Abstract

A method for operating a cardiac pacemaker in which the mode of operation of the pacemaker is altered in response to detecting an episode of atrial tachycardia. In accordance with the invention, the pacemaker'"'"'s pacing mode is altered in a manner that attempts to maintain hemodynamic stability during the atrial tachycardia. Such a mode switch is particularly applicable to pacemaker patients suffering from some degree of congestive heart failure.

Claims

1. A cardiac pacemaker, comprising:

an atrial sensing channel for sensing atrial depolarizations and generating atrial sense signals in accordance therewith;

one or more ventricular sensing channels for sensing ventricular depolarizations and generating ventricular sense signals in accordance therewith;

right and left pacing channels for delivering paces to the right and left ventricles;

a controller for controlling the delivery of paces and detecting tachyarrhythmias;

wherein the controller is configured to operate in a normal pacing mode that does not include resynchronization pacing when no atrial tachyarrhythmia is detected; and

,

wherein the controller is configured to switch the pacemaker to an atrial fibrillation pacing mode upon detection of an atrial tachyarrhythmia, and wherein the atrial fibrillation pacing mode includes resynchronization pacing that paces the left ventricle.

2. The pacemaker of claim 1 wherein the atrial fibrillation pacing mode paces the left ventricle in a biventricular pacing inhibited demand pacing mode.

3. The pacemaker of claim 1 wherein the atrial fibrillation pacing mode paces the left ventricle in an LV-only pacing mode.

4. The pacemaker of claim 1 wherein the atrial fibrillation pacing mode paces the left ventricle in a biventricular triggered pacing mode.

5. The pacemaker of claim 1 wherein the normal pacing mode includes atrial-triggered ventricular pacing, and wherein switching to an atrial fibrillation mode includes reverting to a non-atrial triggered ventricular pacing mode.

6. The pacemaker of claim 1 wherein the normal pacing mode that is switched from upon detection of an atrial tachyarrhythmia includes rate-adaptive pacing and further wherein the atrial fibrillation mode includes an adjustment to a rate-adaptive parameter.

7. The pacemaker of claim 1 wherein the normal pacing mode that is switched from upon detection of an atrial tachyarrhythmia is non-rate-adaptive and the atrial fibrillation mode is rate-adaptive.

8. The pacemaker of claim 1 wherein the atrial fibrillation mode includes ventricular rate regularization by adjustment of a ventricular escape interval in order to regularize the rate at which ventricular beats occur.

9. The pacemaker of claim 8 wherein the adjustment of the ventricular escape interval is based on a previous value of the ventricular escape interval and a measured intrinsic ventricular rate.

10. The pacemaker of claim 8 wherein the adjustment of the ventricular escape interval increases the ventricular escape interval after a ventricular pace is delivered and decreases the ventricular escape interval after a ventricular sense.

11. A method for operating cardiac pacemaker, comprising:

sensing atrial depolarizations to detect atrial tachyarrhythmias;

operating in a normal pacing mode that does not include resynchronization pacing when no atrial tachyarrhythmia is detected; and

,

switching the pacemaker to an atrial fibrillation pacing mode upon detection of an atrial tachyarrhythmia, wherein the atrial fibrillation pacing mode includes resynchronization pacing that paces the left ventricle.

12. The method of claim 11 wherein the atrial fibrillation pacing mode paces the left ventricle in a biventricular pacing inhibited demand pacing mode.

13. The method of claim 11 wherein the atrial fibrillation pacing mode paces the left ventricle in an LV-only pacing mode.

14. The method of claim 11 wherein the atrial fibrillation pacing mode paces the left ventricle in a biventricular triggered pacing mode.

15. The method of claim 11 wherein the normal pacing mode includes atrial-triggered ventricular pacing, and wherein switching to an atrial fibrillation mode includes reverting to a non-atrial triggered ventricular pacing mode.

16. The method of claim 11 wherein the normal pacing mode that is switched from upon detection of an atrial tachyarrhythmia includes rate-adaptive pacing and further wherein the atrial fibrillation mode includes an adjustment to a rate-adaptive parameter.

17. The method of claim 11 wherein the normal pacing mode that is switched from upon detection of an atrial tachyarrhythmia is non-rate-adaptive and the atrial fibrillation mode is rate-adaptive.

18. The method of claim 11 wherein the atrial fibrillation mode includes ventricular rate regularization by adjustment of a ventricular escape interval in order to regularize the rate at which ventricular beats occur.

19. The method of claim 18 wherein the adjustment of the ventricular escape interval is based on a previous value of the ventricular escape interval and a measured intrinsic ventricular rate.
