Abstract

A system and method for predicting the effect of patient self-care actions on a disease control parameter. A future disease control parameter value X(t_j) at time t_j is determined from a prior disease control parameter value X(t_i) at time t_i based on an optimal control parameter value R(t_j) at time t_j, the difference between the prior disease control parameter value X(t_i) and an optimal control parameter value R(t_i) at time t_i, and a set of differentials between patient self-care parameters having patient self-care values S_M(t_i) at time t_i and optimal self-care parameters having optimal self-care values O_M(t_i) at time t_i. The differentials are multiplied by corresponding scaling factors K_M. The system includes an input device for entering the patient self-care values S_M(t_i). A memory stores the optimal control parameter values R(t_i) and R(t_j), the prior disease control parameter value X(t_i), the optimal self-care values O_M(t_i), and the scaling factors K_M. A processor in communication with the input device and memory calculates the future disease control parameter value X(t_j). A display is connected to the processor to display the future disease control parameter value X(t_j) to a patient.

Claims

1. A system configured to determine a future disease control parameter value comprising:

a recording device configured to record a prior disease control parameter value;

a processor unit configured to communicate with the recording unit; and

a memory comprising (i) an optimal disease control parameter value, (ii) one or more optimal self-care values, (iii) one or more scaling factors for weighting an impact of one or more self-care actions by the patient on a disease control parameter value of the patient and (iv) one or more actual self-care values of the patient, wherein the memory is readable by the processor unit and the processor unit is configured to calculate the future disease control parameter value based upon (i) the optimal disease control parameter value, (ii) the one or more optimal self-care values, (iii) the one or more scaling factors, (iv) the one or more actual self-care values of a patient and (v) the prior disease control parameter value.

2. The system according to claim 1, wherein the prior disease control parameter value comprises a simulated disease control parameter value.

3. The system according to claim 1, wherein the prior disease control parameter value comprises a measured actual disease control parameter value.

4. The system according to claim 1, wherein the memory comprises a data storage card.

5. The system according to claim 1, further comprising a housing, wherein the memory and the processor unit are housed within said housing, and further comprising an output port, said output port being integral with said housing, thereby providing a hand-held, readily transportable device.

6. The system according to claim 5, wherein the recording device is arranged within said housing.

7. The system according to claim 1, further comprising a display configured to display information according to said future disease control parameter value, thereby enabling the patient to select appropriate self care actions.

8. The system according to claim 7, wherein said display is configured for displaying future disease control parameter values in graphical form.

9. The system according to claim 1, wherein the recording device further comprises one or more monitoring devices for collecting one or more measurements of a physiological condition of the patient.

The system according to claim 9, wherein the one or more monitoring devices receive the one or more measurements from one or more sensors selected from the group consisting of a sensor for measuring a physiological condition, a sensor for measuring respiratory flow, a sensor for measuring a weight-related condition, a sensor for measuring a heart related condition, a sensor for measuring blood pressure and a sensor for measuring electrical activity of the individual.

11. A system configured to determine a future disease control parameter value comprising:

a recording device configured to record a prior disease control parameter value;

a processor unit configured to communicate with the recording unit;

a server configured to communicate with the processor unit; and

a memory comprising (i) an optimal disease control parameter value, (ii) one or more optimal self-care values, (iii) one or more scaling factors for weighting an impact of one or more self-care actions by a patient on a disease control parameter value of the patient and (iv) one or more actual self-care values of the patient, wherein the memory is readable by the server and the server is configured to calculate the future disease control parameter value based upon (i) the optimal disease control parameter value, (ii) the one or more optimal self-care values, (iii) the one or more scaling factors, (iv) the one or more actual self-care values of a patient and (v) the prior disease control parameter value.

12. The system according to claim 11, wherein said server is configured to communicate with said processor unit via at least one communication network.

The system according to claim 12, wherein the at least one communication network comprises a network type or a combination of two or more network types selected from the group consisting of wireless networks, cellular networks, telephone networks, satellite networks, broadcast networks, digital broadcast networks, digital television networks and cable networks.

14. The system according to claim 11, wherein said server comprises a world wide web server.

15. The system according to claim 11, wherein said recording device comprises a device configured to communicate with said server via a wireless network.

16. A method for determining a future disease control parameter value comprising the steps of:

storing in one or more computer readable storage media (i) a prior disease control parameter value, (ii) an optimal disease control parameter value, (iii) one or more optimal self-care values, (iv) one or more scaling factors for weighting an impact of one or more self-care actions by a patient on a disease control parameter value of the patient and (v) one or more actual self-care values of the patient;

calculating the future disease control parameter value based upon (i) the optimal disease control parameter value, (ii) the one or more optimal self-care values, (iii) the one or more scaling factors, (iv) the one or more actual self-care values of a patient and (v) the prior disease control parameter value.

17. The method according to claim 16, wherein the one or more storage media comprise a memory.

18. The method according to claim 16, wherein the one or more storage media comprise a smart card.

19. A system configured to determine a future disease control parameter value comprising:

a server; and

a memory comprising (i) an optimal disease control parameter value, (ii) one or more optimal self-care values, (iii) one or more scaling factors for weighting an impact of one or more self-care actions by a patient on a disease control parameter value of the patient and (iv) one or more actual self-care values of the patient, wherein the memory is readable by the server and the server is configured to calculate the future disease control parameter value based upon (i) the optimal disease control parameter value, (ii) the one or more optimal self-care values, (iii) the one or more scaling factors, (iv) the one or more actual self-care values of a patient and (v) the prior disease control parameter value.

20. The system according to claim 19, further comprising a display configured to display information according to said future disease control parameter value, thereby enabling the patient to select appropriate self care actions.
