Abstract

A method for delivering ultrasound energy to a patient'"'"'s intracranial space involves forming at least one hole in the patient'"'"'s skull, advancing at least one ultrasound delivery device at least partway through the hole(s), and transmitting ultrasound energy from the ultrasound delivery device(s). According to various embodiments, ultrasound delivery devices may be advanced into the epidural space, one or both ventricles and/or an intracerebral space of the patient'"'"'s brain. In alternative embodiments, one or multiple holes may be formed in the skull, and any number of ultrasound delivery devices may be used. Intracranial ultrasound delivery may be used in treatment of ischemic stroke, head trauma, atherosclerosis, perfusion disorders and other acute or chronic neurological conditions.

Claims

1. A method for delivering ultrasound energy to a patient'"'"'s intracranial space, the method comprising:

forming at least one hole in the patient'"'"'s skull;

advancing at least one ultrasound delivery device at least partway through the hole(s); and

transmitting ultrasound energy from the ultrasound delivery device(s).

2. The method of claim 1, wherein multiple holes are formed in the skull, and at least one ultrasound delivery device is advanced at least partway through each hole.

3. The method of claim 1, wherein one hole is formed in the skull, and multiple ultrasound delivery devices are advanced at least partway through the hole.

4. The method of claim 1, wherein the at least one hole is formed using a drill or burr device.

5. The method of claim 1, wherein advancing the at least one delivery device comprises advancing the device(s) into the patient'"'"'s epidural space.

6. The method of claim 1, wherein advancing the at least one delivery device comprises advancing the device(s) into at least one ventricle of the patient'"'"'s brain.

7. The method of claim 1, wherein advancing the at least one delivery device comprises advancing the device(s) into an intracerebral space of the patient'"'"'s brain.

8. The method of claim 1, further comprising, after the forming step, placing an introducer device at least partway into the at least one hole, wherein the at least one ultrasound delivery device is advanced through the introducer device.

9. The method of claim 8, wherein placing the introducer device comprises advancing the introducer device through the hole and into the patient'"'"'s epidural space.

10. The method of claim 8, wherein placing the introducer device comprises advancing the introducer device through the hole and into a ventricle of the patient'"'"'s brain.

11. The method of claim 8, wherein placing the introducer device comprises advancing the introducer device through the hole and into an intracerebral space of the patient'"'"'s brain.

12. The method of claim 1, wherein advancing the ultrasound delivery device comprises advancing a device selected from the group consisting of an ultrasound transducer and a transducer-tipped ultrasound catheter.

13. The method of claim 1, wherein the ultrasound energy is transmitted acutely.

14. The method of claim 1, wherein the ultrasound energy is transmitted chronically.

15. The method of claim 1, further comprising delivering at least one pharmacologic agent to the patient.

16. The method of claim 15, wherein the agent is selected from a group consisting of blood clot reducing agents such as tissue plasminogen activator, rTPA, Urokinease, Streptokinase, Alteplase and Desmoteplase, antiplatelet agents such as aspirin, Clopidorgel and Ticclopidine, and GIIb/IIIa inhibitors, such as Abciximab, Tirofiban and Eptifibatide.

17. The method of claim 15, wherein delivering the agent comprises using a method selected from the group consisting of intravenous, arterial and oral delivery.

18. The method of claim 1, further comprising delivering microbubbles or nanobubbles into the patient'"'"'s bloodstream.

19. The method of claim 18, wherein the microbubbles or nanobubbles are delivered intravenously or arterially.

20. The method of claim 18, wherein the microbubbles or nanobubbles are delivered in combination or sequentially.

21. The method of claim 18, further comprising providing at least one pharmacologic agent to the patient.

22. The method of claim 1, wherein the ultrasound energy is transmitted to a blood clot in the patient'"'"'s brain to help disrupt the blood clot.

23. The method of claim 22, wherein the clot resides in a blood vessel.

24. The method of claim 22, wherein the clot resides in an intracerebral space of the patient'"'"'s brain, outside of blood vessels.

