- a kiosk system including three or more input devices receiving one or more pieces of health information and one or more pieces of personal identification information from the user, a display screen displaying a set of health data to the user, and a network connection transmitting the one or more pieces of health information and the one or more pieces of personal identification information;
a storage system receiving the one or more pieces of health information and the one or more pieces of personal identification information from the kiosk system and storing the one or more pieces of health information and the one or more pieces of personal identification information in a personal health record corresponding to the user; and
a network communicatively coupling the network connection of the kiosk system to the storage system.

The disclosed kiosk systems collect health data from a user to create and transmit a personal health record and to facilitate communication of the health data to remotely located health care professionals and researchers.
