Abstract

Apparatus and methods for accessing an inferior vertebra, a superior vertebra, and a disc space therebetween via a transpedicular approach in the inferior vertebra that may include creating a channel normal to the pedicle and using an offset guide to create a second transpedicular channel at an angle to the first, normal pedicle channel where the second transpedicular channel passes into the inferior vertebra, superior vertebra, or the disc space therebetween.

Claims

1. An system, including:

an offset guide, the guide including a first channel with a first longitudinal axis and a second channel with a second longitudinal axis, the first longitudinal axis offset a predetermined angle from the second longitudinal axis.

2. The system of claim 1, the guide including a slot along the first longitudinal axis.

3. The system of claim 1, the first channel capable of receiving one of a guide wire having longitudinal axis and a cannula having a longitudinal axis.

4. The system of claim 1, the second channel capable of receiving one of a guide wire having a longitudinal axis and a cannula having a longitudinal axis.

5. The system of claim 4, further including a cannula with a longitudinal axis, a proximal end, and a distal end, the distal end capable of passing through the second channel and the distal end including a slot along the longitudinal axis.

6. The system of claim 5, further including a guide wire with a longitudinal axis, the guide wire capable of being inserted within the first channel and capable of passing through the cannula distal slot.

7. The system of claim 5, wherein the cannula is slidably coupled to the second channel.

8. The system of claim 7, further including a rotatable knob operatively coupled to the cannula so that when rotated the cannula is linearly translated along the second channel longitudinal axis.

9. The system of claim 5, the cannula capable of engaging a vertebra.

10. The system of claim 5, the cannula capable of engaging a vertebra pedicle at approximately normal to the vertebra.

11. The system of claim 10, further including a guide wire capable of being inserted into the first channel, passing through the vertebra pedicle and cannula distal slot into an adjacent disc space.

12. The system of claim 10, further including a guide wire capable of being inserted into the first channel, passing through the vertebra pedicle, cannula distal slot, an adjacent disc space, and into an adjacent bony structure.

13. The system of claim 12, wherein the adjacent bony structure is a vertebra.

14. A method, including:

employing an offset guide including a first channel with a first longitudinal axis and a second channel with a second longitudinal axis, the first longitudinal axis offset a predetermined angle from the second longitudinal axis to access a bony structure.

15. The method of claim 14, further including inserting one of a guide wire having longitudinal axis and a cannula having a longitudinal axis in the first channel.

16. The method of claim 15, further including inserting one of a guide wire having a longitudinal axis and a cannula having a longitudinal axis in the second channel.

17. The method of claim 14, further including inserting a cannula with a longitudinal axis, a proximal end, and a distal end, the distal end capable of passing through the second channel and the distal end including a slot along the longitudinal axis.

18. The method of claim 17, further including inserting a guide wire with a longitudinal axis in the first channel and through the cannula distal slot.

19. The method of claim 17, further including rotating a knob operatively coupled to the cannula to linearly translate the cannula along the second channel longitudinal axis.

20. The method of claim 17, further including engaging a vertebra via the cannula.

21. The method of claim 17, further including engaging a vertebra pedicle at approximately normal to the vertebra via the cannula.

22. The method of claim 21, further including inserting a guide wire into the first channel, through the vertebra pediele, the cannula distal slot, the an adjacent disc space, and into an adjacent bony structure.

23. A surgical kit, including:

an offset guide, the guide including a first channel with a first longitudinal axis and a second channel with a second longitudinal axis, the first longitudinal axis offset a predetermined angle from the second longitudinal axis; and

a guide wire having longitudinal axis and a cannula having a longitudinal axis capable of insertion within the first channel.

24. The surgical kit of claim 23, a cannula having a longitudinal axis capable of insertion within the first channel.

25. The surgical kit of claim 23, a guide wire having a longitudinal axis capable of insertion within the second channel.

26. The surgical kit of claim 23, further including a cannula with a longitudinal axis, a proximal end, and a distal end, the distal end capable of passing through the second channel and the distal end including a slot along the longitudinal axis.

27. The surgical kit of claim 26, wherein the cannula is capable of engaging a vertebra pedicle at approximately normal to the vertebra.

28. The surgical kit of claim 27, the guide wire capable of being inserted into the first channel, passing through the vertebra pedicle, cannula distal slot, and into an adjacent disc space.

29. The surgical kit of claim 27, the capable of being inserted into the first channel, passing through the vertebra pedicle, cannula distal slot, an adjacent disc space, and into an adjacent bony structure.