25. The method of claim 1, wherein the ultrasound energy is transmitted to a blood vessel in the patient'"'"'s brain to help treat atherosclerosis of the blood vessel.

26. The method of claim 1, wherein the ultrasound energy is transmitted to one or more blood vessels in the brain to help treat a blood perfusion abnormality in the patient'"'"'s brain.

27. The method of claim 1, wherein transmitting ultrasound energy comprises simultaneously transmitting energy from multiple delivery devices.

28. The method of claim 1, wherein transmitting ultrasound energy comprises sequentially transmitting energy from multiple delivery devices.

29. The method of claim 1, wherein the ultrasound energy is transmitted from multiple delivery devices with the same frequency and intensity.

30. The method of claim 1, wherein the ultrasound energy is transmitted from multiple delivery devices with at least one of different frequencies, different intensities and different modes.

31. The method of claim 1, wherein transmitting ultrasound energy comprises transmitting energy having a frequency between about 20 KHz and 10 MHz.

32. The method of claim 1, wherein the ultrasound energy is transmitted in continuous mode.

33. The method of claim 1, wherein the ultrasound energy is transmitted in pulse mode.

34. The method of claim 1, wherein the ultrasound energy is modulated.

35. The method of claim 1, further comprising monitoring treatment progress using one or more visualization or sensing devices.

96. The method of claim 1, further providing a conductive medium which interfaces between the ultrasound transducer and the patient.

97. The method of claim 96, wherein the conductive medium comprises a compliant pack.

98. The method of claim 97, further providing gel or oil inside the pack.

99. The method of claim 8, further providing a conductive medium which interfaces between the ultrasound transducer and the patient.

100. The method of claim 99, wherein the conductive medium comprises a compliant pack.

101. The method of claim 100, further providing gel or oil inside the pack.

36. A method for delivering ultrasound energy from within a patient'"'"'s epidural space, the method comprising:

advancing at least one ultrasound delivery device through at least one hole in the patient'"'"'s skull to locate at least a distal portion of the device in the patient'"'"'s epidural space; and

transmitting ultrasound energy from the ultrasound delivery device(s).

37. The method of claim 36, further comprising forming the at least one hole in the patient'"'"'s skull.

38. The method of claim 36, further comprising placing an introducer device at least partway into the at least one hole, wherein the at least one ultrasound delivery device is advanced through the introducer device.

39. The method of claim 36, wherein multiple ultrasound delivery devices are advanced through one hole in the patient'"'"'s skull.

40. The method of claim 36, wherein at least one ultrasound delivery device is advanced through each of multiple holes in the patient'"'"'s skull.

41. The method of claim 36, wherein advancing the ultrasound delivery device comprises advancing a device selected from the group consisting of an ultrasound transducer and a transducer-tipped ultrasound catheter.

42. The method of claim 36, wherein the ultrasound energy is transmitted acutely.

43. The method of claim 36, wherein the ultrasound energy is transmitted chronically.

44. The method of claim 36, further comprising delivering at least one pharmacologic agent to the patient.

The method of claim 44, wherein the agent is selected from a group consisting of blood clot reducing agents such as tissue plasminogen activator, rTPA, Urokinease, Streptokinase, Alteplase and Desmoteplase, antiplatelet agents such as aspirin, Clopidorgel and Ticclopidine, and GIIb/IIIa inhibitors, such as Abciximab, Tirofiban and Eptifibatide.

48. The method of claim 47, wherein the microbubbles or nanobubbles are delivered intravenously or arterially.

50. The method of claim 47, further comprising providing at least one pharmacologic agent to the patient.

51. The method of claim 36, wherein the ultrasound energy is transmitted to a blood clot in the patient'"'"'s brain to help disrupt the blood clot.

52. The method of claim 51, wherein the clot resides in a blood vessel.

53. The method of claim 51, wherein the clot resides in an intracerebral space of the patient'"'"'s brain, outside of blood vessels.

54. The method of claim 36, wherein the ultrasound energy is transmitted to a blood vessel in the patient'"'"'s brain to help treat atherosclerosis of the blood vessel.

