Abstract

The invention relates to an integrated surgical navigational and neuromonitoring system. The integrated system provides real-time visualization of an instrument relative to a visualization of patient anatomy. The integrated system also acquires and incorporates neuromonitoring information into the visualization of the patient anatomy.

Claims

1. An apparatus comprising:

an intraoperative neurological monitoring system configured to provide real-time neurological information regarding a neural structure of a patient; and

a surgical navigational system communicatively linked with the intraoperative neurological monitoring system and configured to provide a geographical representation of the neural structure.

2. The apparatus of claim 1 wherein the intraoperative neurological monitoring system includes a surgical tool configured to acquire and relay the real-time neurological information to the surgical navigational system and wherein the surgical navigational system includes a monitor configured to display the geographical representation of the neural structure and a representative portion of the surgical tool relative to the geographical representation of the neural structure.

3. The apparatus of claim 2 wherein the surgical tool is configured to apply an electrical stimulus to the neural structure and acquire the real-time neurological information from a response of the neural structure to the electrical stimulus.

4. The apparatus of claim 2 wherein the surgical navigational system includes an electrode for detecting the real-time neurological information and wherein the surgical tool includes an anode and a cathode for applying the electrical stimulus.

5. The apparatus of claim 2 wherein the surgical navigational system includes an image database configured to store images containing the neural structure.

6. The apparatus of claim 5 wherein the images include at least one of anatomical images and functional images.

7. The apparatus of claim 5 wherein the images include at least one of CT images, MR images, PET images, x-ray images, and ultrasound images.

The apparatus of claim 1 further comprising a computer operatively coupled to the intraoperative neurological monitoring system and the surgical navigational system, and configured to register the real-time neurological information and the geographical representation of the neural structure for display in a GUI.

9. The apparatus of claim 8 wherein the surgical navigational system provides visualization of an anatomical setting of the neural structure and the intraoperative neurological monitoring system provides integrity information regarding the neural structure, and wherein the computer is further configured to visually convey the integrity information for the neural structure on the GUI.

10. The apparatus of claim 1 wherein the surgical navigation system further includes a surgical assistance interface configured to provide on-demand surgical information.

11. The apparatus of claim 10 wherein the surgical assistance interface is further configured to play at least one of an on-demand video recording and an on-demand audio recording, wherein the at least one of the on-demand video recording and the on-demand audio recording provides information usable by a surgeon for executing a given surgical procedure.

12. The apparatus of claim 1 wherein the surgical navigational system is further configured to show the geographical representation of the neural structure relative to an atlas image of the patient.

13. A method of guiding a neural surgical procedure, the method comprising:

applying electrostimulation to a neural structure of a patient;

determining a response to the electrostimulation by the neural structure;

determining positional information of the neural structure; and

displaying a geographical representation of the neural structure from the positional information of the neural structure on a GUI.

14. The method of claim 13 wherein the electrostimulation is applied by a surgical tool, and further comprising tracking a position of the surgical tool and indicating the position of the surgical tool on the GUI.

15. The method of claim 14 further comprising optically or electromagnetically tracking the surgical tool.

16. The method of claim 13 further comprising determining an integrity of the neural structure from a response of the neural structure to the electrostimulation and visually indicating the integrity on the GUI.

17. The method of claim 16 further comprising determining the integrity by at least one of electromyography (EMG), motor evoke potentials (MEP), somatosensory evoked potentials (SSEP), and tissue impedance.

18. The method of claim 13 further comprising displaying the geographical representation of the neural structure on an anatomical background derived from a diagnostic image of the patient.

19. The method of claim 18 further comprising recalling the diagnostic image from an image archival and storage system.

20. The method of claim 13 further comprising comparing neural structure response to the electrostimulation to an expected response, and providing a first indication if the neural structure response is consistent with the expected response and providing a second indication, different from the first indication, if the neural structure response is inconsistent with the expected response.

21. The method of claim 20 further comprising associating a unique visual identifier to the geographical representation of the neural structure as the second indication.

22. The method of claim 20 further comprising providing an audio alarm signal as the second indication.

23. The method of claim 20 further comprising generating and maintaining a log of applied electrostimulation and reaction thereto for electrostimulated neural structures of the patient.

24. A surgical method comprising:

applying a stimulus to a neural structure; and

visually determining a position of the stimulated neural structure from visual inspection of a GUI showing a geographical position of the stimulated neural structure.

25. The surgical method of claim 24 further comprising applying the electrical stimulus with a surgical tool.

26. The surgical method of claim 24 further comprising determining an integrity of the stimulated neural structure.

27. The surgical method of claim 24 further comprising determining a class of the stimulated neural structure from a coding afforded the stimulated neural structure in a visualization of the stimulated neural structure on the GUI.

28. The surgical method of claim 24 further comprising dictating a note regarding at least one of the position and an integrity of the stimulated neural structure and instructing an integrated neural monitoring and surgical navigation system displaying the GUI to associate the dictated note with an electronic marker of the stimulated neural structure.

29. The surgical method of claim 24 further comprising selecting a link on the GUI to an electronic file containing information regarding care of the stimulated neural structure based on at least one of the position and integrity of the stimulated neural structure.

30. A method comprising:

displaying a preoperative visualization of anatomy of a patient;

tracking placement of a surgical tool in the patient;

applying a stimulus to an anatomical feature of the patient;

determining a response of the anatomical feature to the stimulus; and

modifying the preoperative visualization to vary a visualization of the anatomical feature based on the response of the anatomical feature to the stimulus.
