Abstract

The disclosure provides techniques for parameter-directed shifting of electrical stimulation electrode combinations having substantially similar electrode patterns. An external programmer permits a user to shift electrode combinations along the length of a lead or leads. The external programmer accepts parameter-directed shift input and causes an electrical stimulator to shift electrode combinations as indicated. The external programmer may present an electrode combination as a parameter that can be adjusted or selected to shift the electrode combination along the length of a lead. An electrode combination may be presented as a value that can be incremented, decremented, or otherwise adjusted to indicate a shift in a desired direction. An external programmer that permits a patient or other user to shift electrode combinations in a manner similar to adjustments of other parameters may enable the patient to maintain or improve therapeutic efficacy.

Claims

1. A method comprising:

causing an electrical stimulator to select a first electrode combination that, when active, delivers electrical stimulation from the electrical stimulator to a patient;

receiving user input indicating a parameter adjustment that indicates a shift to a second electrode combination having an electrode pattern that is substantially similar to that of the first electrode combination; and

causing the electrical stimulator to select the second electrode combination in response to the user input.

2. The method of claim 1, wherein the electrode pattern is identical with respect to a relative position and spacing of electrodes in the first and second electrode combinations.

3. The method of claim 1, wherein causing the electrical stimulator to select the first electrode combination comprises causing the electrical stimulator to select the first electrode combination that, when active, delivers voltage-based stimulation from the electrical stimulator to the patient.

4. The method of claim 1, wherein causing the electrical stimulator to select the first electrode combination comprises causing the electrical stimulator to select the first electrode combination that, when active, delivers current-based stimulation from the electrical stimulator to the patient.

5. The method of claim 1, wherein the parameter adjustment includes selection of one of a plurality of parameter values in a set of parameters to indicate the shift.

6. The method of claim 5, wherein each of the plurality of parameter values indicates a shift to a different electrode combination.

7. The method of claim 1, wherein the parameter adjustment includes adjustment from a first parameter value to a second parameter value to indicate the shift.

8. The method of claim 7, wherein the first parameter value and the second parameter value are a subset of a plurality of parameter values, and wherein each of the plurality of parameter values indicates a shift to a different electrode combination.

9. The method of claim 1, further comprising:

receiving additional user input indicating another parameter adjustment that indicates a shift to a third electrode combination having an electrode pattern that is substantially similar to that of both the first and second electrode combinations; and

causing the electrical stimulator to select the third electrode combination in response to the additional user input.

10. The method of claim 1, wherein the user input specifies selection of one of a plurality of alphanumeric characters or icons, and wherein each of the alphanumeric characters or icons indicates a shift to one of a plurality of electrode combinations, each of the plurality of electrode combinations having a substantially similar electrode pattern.

11. The method of claim 1, wherein each of the electrode combinations specifies a combination of electrodes arranged along a length of a lead, and the parameter adjustment indicates a shift to the second electrode combination in a first direction along the length of the lead.

12. The method of claim 11, further comprising:

receiving additional user input indicating another parameter adjustment that indicates a shift to a third electrode combination having an electrode pattern that is substantially similar to that of both the first and second electrode combinations; and

causing the electrical stimulator to select the third electrode combination in response to the additional user input,

wherein the other parameter adjustment indicates a shift to the third electrode combination in a second direction along the length of the lead, the second direction being opposite to the first direction.

13. The method of claim 1, wherein each of the electrode combinations specifies a combination of electrodes arranged along lengths of two or more leads, and the parameter adjustment indicates a shift to the second electrode combination in a first direction along the lengths of the leads.

14. The method of claim 13, further comprising:

receiving additional user input indicating another parameter adjustment that indicates a shift to a third electrode combination having an electrode pattern that is substantially similar to that of both the first and second electrode combinations; and

causing the electrical stimulator to select the third electrode combination in response to the additional user input,

wherein the other parameter adjustment indicates a shift to the third electrode combination in a second direction along the lengths of the leads, the second direction being opposite to the first direction.

15. The method of claim 1, further comprising:

in response to the user input, causing termination of delivery of electrical stimulation via the first electrode combination; and

causing initiation of delivery of electrical stimulation via the second electrode combination following termination of delivery of electrical stimulation via the first electrode combination.

The method of claim 15, wherein causing termination of delivery of electrical stimulation includes causing the electrical stimulator to immediately terminate delivery of electrical stimulation, and causing initiation of delivery of electrical stimulation via the second electrode combination includes causing the electrical stimulator to ramp up delivery of electrical stimulation to a desired level.

