Abstract

A dual lumen guidewire support catheter for accurately crossing a chronic total occlusion in a vessel is disclosed. The dual lumen guidewire support catheter includes a first lumen portion and a second lumen portion sharing a common side and a common distal end. The first lumen portion can be a rapid exchange or over-the-wire portion, while the second lumen portion is an over the wire portion. One or both of the portions can include a discontinuity to enable removal of a guidewire following placement through or at an occlusion or lesion.

Claims

1. A dual lumen support catheter comprising:

a first lumen portion having a first lumen and a first length; and

a second lumen portion having a second lumen and a second length longer than or equal to said first length, said second lumen portion and said first lumen portion being laterally disposed one to another and said second lumen portion having a discontinuity extending from a common distal end of said first lumen portion and said second lumen portion toward a distal end of said second lumen portion.

2. The dual lumen support catheter of claim 1, wherein said first lumen portion is a rapid exchange lumen portion and said second lumen portion is an over the wire lumen portion.

3. The dual lumen support catheter of claim 2, wherein said discontinuity comprises a continuous slit.

4. The dual lumen support catheter of claim 3, wherein said discontinuity is capable of passing a guidewire therethrough.

5. The dual lumen support catheter of claim 2, wherein said second lumen portion comprises another discontinuity.

6. The dual lumen support catheter of claim 5, wherein each of said discontinuity and said another discontinuity is capable of passing a guidewire therethrough.

7. The dual lumen support catheter of claim 1, wherein said first lumen portion and said second lumen portion are an over the wire portion.

8. The dual lumen support catheter of claim 7, wherein said first lumen portion and said second lumen portion have a discontinuity in their walls comprising a continuous slit from their proximal ends to their distal ends.

9. The dual lumen support catheter of claim 8, wherein said discontinuities are capable of passing a guidewire therethrough.

10. The dual lumen support catheter of claim 1, further comprising a radiopaque position indicator.

11. The dual lumen support catheter of claim 1 comprising at least one port in a lateral surface of the proximal terminal portion of said dual lumen support catheter.

12. A dual lumen support catheter comprising:

a first lumen portion having a first interior surface defining a first lumen, a first length, and a first exterior surface, said first interior surface having a lubricious coating and said first exterior surface having a lubricious coating; and

a second lumen portion having a second interior surface defining a second lumen, a second length longer than or equal to said first length, and a second exterior surface, said second interior surface having a lubricious coating and said second exterior surface having a lubricious coating, said second lumen portion and said first lumen portion being laterally disposed one to another and said second lumen portion having a discontinuity extending from a common distal end of said first lumen portion and said second lumen portion toward a distal end of said second lumen portion.

13. The dual lumen support catheter of claim 12, wherein a lubricious coating is formed on said first lumen first exterior surface and said second lumen second exterior surface comprise a lubricious coating.

14. The dual lumen support catheter of claim 12, wherein said first lumen portion is a rapid exchange lumen portion and said second lumen portion is an over the wire lumen portion.

15. The dual lumen support catheter of claim 14, wherein said discontinuity comprises a continuous slit.

16. The dual lumen support catheter of claim 15, wherein said discontinuity is capable of passing a guidewire therethrough.

17. The dual lumen support catheter of claim 14, wherein said second lumen portion comprises another discontinuity.

18. The dual lumen support catheter of claim 17, wherein each of said discontinuity and said another discontinuity is capable of passing a guidewire therethrough.

19. The dual lumen support catheter of claim 12, wherein said first lumen portion and said second lumen portion are an over the wire portion.

20. The dual lumen support catheter of claim 19, wherein said first lumen portion and said second lumen portion have a discontinuity in their walls comprising a continuous slit from their proximal ends to their distal ends.

21. The dual lumen support catheter of claim 20, wherein said discontinuities are capable of passing a guidewire therethrough.

22. The dual lumen support catheter of claim 12, further comprising a radiopaque position indicator.

23. The dual lumen support catheter of claim 12, comprising at least one port in a lateral surface of the proximal terminal portion of said dual lumen support catheter.

24. A dual lumen support catheter, comprising:

a first lumen and a second lumen each having a distal terminal end defining a distal terminal portion for placement in a vessel proximal to a chronic total occlusion of said vessel;

a port defined in a lateral surface of said terminal, proximal portion of the catheter, said port being sized so as to allow passage therethrough of a stent delivery catheter, balloon catheter or guidewire into the vessel; and

a radiopaque marker positioned at said distal terminal end of said catheter.

25. The catheter as defined in claim 24, wherein said port is in communication with a lumen of the catheter, and wherein a second guidewire is configured to track via said second lumen of the catheter.

26. A method for using a dual lumen support catheter comprising:

inserting a first guidewire into a body vessel;

guiding said first guidewire into or near a proximal side of a chronic total occlusion of said body vessel; and

inserting a dual lumen support catheter over said first guidewire through a first lumen until a distal end of said dual lumen support catheter is proximal to said proximal side of said chronic total occlusion.

27. The method as recited in claim 26, further comprising inserting a second guidewire through a second lumen of said dual lumen support catheter to position said second guidewire proximally to said body vessel chronic total occlusion.

28. The method as recited in claim 27, further comprising breaching said chronic total occlusion with said second guidewire.

29. The method as recited in claim 28, further comprising separating said dual lumen support catheter from said second guidewire by removing said second guidewire through a discontinuity in said second lumen.

30. The method as recited in claim 27, pushing said second guidewire from a first region of a true lumen through a proximal side of said chronic total occlusion into a distal side of said chronic total occlusion and into a second region of a true lumen of the body vessel.

31. The method as recited in claim 30, further including the step of removing said dual lumen support catheter, and said first guidewire from the lumen of the body vessel and thereafter performing additional procedures by advancing catheters over said second guidewire.

32. A method as recited in claim 31, wherein the catheters advanced over the second guidewire are catheters to perform balloon angioplasty and stent deployment.
