Abstract

A system and method for generating and/or updating a personal/individual health record. Inputs of data to the system may come from diverse sources including, but not limited to, patient questionnaires, insurance company (or other payor) claims data, hospitals, clinics and other institutional providers, and individual physicians and physicians'"'"' offices.

Claims

1. A method for generating a personal/individual health record, said method comprising:

receiving health-related data associated with a patient, wherein said health-related data was created during an encounter between said patient and a health care provider; and

updating a personal/individual health record of said patient with said health-related data.

2. The method of claim 1, wherein said receiving step includes downloading said health-related data from a computer system associated with said health care provider.

3. The method of claim 1, wherein said health-related data is selected from the group consisting of diagnosis data that is indicative of a diagnosis by said health care provider during said encounter, procedure data that is indicative of a procedure by said health care provider during said encounter, and image data that is indicative of an image created during said encounter.

4. A method for generating a personal/individual health record, said method comprising:

receiving medication data associated with a patient, wherein said medication data is indicative of a medication prescribed to said patient; and

updating a personal/individual health record of said patient with said medication data.

5. The method of claim 4, wherein said receiving step includes downloading said medication data from a computer system associated with a pharmacy benefits manager.

6. A method for generating a personal/individual health record, said method comprising:

receiving laboratory data associated with a patient, wherein said laboratory data is indicative of a test performed by a laboratory; and

updating a personal/individual health record of said patient with said laboratory data.

7. The method of claim 6, wherein said receiving step includes downloading said laboratory data from a computer system associated with a laboratory.

8. A method for generating a personal/individual health record, said method comprising:

receiving health-related data from a computer system associated with a patient, wherein said health-related data is indicative of a health-related parameter of said patient; and

updating a personal/individual health record of said patient with said health-related data.

9. The method of claim 8, further comprising receiving a request that said patient be provided access to said personal/individual health record, wherein said request is associated with a payor.

10. The method of claim 8, further comprising receiving a request that said patient be provided access to said personal/individual health record, wherein said request is associated with an employer of said patient.

11. The method of claim 8, wherein said personal/individual health record was generated prior to said updating step based on a collection of data selected from the group consisting of payor claims data, medication data associated with a pharmacy benefits manager, diagnosis data associated with said health care provider, procedure data associated with said health care provider, laboratory data associated with a laboratory, and imaging data associated with said health care provider.
