Abstract

Methods of arthroscopic harvesting of bone marrow aspirate from bone joints and therapeutic application of the bone marrow aspirate in a bone joint. The method of harvesting includes extracting bone marrow from a joint, adding a biomaterial, and adding clotting agent to form a clot.

Claims

1. A method of forming a bone marrow aspirate patch, comprising:

establishing a portal proximal to a human joint using a trocar;

inserting an aspiration needle through the portal into the joint and subsequently, removing the trocar;

connecting a first syringe to the aspiration needle and subsequently, drawing bone marrow from the human joint using the first syringe;

extracting the bone marrow from the first syringe into a tray; and

adding a biomaterial and a clotting agent to the bone marrow to form a clot.

2. The method of claim 1, wherein the biomaterial comprises collagen.

3. The method of claim 1, wherein the clotting agent comprises calcium salt comprising calcium chloride solution having a concentration of about 10%.

4. The method of claim 1, wherein the clotting agent comprises thrombin solution having a concentration of 1000 I.U./ml.

5. The method of claim 1, wherein the human joint comprises one of a shoulder joint, a hip joint, an elbow joint, and a knee joint.

17. An arthroscopic method of treatment comprising:

forming a bone marrow aspirate patch as in claim 1; and

delivering the patch to an injured tissue.

18. The method of claim 17, wherein the step of delivering the patch further comprising:

passing a suture through a lateral cannula and through a rotator cuff of a shoulder joint, the suture having the patch on one end and a button on another end;

pulling the suture to place the patch under a tissue; and

pulling the button through the lateral cannula and subsequently, securing the suture in a criss-cross arrangement using knotless fixation devices and suture anchors.

6. A method of forming a bone marrow aspirate patch, comprising:

establishing a portal proximal to a human joint using a trocar;

inserting an aspiration needle through the portal into the joint and subsequently, removing the trocar;

connecting a first syringe to the aspiration needle and subsequently, drawing bone marrow from the human joint using the first syringe;

transferring the bone marrow from the first syringe to a centrifuge tube and subsequently, centrifuging the bone marrow at a plurality of speeds and times;

extracting serum and subsequently, adding a biomaterial comprising collagen; and

adding a clotting agent comprising calcium chloride solution having a concentration of about 10%, to form a clot.

7. The method of claim 6, wherein one of the plurality of speeds and times is 1100 rcf for 6 minutes.

8. The method of claim 6, wherein volume by volume ratio of the calcium chloride solution to the serum is 1:

50.

19. An arthroscopic method of treatment comprising:

forming a bone marrow aspirate patch as in claim 6; and

delivering the patch to an injured tissue.

9. A method of forming a bone marrow aspirate patch, comprising:

establishing a portal proximal to a human joint using a trocar;

inserting an aspiration needle through the portal into the joint and subsequently, removing the trocar;

connecting a first syringe to the aspiration needle and subsequently, drawing bone marrow from the human joint using the first syringe;

transferring the bone marrow from the first syringe to a second syringe filled with a biomaterial comprising demineralized bone matrix; and

adding a clotting agent in the second syringe to form a clot, the clotting agent comprising calcium chloride solution having a concentration of about 10%.

20. An arthroscopic method of treatment comprising:

forming a bone marrow aspirate patch as in claim 9; and

delivering the patch to an injured tissue.

10. A method of forming a bone marrow aspirate patch, comprising:

establishing a portal proximal to a human joint using a trocar;

inserting an aspiration needle through the portal into the joint and subsequently, removing the trocar;

connecting a first syringe to the aspiration needle and subsequently, drawing bone marrow from the human joint using the first syringe;

injecting the bone marrow from the first syringe into a port of a cannula;

adding a clotting agent to the bone marrow in the cannula and removing the first syringe subsequent to formation of a clot in the cannula;

passing a suture through the clot in the cannula using a needle and loosening the clot in the cannula using an obturator; and

removing the clot from the cannula by pulling on the suture, the clot being on the suture subsequent to removal from the cannula.

11. The method of claim 10, wherein the clotting agent comprises calcium salt comprising calcium chloride solution having a concentration of about 10%.

12. The method of claim 10, wherein the clotting agent comprises thrombin solution having a concentration of 1000 I.U./ml.

13. The method of claim 12, wherein volume by volume ratio of the thrombin solution to the bone marrow is 1:

14. The method of claim 10, wherein the human joint comprises one of a shoulder joint, a hip joint, an elbow joint, and a knee joint.

15. The method of claim 10, wherein the human joint is a shoulder joint, and the step of inserting the aspiration needle comprises inserting the aspiration needle into a proximal humeral head posterior intra-articularly.

16. The method of claim 10, wherein the human joint is a knee joint, and the step of inserting the aspiration needle comprises inserting the aspiration needle into a medullary canal of a femur by approaching a intercondylar notch of the femur.

21. An arthroscopic method of treatment comprising:

forming a bone marrow aspirate patch as in claim 10; and

delivering the patch to an injured tissue.

22. An apparatus for bone marrow harvesting, comprising:

at least one syringe;

a needle, connected to the base of the syringe;

a cannula having a leur lock side port, the base of the syringe being interchangeably connected to the port and the needle; and

a suture to remove a clot from the cannula.