17. The method of claim 15, wherein causing termination of delivery of electrical stimulation includes causing the electrical stimulator to ramp down the delivery of electrical stimulation to a minimum level, and causing initiation of delivery of electrical stimulation via the second electrode combination includes causing the electrical stimulator to ramp up the delivery of electrical stimulation to a desired level.

18. The method of claim 1, wherein the parameter adjustment includes adjustment of a parameter that does not indicate a position of the second electrode combination, a direction of the shift, or polarities of electrodes within the second electrode combination.

19. The method of claim 1, further comprising causing delivery of the electrical stimulation to provide one of spinal cord stimulation, pelvic floor stimulation, gastric stimulation, brain stimulation, or peripheral nerve stimulation.

20. The method of claim 1, wherein the electrical stimulator includes an implantable stimulator.

21. The method of claim 1, further comprising prohibiting parameter adjustment if a program associated with the first electrode combination is indicated as not being shiftable.

22. The method of claim 1, wherein the shift from the first electrode combination to the second electrode combination is axial along a length of one or more adjacent leads, or axial along the length of one or more columns on a single lead.

23. The method of claim 1, wherein the shift from the first electrode combination to the second electrode combination is lateral across a set of two or more adjacent leads, or lateral across columns of electrodes on a single lead.

24. The method of claim 1, wherein receiving user input indicating the parameter adjustment comprises receiving user input from the patient.

25. The method of claim 1, wherein the first electrode combination and the second electrode combination have a common number of electrodes.

26. The method of claim 1, further comprising:

receiving additional user input to activate the shift to the second electrode combination; and

causing the electrical stimulator to deliver electrical stimulation to the patient through the second electrode combination in response to the additional user input.

27. An external programmer for an electrical stimulator, the programmer comprising:

a processor that generates a command to cause an electrical stimulator to select a first electrode combination that, when active, delivers electrical stimulation from the electrical stimulator to a patient; and

a user interface that receives user input indicating a parameter adjustment that indicates a shift to a second electrode combination having an electrode pattern that is substantially similar to that of the first electrode combination,

wherein the processor generates a command to cause the electrical stimulator to select the second electrode combination in response to the user input.

28. The programmer of claim 27, wherein the electrode pattern is identical with respect to a relative position and spacing of electrodes in the first and second electrode combinations.

29. The programmer of claim 27, wherein the electrical stimulation comprises voltage-based stimulation.

30. The programmer of claim 27, wherein the electrical stimulation comprises current-based stimulation.

31. The programmer of claim 27, wherein the parameter adjustment includes selection of one of a plurality of parameter values in a set of parameters to indicate the shift.

32. The programmer of claim 31, wherein each of the plurality of parameter values indicates a shift to a different electrode combination.

33. The programmer of claim 27, wherein the parameter adjustment includes adjustment from a first parameter value to a second parameter value to indicate the shift.

34. The programmer of claim 33, wherein the first parameter value and the second parameter value are a subset of a plurality of parameter values, and wherein each of the plurality of parameter values indicates a shift to a different electrode combination.

35. The programmer of claim 27, wherein the user interface receives additional user input indicating another parameter adjustment that indicates a shift to a third electrode combination having an electrode pattern that is substantially similar to that of both the first and second electrode combinations, and the processor generates a command to cause the electrical stimulator to select the third electrode combination in response to the additional user input.

36. The programmer of claim 27, wherein the user input specifies selection of one of a plurality of alphanumeric characters or icons, and wherein each of the alphanumeric characters or icons indicates a shift to one of a plurality of electrode combinations, each of the plurality of electrode combinations having a substantially similar electrode pattern.

37. The programmer of claim 27, wherein each of the electrode combinations specifies a combination of electrodes arranged along a length of a lead, and the parameter adjustment indicates a shift to the second electrode combination in a first direction along the length of the lead.

38. The programmer of claim 37, wherein the user interface receives additional user input indicating another parameter adjustment that indicates a shift to a third electrode combination having an electrode pattern that is substantially similar to that of both the first and second electrode combinations, and the processor generates a command to cause the electrical stimulator to select the third electrode combination in response to the additional user input, and wherein the other parameter adjustment indicates a shift to the third electrode combination in a second direction along the length of the lead, the second direction being opposite to the first direction.

39. The programmer of claim 27, wherein each of the electrode combinations specifies a combination of electrodes arranged along lengths of two or more leads, and the parameter adjustment indicates a shift to the second electrode combination in a first direction along the lengths of the leads.