55. The method of claim 36, wherein the ultrasound energy is transmitted to one or more blood vessels in the brain to help treat a blood perfusion abnormality in the patient'"'"'s brain.

102. The method of claim 36, further providing a conductive medium which interfaces between the ultrasound transducer and the patient.

103. The method of claim 38, further providing a conductive medium which interfaces between the ultrasound transducer and the patient.

56. A method for delivering ultrasound energy from within at least one ventricle of a patient'"'"'s brain, the method comprising:

advancing at least one ultrasound delivery device through at least one hole in the patient'"'"'s skull to locate at least a distal portion of the device in at least one ventricle of the patient'"'"'s brain; and

transmitting ultrasound energy from the ultrasound delivery device(s).

57. The method of claim 56, further comprising forming the at least one hole in the patient'"'"'s skull.

58. The method of claim 56, further comprising placing an introducer device at least partway into the at least one hole, wherein the at least one ultrasound delivery device is advanced through the introducer device.

59. The method of claim 56, wherein multiple ultrasound delivery devices are advanced through one hole in the patient'"'"'s skull.

60. The method of claim 56, wherein at least one ultrasound delivery device is advanced through each of multiple holes in the patient'"'"'s skull.

61. The method of claim 56, wherein advancing the ultrasound delivery device comprises advancing a device selected from the group consisting of an ultrasound transducer and a transducer-tipped ultrasound catheter.

62. The method of claim 56, wherein the ultrasound energy is transmitted acutely.

63. The method of claim 56, wherein the ultrasound energy is transmitted chronically.

64. The method of claim 56, further comprising delivering at least one pharmacologic agent to the patient.

The method of claim 64, wherein the agent is selected from a group consisting of blood clot reducing agents such as tissue plasminogen activator, rTPA, Urokinease, Streptokinase, Alteplase and Desmoteplase, antiplatelet agents such as aspirin, Clopidorgel and Ticclopidine, and GIIb/IIIa inhibitors, such as Abciximab, Tirofiban and Eptifibatide.

66. The method of claim 64, wherein delivering the agent comprises using a method selected from the group consisting of intravenous, arterial and oral delivery.

67. The method of claim 56, further comprising delivering microbubbles or nanobubbles into the patient'"'"'s bloodstream.

68. The method of claim 67, wherein the microbubbles or nanobubbles are delivered intravenously or arterially.

69. The method of claim 67, wherein the microbubbles or nanobubbles are delivered in combination or sequentially.

70. The method of claim 67, further comprising providing at least one pharmacologic agent to the patient.

75. The method of claim 56, wherein the ultrasound energy is transmitted to one or more blood vessels in the brain to help treat a blood perfusion abnormality in the patient'"'"'s brain.

104. The method of claim 56, further providing a conductive medium which interfaces between the ultrasound transducer and the patient.

76. A method for delivering ultrasound energy from within an intracerebral space of a patient'"'"'s brain, the method comprising:

advancing at least one ultrasound delivery device through at least one hole in the patient'"'"'s skull to locate at least a distal portion of the device an intracerebral space of the patient'"'"'s brain; and

transmitting ultrasound energy from the ultrasound delivery device(s).

77. The method of claim 76, further comprising forming the at least one hole in the patient'"'"'s skull.

78. The method of claim 76, further comprising placing an introducer device at least partway into the at least one hole, wherein the at least one ultrasound delivery device is advanced through the introducer device.

79. The method of claim 76, wherein multiple ultrasound delivery devices are advanced through one hole in the patient'"'"'s skull.

80. The method of claim 76, wherein at least one ultrasound delivery device is advanced through each of multiple holes in the patient'"'"'s skull.

81. The method of claim 76, wherein advancing the ultrasound delivery device comprises advancing a device selected from the group consisting of an ultrasound transducer and a transducer-tipped ultrasound catheter.

82. The method of claim 76, wherein the ultrasound energy is transmitted acutely.

83. The method of claim 76, wherein the ultrasound energy is transmitted chronically.

84. The method of claim 76, further comprising delivering at least one pharmacologic agent to the patient.
