Abstract

Using a brokerage system, a consumer of services is matched with a service provider and a comprehensive health management plan is generated for the consumer during the consumer'"'"'s interaction with the brokerage system. The brokerage system uses a rules engine to identify one or more conditions of the consumer'"'"'s health and generate one or more follow-up actions based at least in part on the consumer'"'"'s identified health condition. The consumer is notified of at least one of the identified conditions and the generated follow-up actions.

Claims

1. A computer-implemented method comprises:

matching a consumer of services with a service provider through a brokerage system;

generating a comprehensive health management plan for the consumer during interaction with the brokerage system using a rules engine to;

identify one or more conditions of the consumer'"'"'s health, and

generate one or more follow-up actions based at least in part on the consumer'"'"'s identified health conditions; and

notifying the consumer of at least one of the identified conditions and the generated follow-up actions.

2. The computer-implemented method of claim 1 wherein generating further comprises:

collecting the consumer'"'"'s health care information; and

classifying the consumer'"'"'s collected health care information.

3. The computer-implemented method of claim 1 further comprising:

notifying the service provider of at least one of the identified conditions and the generated follow-up actions.

4. The method of claim 1 further comprising:

providing a graphical user interface that when rendered on a display displays for the service provider one or more proposed health conditions;

receiving a notification of the provider'"'"'s acceptance of the one or more proposed health conditions; and

consolidating the provider'"'"'s accepted one or more health conditions with prior health conditions.

5. The method of claim 4 further comprising:

notifying the consumer of the consolidated health conditions.

6. The method of claim 1 further comprising:

providing a graphical user interface that when rendered on a display displays for the service provider one or more proposed follow-up actions;

receiving a notification of the provider'"'"'s acceptance of the one or more proposed follow-up actions; and

consolidating the provider'"'"'s accepted one or more follow-up actions with prior follow-up actions.

7. The method of claim 6 further comprising:

notifying the consumer of the consolidated follow-up actions.

8. The method of claim 1 wherein the follow-up actions comprise at least one of recommendations to consult with one or more service providers, follow-up actions for the consumer to complete, and follow-up actions that are added to a consumer'"'"'s profile.

9. The method of claim 1 wherein notifying the consumer comprises notifying the consumer through a consumer identified communication channel.

10. The method of claim 1 wherein notifying the consumer comprises notifying the consumer using a consumer'"'"'s preferred communication channel.

11. A computer program product residing on a computer readable medium, the computer program product comprising instructions for causing a computer to:

match a consumer of services with a service provider through a brokerage system;

generate a comprehensive health management plan for the consumer during interaction with the brokerage system using a rules engine to;

identify one or more conditions of the consumer'"'"'s health, and

generate one or more follow-up actions based at least in part on the consumer'"'"'s identified health conditions; and

notify the consumer of at least one of the identified conditions and the generated follow-up actions.

12. The computer program product of claim 11 wherein instructions to generate further comprises instructions to:

collect the consumer'"'"'s health care information; and

classifying the consumer'"'"'s collected health care information.

13. The computer program product of claim 11 further comprising instructions to:

notify the service provider of at least one of the identified conditions and the generated follow-up actions.

14. The computer program product of claim 11 further comprising instructions to:

provide a graphical user interface that when rendered on a display displays for the service provider one or more proposed health conditions;

receive a notification of the provider'"'"'s acceptance of the one or more proposed health conditions; and

consolidate the provider'"'"'s accepted one or more health conditions with prior health conditions.

15. The computer program product of claim 14 further comprising instructions to:

notify the consumer of the consolidated health conditions.

16. The computer program product of claim 11 further comprising instructions to:

provide a graphical user interface that when rendered on a display displays for the service provider one or more proposed follow-up actions;

receive a notification of the provider'"'"'s acceptance of the one or more proposed follow-up actions; and

consolidate the provider'"'"'s accepted one or more follow-up actions with prior follow-up actions.

17. The computer program product of claim 16 further comprising instructions to:

notify the consumer of the consolidated follow-up actions.

18. The computer program product of claim 11 wherein the follow-up actions comprise at least one of recommendations to consult with one or more service providers, follow-up actions for the consumer to complete, and follow-up actions that are added to a consumer'"'"'s profile.

19. The computer program product of claim 11 wherein instructions to notify comprise instructions to notify the consumer through a consumer identified communication channel.

20. The computer program product of claim 11 wherein instructions to notify comprise instructions to notify the consumer using a consumer'"'"'s preferred communication channel.

21. An apparatus comprising:

a processor; and

a computer program product residing on a computer readable medium for producing an agenda for a consumer of services using a brokerage system capable of matching the consumer of services with a provider of services, the computer program product comprising instructions for causing the processor to;

match a consumer of services with a service provider through a brokerage system;

generate a comprehensive health management plan for the consumer during interaction with the brokerage system using a rules engine to;

identify one or more conditions of the consumer'"'"'s health, and

generate one or more follow-up actions based at least in part on the consumer'"'"'s identified health conditions; and

notify the consumer of at least one of the identified conditions and the generated follow-up actions.

22. The apparatus of claim 21 wherein instructions to generate further comprise instructions to:

collect the consumer'"'"'s health care information; and

classifying the consumer'"'"'s collected health care information.

23. The apparatus of claim 21 further comprising instructions to:

notify the service provider of at least one of the identified conditions and generated follow-up actions.

24. The apparatus of claim 21 further comprising instructions to:

provide a graphical user interface that when rendered on a display displays for the service provider one or more proposed health conditions;

receive a notification of the provider'"'"'s acceptance of the one or more proposed health conditions; and

consolidate the provider'"'"'s accepted one or more health conditions with prior health conditions.

25. The apparatus of claim 24 further comprising instructions to:

notify the consumer of the consolidated health conditions.

26. The apparatus of claim 21 further comprising instructions to:

provide a graphical user interface that when rendered on a display displays for the service provider one or more proposed follow-up actions;

receive a notification of the provider'"'"'s acceptance of the one or more proposed follow-up actions; and

consolidate the provider'"'"'s accepted one or more follow-up actions with prior follow-up actions.

27. The apparatus of claim 26 further comprising instructions to:

notify the consumer of the consolidated follow-up actions.
