Abstract

A drill guide assembly having an elongated body is configured with a first and second arm members extending from the elongated body. The second arm member is configured and adapted to be longitudinally movable along the elongated body towards and away from the first arm member for reducing fractured bones for repair. A guide housing provided on the second arm member is fitted with a removable sleeve. The removable sleeve is provided with a longitudinal bore for guiding a guide wire or a drill bit for drilling into the bones.

Claims

1. An orthopedic drill guide assembly comprising:

an elongated body having first and second ends;

a first arm member extending from said first end of the elongated body;

a second arm member extending from said elongated body and configured and adapted to be longitudinally movable along said elongated body;

one of the two arm members provided with a guide housing; and

a sleeve having a longitudinal bore, wherein said guide housing is configured and adapted to removably receive said sleeve.

2. The orthopedic drill guide assembly of claim 1, wherein said elongated body is radiolucent.

3. The orthopedic drill guide assembly of claim 2, wherein said elongated body is made from a polyetheretherketone polymer.

4. The orthopedic drill guide assembly of claim 1, wherein said first arm member is radiolucent.

5. The orthopedic drill guide assembly of claim 4, wherein said first arm member is made from a polyetheretherketone polymer.

6. The orthopedic drill guide assembly of claim 5, wherein said first arm member further comprises an outer-end tip that is radiopaque.

7. The orthopedic drill guide assembly of claim 1, wherein said first arm member is radiopaque.

8. The orthopedic drill guide assembly of claim 1, wherein said first arm member having an alignment notch provided near the first arm'"'"'s outer-end tip and said longitudinal bore of the sleeve is in longitudinal alignment with said alignment notch when the sleeve is received in the guide housing.

9. The orthopedic drill guide assembly of claim 1, wherein said second arm member engaging said elongated body by a ratcheting mechanism.

10. The orthopedic drill guide assembly of claim 9, wherein said ratcheting mechanism comprises a plurality of one-way ratcheting teeth provided on the elongated body and said second arm member configured and adapted with a ratchet release mechanism.

11. The orthopedic drill guide assembly of claim 1, wherein said elongated body has a non-cylindrical shape and said second arm member is configured and adapted to engage said-non-cylindrical shape whereby the second arm member is only longitudinally movable along the elongated body without any rotational movement about the elongated body.

12. The orthopedic drill guide assembly of claim 1, wherein said sleeve has a cylindrical body and said guide housing comprises a sleeve-receiving bore for receiving the cylindrical body of the sleeve.

13. The orthopedic drill guide assembly of claim 1, wherein said sleeve and said guide housing configured and adapted with a retention mechanism preventing the sleeve from falling out of the guide housing.

The orthopedic drill guide assembly of claim 1, wherein said guide housing is provided with at least one spike extending towards the first arm member whereby one or more bone pieces positioned between the first and second arm members is secured between said at least one spike and said first arm'"'"'s outer end by moving the second arm member towards the first arm member.

15. The orthopedic drill guide assembly of claim 1, wherein said elongated body having a non-symmetric lateral cross-sectional shape.

16. The orthopedic drill guide assembly of claim 15, wherein said non-symmetric lateral cross-sectional shape is a rectilinear shape with a chamfered corner.

17. An orthopedic drill guide assembly comprising:

an elongated body having first and second ends;

a first arm member extending from said first end of the elongated body;

a second arm member extending from said elongated body and configured and adapted to be longitudinally movable along said elongated body;

one of the two arm members provided with a guide housing; and

a sleeve having a longitudinal bore, wherein said guide housing is configured and adapted to removably receive said sleeve, wherein said guide housing provided with at least one spike extending towards the first arm member whereby one or more bone pieces positioned between the first and second arm members is secured between said at least one spike and said first arm'"'"'s outer end by moving the second arm member towards the first arm member.

18. A depth gage comprising:

a hollow elongated body having an opening at one end for receiving an elongated member having a length;

a spring-loaded piston provided within said hollow elongated body for urging against said elongated member received in said opening of the hollow elongated body, wherein when the elongated member is inserted into said opening and pushes the spring-loaded piston away from said opening, the distance traveled by the piston provides an indication about the length of the elongated member.

19. The depth gage of claim 18, wherein said indication about the length of the elongated member relates to a portion of the elongated member remaining outside the depth gage.

20. The depth gage of claim 18, wherein said elongated member is a K-wire.

21. A depth gage comprising:

a cannulated elongated body having first and second ends and a bore longitudinally extending through the length of the elongated body;

wherein said bore is closed at said first end of the elongated body and open at said second end of the elongated body, said open second end configured and adapted to receive an elongated member;

an elongated slot opening provided in the elongated body, said elongated slot extending longitudinally over a portion of the elongated body;

a piston provided within said bore configured to travel in longitudinal direction within said bore;

an elastically compressible member provided within said bore extending between said closed first end of the elongated body and said piston;

a graduated rule provided on the elongated body'"'"'s outer surface along said elongated slot opening;

an indicator connected to said piston through said elongated slot opening whereby when an elongated member is inserted into said open second end of the elongated body and urges said piston towards the closed first end of the elongated body compressing said elastically compressible member, said indicator moves along with the piston and indicates a value on the graduated rule,

wherein the elastically compressible member keeps the piston in contact with said elongated member.

22. A depth gage of claim 21, wherein the elongated member is a guide wire and said bore longitudinally extending through the length of the elongated body has a diameter sufficient to receive said guide wire.

