Abstract

A reciprocating surgical file system for precisely removing bone and/or other tissue is described. The system allows a user to maneuver the system and navigate into hard-to-access sites under a direct vision mechanism. A transmission mechanism converts rotary motion from a motor into reciprocating motion and provides it to the surgical file for precision removal of bone or other tissue. A pulsatile pump mechanism is operatively coupled with the transmission mechanism and provides irrigating fluid to the surgical site.

Claims

1. A surgical instrument comprising:

a blade for removing tissue, said blade having a plurality of throughholes; and

a lower blade structure that couples to said blade;

wherein said lower blade structure comprises a fluid delivery channel that extends substantially along a long axis of said lower blade structure;

wherein at least a portion of said fluid delivery channel is aligned with at least one of said throughholes in said blade so as to permit flow through said at least one of said throughholes; and

wherein said blade is configured to move along said long axis with respect to said lower blade structure.

2. The surgical instrument of claim 1, wherein said instrument is configured to produce pulsatile flow of fluid when said blade is moved reciprocally along said long axis.

3. The surgical instrument of claim 1, wherein said instrument is configured to produce a substantially pulsatile flow of fluid when said blade is moved reciprocally along said long axis, and while said fluid is supplied to said fluid delivery channel at a substantially constant pressure.

4. The surgical instrument of claim 1, wherein at least one of said throughholes is adjacent to said fluid delivery channel such that fluid can flow substantially continuously through said at least one of said throughholes.

The surgical instrument of claim 1, wherein at least one of said throughholes is aligned with said fluid delivery channel to permit fluid through said at least one of said throughholes when said blade is in a first position with respect to said lower blade structure, and wherein said at least one of said throughholes is not aligned with said fluid delivery channel when said blade is in a second position with respect to said lower blade structure.

6. The surgical instrument of claim 1, wherein said fluid delivery channel comprises a elongate portion extending along said long axis and a plurality of side channels extending from said elongate portion.

7. The surgical instrument of claim 6, wherein said instrument is configured to permit fluid flow from at least one side channel through at least one of said throughholes when said at least one throughhole is aligned with said at least one side channel.

8. The surgical instrument of claim 7, wherein said instrument is configured to substantially obstruct fluid flow from said at least one side channel through said at least one of said throughholes when said at least one of said throughholes is not aligned with said at least one side channel.

9. The surgical instrument of claim 7, wherein said blade is configured to move reciprocally such that said at least one of said throughholes and at least one of said side channels are alternatingly aligned and not aligned.

10. A surgical instrument comprising:

means for removing tissue from a mammal; and

means for delivering fluid through said means of removing tissue;

wherein movement of said means for removing tissue relative to said means for delivering fluid produces substantially pulsatile flow.

11. The surgical instrument, comprising:

a blade configured to remove tissue from a patient; and

a lower blade structure that couples with said blade;

wherein reciprocating motion of said blade with respect to said lower blade structure generates substantially pulsatile flow of fluid through said blade when said fluid is supplied to said lower blade structure at a substantially constant pressure.

12. The surgical instrument of claim 11, wherein said tissue is bone.

13. The surgical instrument of claim 11, wherein said lower blade structure comprises a fluid delivery channel configured to permit fluid flow therethrough.

14. The surgical instrument of claim 13, wherein said blade comprises at least one throughhole that is adjacent to said fluid delivery channel such that fluid can flow substantially continuously through said at least one throughhole.

15. The surgical instrument of claim 11, wherein said blade comprises at least one throughhole that is aligned with said fluid delivery channel to permit fluid through said at least one throughhole when said blade is in a first position with respect to said lower blade structure, and wherein said at least one throughhole is not aligned with said fluid delivery channel when said blade is in a second position with respect to said lower blade structure.

16. The surgical instrument of claim 11, wherein said fluid delivery channel comprises an elongate portion extending along a long axis of said lower blade structure and a plurality of side channels extending from said elongate portion.

17. The surgical instrument of claim 16, wherein said blade comprises at least one throughhole, wherein said instrument is configured to obstruct substantially fluid flow from said at least one of said plurality of side channels through said at least one throughhole when said at least one throughhole is not aligned with said at least one of said plurality of side channels.

18. The surgical instrument of claim 17, wherein said blade is configured to move reciprocally such that said at least one throughhole and at least one of said plurality of side channels are alternatingly aligned and not aligned.