40. The programmer of claim 39, wherein the user interface receives additional user input indicating another parameter adjustment that indicates a shift to a third electrode combination having an electrode pattern that is substantially similar to that of both the first and second electrode combinations, and the processor generates a command to cause the electrical stimulator to select the third electrode combination in response to the additional user input, and wherein the other parameter adjustment indicates a shift to the third electrode combination in a second direction along the lengths of the leads, the second direction being opposite to the first direction.

41. The programmer of claim 27, wherein the processor generates one or more commands that, in response to the user input, cause the electrical stimulator to terminate delivery of electrical stimulation via the first electrode combination, and initiate delivery of electrical stimulation via the second electrode combination following termination of delivery of electrical stimulation via the first electrode combination.

42. The programmer of claim 41, wherein the processor generates one or more commands that cause the electrical stimulator to immediately terminate delivery of electrical stimulation to terminate delivery of electrical stimulation via the first electrode combination, and ramp up delivery of electrical stimulation to a desired level to initiate delivery of electrical stimulation via the second electrode combination following termination of delivery of electrical stimulation via the first electrode combination.

43. The programmer of claim 41, wherein the processor generates one or more commands that cause the electrical stimulator to ramp down the delivery of electrical stimulation to a minimum level to terminate delivery of electrical stimulation via the first electrode combination, and ramp up the delivery of electrical stimulation to a desired level to initiate delivery of electrical stimulation via the second electrode combination following termination of delivery of electrical stimulation via the first electrode combination.

44. The programmer of claim 27, wherein the parameter adjustment includes adjustment of a parameter that does not indicate a position of the second electrode combination, a direction of the shift, or polarities of electrodes within the second electrode combination.

45. The programmer of claim 27, wherein the processor generates a command to cause delivery of the electrical stimulation to provide one of spinal cord stimulation, pelvic floor stimulation, gastric stimulation, brain stimulation, or peripheral nerve stimulation.

46. The programmer of claim 27, wherein the electrical stimulator includes an implantable stimulator.

47. The programmer of claim 27, wherein the processor causes the user interface to prohibit parameter adjustment if a program associated with the first electrode combination is indicated as not being shiftable.

48. The programmer of claim 27, wherein the shift from the first electrode combination to the second electrode combination is axial along a length of one or more adjacent leads, or axial along the length of one or more columns on a single lead.

49. The programmer of claim 27, wherein the shift from the first electrode combination to the second electrode combination is lateral across a set of two or more adjacent leads, or lateral across columns of electrodes on a single lead.

50. The programmer of claim 27, wherein the user input received by the user interface indicating the parameter adjustment comprises user input from the patient.

51. The programmer of claim 27, wherein the first electrode combination and the second electrode combination have a common number of electrodes.

52. The programmer of claim 27, wherein:

the user interface receives additional user input to activate the shift to the second electrode combination; and

the processor causes the electrical stimulator to deliver electrical stimulation to the patient through the second electrode combination in response to the additional user input.

53. An electrical stimulator system comprising:

an electrical stimulator that delivers electrical stimulation via combinations of electrodes carried by one or more leads; and

an external programmer comprising;

a processor that generates a command to cause an electrical stimulator to select a first electrode combination that, when active, delivers electrical stimulation from the electrical stimulator to a patient; and

a user interface that receives user input indicating a parameter adjustment that indicates a shift to a second electrode combination having an electrode pattern that is substantially similar to that of the first electrode combination,

wherein the processor generates a command to cause the electrical stimulator to select the second electrode combination in response to the user input.

54. The system of claim 53, wherein the electrode pattern is identical with respect to a relative position and spacing of electrodes in the first and second electrode combinations.

55. The system of claim 53, wherein the electrical stimulation comprises voltage-based stimulation.

56. The system of claim 53, wherein the electrical stimulation comprises current-based stimulation.

57. The system of claim 53, wherein the parameter adjustment includes selection of one of a plurality of parameter values in a set of parameters to indicate the shift.

58. The system of claim 57, wherein each of the plurality of parameter values indicates a shift to a different electrode combination.

59. The system of claim 53, wherein the parameter adjustment includes adjustment from a first parameter value to a second parameter value to indicate the shift.

60. The system of claim 59, wherein the first parameter value and the second parameter value are a subset of a plurality of parameter values, and wherein each of the plurality of parameter values indicates a shift to a different electrode combination.

61. The system of claim 53, wherein the user interface receives additional user input indicating another parameter adjustment that indicates a shift to a third electrode combination having an electrode pattern that is substantially similar to that of both the first and second electrode combinations, and the processor generates a command to cause the electrical stimulator to select the third electrode combination in response to the additional user input.