23. A method for positioning a passageway in two neighboring bones comprising the steps of:

providing a device for guiding and positioning a guide wire or a drill to a point on a first bone surface where the passageway is to begin, said device comprising;

an elongated body having first and second ends;

a first arm member extending from said first end of the elongated body;

a second arm member extending from said elongated body and configured and adapted to be longitudinally movable along said elongated body;

one of the two arm members provided with a guide housing and the other of the two arm members having a generally curved tip portion; and

a sleeve having a longitudinal bore, wherein said guide housing is configured and adapted to removably receive said sleeve, wherein said guide housing is provided with at least one spike extending towards the first arm member whereby one or more bone pieces positioned between the first and second arm members is secured between said at least one spike and said first arm'"'"'s outer end by moving the second arm member towards the first arm member;

making an incision to access said two neighboring bones;

placing said first and second arm members around the two bones;

reducing the two neighboring bones by compressing the first and second arm members;

drilling a guide wire through the guide housing of the second arm member and into the two neighboring bones to a desired depth which is until the guide wire hits the first arm member, wherein the guide wire'"'"'s alignment is from the guide housing to the first arm member;

measuring said depth of the guide wire for determining proper length for a bone screw for securing the two bones;

drilling a cannulated drill bit into the two bones to a drill depth using the guide wire as a guide; and

screwing a bone screw into the two bones, said bone screw having a length matching the measured depth of the guide wire.

24. A method for repairing a Lisfranc injury between a second metatarsal and medial cuneiform comprising the steps of:

providing a drill guide assembly device comprising;

an elongated body having first and second ends;

a first arm member extending from said first end of the elongated body;

a second arm member extending from said elongated body and configured and adapted to be longitudinally movable along said elongated body;

one of the two arm members provided with a guide housing and the other of the two arm members having a generally curved tip portion; and

a sleeve having a longitudinal bore, wherein said guide housing is configured and adapted to removably receive said sleeve, wherein said guide housing is provided with at least one spike extending towards the first arm member whereby one or more bone pieces positioned between the first and second arm members is secured between said at least one spike and said first arm'"'"'s outer end by moving the second arm member towards the first arm member;

making an incision to access said Lisfranc injury site;

placing said curved tip portion of one arm member between the second metatarsal and third metatarsal base;

placing said at least one spike provided on said other arm member on the medial side of the medial cuneiform in desired alignment with the curved tip portion by closing the two arm members;

reducing the second metatarsal and the medial cuneiform by compressing the two arm members;

inserting a first sleeve into the guide housing, said first sleeve configured and adapted for receiving a guide wire;

drilling a guide wire through the medial cuneiform and the second metatarsal in an alignment guided by the first sleeve to a desired depth which is until the guide wire hits the curved tip portion;

removing the first sleeve and measuring said depth of the guide wire for determining a proper length for a bone screw for securing the second metatarsal and the medial cuneiform;

inserting a second sleeve into the guide housing, said second sleeve configured and adapted for receiving a cannulated drill bit;

drilling a hole through the medial cuneiform and the second metatarsal using the guide wire as a guide;

securing the medial cuneiform to the second metatarsal by screwing a bone screw into said drilled hole; and

removing the drill guide assembly device.

25. The method of claim 24, wherein after placing said at least one spike on the medial side of the medial cuneiform in desired alignment with the curved tip portion, further comprising a step of verifying the desired alignment of the two arm members by inserting a blunt trocar in the guide housing and verifying the desired alignment with an external imaging technology before reducing the second metatarsal and the medial cuneiform.

26. The method of claim 24, wherein after drilling the guide wire into the bones, further comprising a step of verifying said alignment of the guide wire by an external imaging technology.

27. The method of claim 24, further comprising verifying said drill depth by external imaging after drilling a cannulated drill bit into the fracture bones.

28. The method of claim 24, wherein said elongated body of the drill guide assembly is radiolucent.

29. The method of claim 28, wherein said elongated body is made from a polyetheretherketone polymer.

30. The method of claim 24, wherein said arm member provided with the curved tip portion is radiolucent.

31. The method of claim 30, wherein said radiolucent arm member is made from a polyetheretherketone polymer.

32. The method of claim 30, wherein said curved tip portion is radiopaque.

33. The method of claim 24, wherein said curved tip portion having an alignment notch.

34. The method of claim 24, wherein said second arm member engaging said elongated body by a ratcheting mechanism.

35. The method of claim 34, wherein said ratcheting mechanism comprises a plurality of one-way ratcheting teeth provided on the elongated body and said second arm member configured and adapted with a ratchet release mechanism.

36. The method of claim 24, wherein said elongated body has a non-cylindrical shape and said second arm member is configured and adapted to engage said-non-cylindrical shape whereby the second arm member is only longitudinally movable along the elongated body without any rotational movement about the elongated body.

37. The method of claim 24, wherein said sleeve has a cylindrical body and said guide housing comprises a sleeve-receiving bore for receiving the cylindrical body of the sleeve.

38. The method of claim 24, wherein said sleeve and said guide housing configured and adapted with a retention mechanism preventing the sleeve from falling out of the guide housing.

39. A surgical drill guide kit for repairing Lisfranc injury comprising:

a drill guide assembly comprising;

an elongated body having first and second ends;

a first arm member extending from said first end of the elongated body;

a second arm member extending from said elongated body and configured and adapted to be longitudinally movable along said elongated body;

one of the two arm members provided with a guide housing, said guide housing configured and adapted to removably receive a sleeve, wherein said guide housing provided with at least one spike extending towards the first arm member whereby one or more bone pieces positioned between the first and second arm members is secured between said at least one spike and said first arm'"'"'s outer end by moving the second arm member towards the first arm member;

one or more sleeves;

one or more K-wires;

one or more bone screws; and

a depth gage.

40. The surgical drill guide kit of claim 39 further comprising a clamp.

41. The surgical drill guide kit of claim 40, further comprising a blunt trocar.
