Abstract

An evacuation sheath assembly and method of treating occluded vessels which reduces the risk of distal embolization during vascular interventions is provided. A method of treating a blood vessel, includes providing an elongate member having proximal and distal ends, a lumen therebetween, the distal end having a braided sheath expandable between a reduced and expanded diameter, and a fluid-tight covering. The elongate member is advanced into a blood vessel that supplies blood to the heart. The braided sheath is expanded to engage the endoluminal surface of the blood vessel and stop antegrade flow.

Claims

1. A method of treatment of a blood vessel, comprising the steps of:

providing an elongate member having a proximal end, a distal end, and a lumen therebetween, the distal end having an elongate braided sheath expandable between a reduced diameter and an expanded diameter, the elongate braided sheath further comprising a fluid tight covering;

advancing the elongate member into a blood vessel that supplies blood to the heart;

expanding the elongate braided sheath to engage the endoluminal surface of the blood vessel and stop antegrade blood flow.

2. The method of claim 1, further comprising the step of inducing retrograde blood flow within the blood vessel after antegrade blood flow is stopped.

3. The method of claim 2, further comprising the step of advancing a device across a stenosis in the blood vessel while antegrade blood flow is stopped.

The method of claim 1, wherein the elongate member further comprises an elongate actuation member connected to a distal region of the elongate braided sheath, and wherein the method further comprises the step of pulling the elongate actuation member proximally to expand the elongate braided sheath.

5. The method of claim 3, wherein the device is a guidewire.

6. The method of claim 3, wherein the device is an angioplasty catheter.

7. The method of claim 3, wherein the device is a stent-deployment catheter.

8. The method of claim 1, wherein the elongate member is advanced to a region of interest in the blood vessel that is proximal a lesion.

9. The method of claim 8, further comprising the step of dilating the lesion.

10. The method of claim 8, further comprising the step of deploying a stent in the lesion.

11. The method of claim 1, wherein the step of advancing the elongate member into the blood vessel comprises advancing the elongate member through a guide catheter.

12. An evacuation sheath, comprising:

an elongate catheter having a proximal end, a distal end, and a lumen therebetween;

an elongate braided sheath carried at the distal end of the catheter, the braided sheath having a fluid tight covering, wherein the sheath is expandable from a delivery configuration to an operational configuration; and

an elongate actuation member connected to a distal region of the braided sheath, the actuation member being movable within the catheter from a distal position to a proximal position to expand the braided sheath.

13. The sheath of claim 12, wherein the fluid tight covering is made from polyester.

14. The sheath of claim 12, wherein the fluid tight covering is made from a high strength polymer yarn.

15. The sheath of claim 12, wherein the braided sheath is made from a material selected from the group consisting of stainless steel, Eligiloy™

, and nickel titanium alloy.

16. The sheath of claim 12, wherein the fluid tight covering is made from polyurethane.

17. The sheath of claim 12, wherein the fluid tight covering is made from silicone.

18. The sheath of claim 12, wherein the fluid tight covering is made from an elastomeric material.

19. The sheath of claim 12, wherein the braided sheath and covering define an opening that communicates with an evacuation lumen having open proximal and distal ends.
