data for coding of health encounters with the one or more patients includes:

flagging a field in the universal patient record indicating a need for resolution of the inconsistency.

16. The method of claim 15 wherein the flagging a field in the universal patient record indicating a need for resolution of the inconsistency includes:

providing a field indicative of whether the one or more patients are receiving compatible care from the one or more independent sources of health data.

17. The method of claim 1 wherein said enabling controlled access to the formatted health data via a network connecting the community of independent sources of health data, the network including at least one central server including a central database storing the formatted health data and at least two on-site servers, each on-site server located at one of the independent sources of health data and each on-site server configured to hold a database replicated from the central database further comprises:

providing cryptographic access to the formatted health data.

18. A computer program product comprising:

a signal bearing medium bearing;

one or more instructions for receiving health data from a community of independent sources of health data related to one or more patients, the health data being selected from the set including oral data, written data and/or electronic data;

one or more instructions for formatting the health data to enable creation of a universal patient record for each of the one or more patients by tagging of the health data to enable extraction of pertinent portions of the health data for coding of health encounters with the one or more patients; and

one or more instructions for enabling controlled access to the formatted health data via a network connecting the community of independent sources of health data, the network including at least one central server including a central database storing the formatted health data and at least two on-site servers, each on-site server located at one of the independent sources of health data and each on-site server configured to hold a database replicated from the central database.

19. The computer program product of claim 18 wherein the signal bearing medium comprises:

a recordable medium.

20. The computer program product of claim 18 wherein the signal bearing medium comprises:

a transmission medium.

The computer program product of claim 18 wherein the one or more instructions for said enabling controlled access to the formatted health data via a network connecting the community of independent sources of health data, the network including at least one central server including a central database storing the formatted health data and at least two on-site servers, each on-site server located at one of the independent sources of health data and each on-site server configured to hold a database replicated from the central database further comprises:

one or more instructions for providing cryptographic access to the formatted health data.

22. A computer system comprising:

a processor;

input and output circuitry coupled to the processor;

a memory coupled to the processor; and

a module coupled to the processor, the module configured to incorporate health data records, module configured to receive health data from a community of independent sources of health data related to one or more patients, the health data being oral data, written data and/or electronic data;

automatically format the health data to enable creation of a universal patient record for each of the one or more patients, the formatting including tagging of the health data to enable extraction of pertinent portions of the health data for coding of health encounters with the one or more patients; and

enable controlled access to the formatted health data via a network connecting the community of independent sources health data, the network including at least one central server including a central database of the formatted health data and at least two on-site servers, each on-site server located at one of the independent sources of health data and each on-site server configured to hold a database replicated from the central database.
