Abstract

Adventitious lung sounds indicative of lung congestion are detected using an implantable sensor. The sensor is adapted to be positioned adjacent to a pulmonary system and to output signals indicative of lung sounds in response to pulmonary system activity. A controller receives the signals and processes the signals to detect the presence of adventitious lung sounds. A respiratory cycle sensor operating in conjunction with the lung-sound sensor enables classification of an adventitious lung sound according to its time occurrence within the respiratory cycle. Posture sensing in conjunction with lung-sound sensing provides valuable additional information as to the severity of the lung congestion.

Claims

1. A system, comprising:

an implantable housing comprising;

a sensor adapted for ambulatory operation in a patient and adapted to be positioned adjacent to a pulmonary system of the patient, the sensor adapted to output signals indicative of lung sounds; and

an input/output interface;

filtering circuitry adapted to filter the sensor output signals by removing signal content unrelated to adventitious lung sounds;

detection circuitry adapted to receive filtered sensor output signals from the filtering circuitry, the detection circuitry further adapted to detect presence of adventitious lung sounds based on a comparison between the filtered sensor output signals and a criteria indicative of adventitious lung sounds;

a processor coupled to memory and adapted to record data associated with the adventitious lung sounds; and

an external system comprising an input/output interface and adapted to communicatively couple with the input/output interface of the implantable housing.

2. The system of claim 1, comprising a remote system, the remote system comprising a communications interface adapted to communicatively couple with the input/output interface of the external system.

The system of claim 2, wherein the remote system or the external system is adapted to generate trend data using the sensor output signals and data associated with the adventitious lung sounds, the remote system or the external system further adapted to detect changes in the trend data indicating onset of worsening of the patient'"'"'s heart failure.

4. The system of claim 2, wherein the processor is disposed in the remote system or the external system.

5. The system of claim 1, wherein the processor is disposed in the implantable housing.

6. The system of claim 1, wherein the processor is further adapted to associate a degree parameter with each adventitious lung sound, the degree parameter indicative of severity of pulmonary congestion.

7. The system of claim 1, comprising a posture detector adapted to produce data indicative of the patient'"'"'s posture, wherein the processor is adapted to record the detection data and the posture detection data in the memory.

8. The system of claim 1, comprising a display adapted to provide a visual display of the lung sound data and the detection data.

9. The system of claim 1, wherein one of the detection circuitry, the processor, and the external system is adapted to generate an alert in response to the detection data.

10. The system of claim 1, comprising an audio output device adapted to generate audio outputs representative of the lung sounds and the adventitious lung sounds.

11. The system of claim 1, comprising an audio output device adapted to generate audio outputs representative of the lung sounds and the adventitious lung sounds.

12. A system, comprising:

an implantable housing comprising;

a lung-sound sensor adapted for ambulatory operation in a patient and adapted to be positioned adjacent to a pulmonary system of the patient, the lung-sound sensor adapted to output signals indicative of lung sounds; and

an input/output interface;

a respiration sensor adapted to output signals indicative of a respiratory cycle of the patient'"'"'s pulmonary system;

detection circuitry adapted to receive the output signals of the lung-sound sensor and the respiration sensor, respectively, the detection circuitry further adapted to monitor for adventitious lung sounds only during a predetermined portion of the respiratory cycle and to generate detection data in response to detecting adventitious lung sounds during the predetermined respiratory cycle portion;

a processor coupled to memory and adapted to record the detection data; and

an external system comprising an input/output interface and adapted to communicatively couple with the input/output interface of the implantable housing.

13. The system of claim 12, wherein the detection circuitry is adapted to classify an adventitious lung sound according to its time occurrence within the respiratory cycle.

14. The system of claim 12, wherein the detection circuitry is adapted to classify an adventitious lung sound as between at least two of a late inspiratory rale, an early inspiratory rale, a fine rale, and a course rale.

15. The system of claim 12, comprising a posture detector adapted to produce data indicative of the patient'"'"'s posture, wherein the processor is adapted to record the detection data and the posture detection data in the memory.

16. The system of claim 12, comprising a display adapted to provide a visual display of the detection data and respiratory cycle data.

17. The system of claim 12, wherein one of the detection circuitry, the processor, and the external system is adapted to generate an alert in response to the detection data.

18. The system of claim 12, comprising an audio output device adapted to generate audio outputs representative of the lung sounds and the adventitious lung sounds.

19. The system of claim 12, wherein the external system is adapted to generate trend data using the lung sound data and data associated with the adventitious lung sounds, the external system further adapted to detect changes in the trend data indicating onset of worsening of the patient'"'"'s heart failure.

20. The implantable system of claim 12, wherein the processor is further adapted to associate a degree parameter with each adventitious lung sound, the degree parameter indicative of severity of pulmonary congestion.
