Abstract

A comprehensive medication management system is disclosed. The comprehensive medication management system offers a number of compliance related services that may be offered to patients in to improve medication therapy compliance. The system identifies compliance barriers facing individual patents and recommends services designed to overcome those barriers. The system further provides feedback to patients to document their progress. The system proactively identifies patients who may benefit from compliance services and blocks their transactions until the services are explained. The point of sale block may be expanded to other classes of patients so that the transactions of other classes of patients blocked and other messages delivered or some other customized interactions carried out. The medication management system may serve as a central repository of information about a patient'"'"'s medication treatment program and the medication management system may provide quick and easy access to a patient'"'"'s entire medication history to facilitate consultations between a pharmacist and the patient. The medication management system may generate lists of tasks for pharmacists to perform in order to deliver the compliance related services to individual patients.

Claims

1. A medication management system comprising:

a database for storing medication management data including a patient'"'"'s prescription fill history over a reporting period; and

a processor programmed to calculate a compliance measure based on the patient'"'"'s prescription fill history over the reporting period and periodically generate a patient medication compliance report that includes the calculated compliance measure for the reporting period.

2. The system of claim 1 wherein the compliance measure is the patient'"'"'s medication possession ratio over the reporting period.

3. The system of claim 2 wherein the patient'"'"'s medication possession ratio is calculated by dividing a number of days in the reporting period for which the patient was in possession of the patient'"'"'s medication by a total number of days in the reporting period.

4. The system of claim 1 wherein the medication management data stored in the database include a service intervention date on which a medication management service became available to the patient, and wherein the processor is programmed to calculate the compliance measure for a first period before the service intervention date and a second period after the service intervention date.

5. The system of claim 1 wherein the compliance report generated by the processor includes a list of one or more maintenance medications prescribed for the patient, and a separate compliance measure calculated for each of the one or more maintenance medications.

6. The system of claim 5 wherein the compliance measure comprises the patient'"'"'s medication possession ratio calculated for each of the one or more maintenance medications.

7. The system of claim 6 wherein the processor calculates the patient'"'"'s medication possession ratio by dividing the number of days supply of a medication the patient has on hand for the reporting period by the number of days in the reporting period.

8. A method of providing medication therapy compliance feedback comprising:

tracking a patient'"'"'s prescription fill history over a reporting period;

calculating a medication possession ratio for the patient over the reporting period;

preparing a report including the patient'"'"'s medication possession ratio for the reporting period; and

presenting the report to the patient.

9. The method of claim 8 further comprising performing a service intervention in which a compliance assistance service is provided beginning on a service intervention date, the reporting period including a first period before the service intervention date and a second period after the service intervention date, the report including the patient'"'"'s medication possession ratio calculated for the first period and the second period.

10. The method of claim 9 wherein the service intervention includes asking the patient a plurality of questions to identify compliance barriers, and the method further comprises including questions associated with identified compliance barriers in the report.

11. The method of claim 8 wherein tracking the patient'"'"'s prescription fill history, calculating the patient'"'"'s medication possession ratio, preparing a report, and presenting the report to the patient are repeated on a periodic basis.

12. The method of claim 8 further comprising storing a report due date, and generating the report when the patient presents a prescription to be filled by the pharmacy on a date on or after the report due date.

13. A system for providing medication therapy compliance feedback comprising:

a database storing patient data including a patient'"'"'s prescription fill history and a service intervention date corresponding to a date on which a compliance assistance service is initiated on behalf of the patient;

a processor programmed to calculate a medication possession ratio for a first period before the intervention date and a second period after the intervention date based on the patient'"'"'s prescription fill history, the medication possession ratio calculated by dividing a number of days'"'"' supply of medication a patient possesses during a given period by the total number of days in the period; and

an interface for presenting a compliance report including the patient'"'"'s medication possession ratio for the first and second periods.

14. The system of claim 13 wherein the interface comprises a web-enabled workstation located in a pharmacy configured to communicate with the processor over a network.

15. The system of claim 14 wherein the database stores a patient compliance report due date, the interface configured to present the compliance report on a date on or after the patient compliance report due date.

The system of claim 15 wherein the interface and the processor are configured to perform a patient look up function to retrieve the patient'"'"'s data stored in the database when the patient contacts the pharmacy, the processor is programmed to generate the patient'"'"'s compliance report when a patient look-up is performed on the patient on a date on or after the patient'"'"'s compliance report due date.

17. The system of claim 13 wherein the database stores a question associated with the service intervention and corresponding to a compliance barrier that the compliance service is intended to address, and a patient response to the question, both the question and the patient response to the question included in the compliance report.

18. The system of claim 13 wherein the patient'"'"'s prescription fill history includes prescriptions for a plurality of maintenance medications and the compliance report includes the patient'"'"'s medication ratios for each of the plurality of maintenance medications.

19. A method of improving a patient'"'"'s compliance with a medication therapy, the method comprising:

tracking a measure of the patient'"'"'s compliance with the medication therapy over a first monitored period of time and a second monitored period of time; and

presenting a report to the patient comparing the measure of the patient'"'"'s compliance with the medication therapy over the first monitored period of time to the measure of the patient'"'"'s compliance with the medication therapy over the second monitored period of time.

20. The method of claim 19 wherein the measure of the patient'"'"'s compliance with the medication therapy comprises the patient'"'"'s medication possession ratio.

21. The method of claim 20 wherein the patient'"'"'s medication possession ratio is calculated by dividing a number of days'"'"' supply of a medication in the patient'"'"'s possession over a period of time by a total number of days in the period.

22. The method of claim 20 further comprising providing a compliance assistance service beginning on a service intervention date, the first monitored period of time occurring before the intervention date, and the second monitored period of time occurring after the intervention date.

23. The method of claim 20 further comprising establishing a reporting date and presenting the report to the patient on or after the reporting date.

24. The method of claim 23 wherein presenting the report to the patient comprises at least one of:

handing a copy of the report to the patient or the patient'"'"'s caregiver when the patient visits the pharmacy;

mailing, emailing or faxing the report to the patient of the patient'"'"'s caregiver;

or posting the report to a secure website accessible on-line by the patient, the patient'"'"'s caregiver, the patient'"'"'s physician or other authorized person.

25. The method of claim 20 further comprising performing a patient consultation including asking the patient a plurality of questions for identifying the patient'"'"'s medication compliance barriers, recording the patient'"'"'s responses to the questions, and including at least one of the questions and the patient'"'"'s response to the question on the report.