62. The system of claim 53, wherein the user input specifies selection of one of a plurality of alphanumeric characters or icons, and wherein each of the alphanumeric characters or icons indicates a shift to one of a plurality of electrode combinations, each of the plurality of electrode combinations having a substantially similar electrode pattern.

63. The system of claim 53, wherein each of the electrode combinations specifies a combination of electrodes arranged along a length of the one or more of the leads, and the parameter adjustment indicates a shift to the second electrode combination in a first direction along the length of the one or more leads.

64. The system of claim 63, wherein the user interface receives additional user input indicating another parameter adjustment that indicates a shift to a third electrode combination having an electrode pattern that is substantially similar to that of both the first and second electrode combinations, and the processor generates a command to cause the electrical stimulator to select the third electrode combination in response to the additional user input, and wherein the other parameter adjustment indicates a shift to the third electrode combination in a second direction along the length of the one or more leads, the second direction being opposite to the first direction.

65. The system of claim 63, wherein the one or more leads includes two or more leads, and each of the electrode combinations specifies a combination of electrodes arranged along lengths of the two or more leads, and the parameter adjustment indicates a shift to the second electrode combination in a first direction along the lengths of the leads.

66. The system of claim 65, wherein the user interface receives additional user input indicating another parameter adjustment that indicates a shift to a third electrode combination having an electrode pattern that is substantially similar to that of both the first and second electrode combinations, and the processor generates a command to cause the electrical stimulator to select the third electrode combination in response to the additional user input, and wherein the other parameter adjustment indicates a shift to the third electrode combination in a second direction along the lengths of the leads, the second direction being opposite to the first direction.

67. The system of claim 53, wherein the processor generates one or more commands that, in response to the user input, cause the electrical stimulator to terminate delivery of electrical stimulation via the first electrode combination, and initiate delivery of electrical stimulation via the second electrode combination following termination of delivery of electrical stimulation via the first electrode combination.

68. The system of claim 67, wherein the processor generates one or more commands that cause the electrical stimulator to immediately terminate delivery of electrical stimulation to terminate delivery of electrical stimulation via the first electrode combination, and ramp up delivery of electrical stimulation to a desired level to initiate delivery of electrical stimulation via the second electrode combination following termination of delivery of electrical stimulation via the first electrode combination.

69. The system of claim 67, wherein the processor generates one or more commands that cause the electrical stimulator to ramp down the delivery of electrical stimulation to a minimum level to terminate delivery of electrical stimulation via the first electrode combination, and ramp up the delivery of electrical stimulation to a desired level to initiate delivery of electrical stimulation via the second electrode combination following termination of delivery of electrical stimulation via the first electrode combination.

70. The system of claim 53, wherein the parameter adjustment includes adjustment of a parameter that does not indicate a position of the second electrode combination, a direction of the shift, or polarities of electrodes within the second electrode combination.

71. The system of claim 53, wherein the processor generates a command to cause delivery of the electrical stimulation to provide one of spinal cord stimulation, pelvic floor stimulation, gastric stimulation, brain stimulation, or peripheral nerve stimulation.

72. The system of claim 53, wherein the electrical stimulator includes an implantable stimulator.

73. The system of claim 53, wherein the processor causes the user interface to prohibit parameter adjustment if a program associated with the first electrode combination is indicated as not being shiftable.

74. The system of claim 53, wherein the shift from the first electrode combination to the second electrode combination is axial along a length of one or more adjacent leads, or axial along the length of one or more columns on a single lead.

75. The system of claim 53, wherein the shift from the first electrode combination to the second electrode combination is lateral across a set of two or more adjacent leads, or lateral across columns of electrodes on a single lead.

76. The system of claim 53, wherein the user input received by the user interface indicating the parameter adjustment comprises user input from the patient.

77. The system of claim 53, wherein the first electrode combination and the second electrode combination have a common number of electrodes.

78. The system of claim 53, wherein:

the user interface receives additional user input to activate the shift to the second electrode combination; and

the processor causes the electrical stimulator to deliver electrical stimulation to the patient through the second electrode combination in response to the additional user input.

79. A programmer for an electrical stimulator, the programmer comprising:

means for causing the electrical stimulator to select a first electrode combination that, when active, delivers electrical stimulation from the electrical stimulator to a patient;

means for receiving user input indicating a parameter adjustment that indicates a shift to a second electrode combination having an electrode pattern that is substantially similar to that of the first electrode combination; and

means for causing the electrical stimulator to select the second electrode combination in response to the user input.
