Abstract

A method for the treatment of obesity or other disorders by electrical activation or inhibition of the sympathetic nervous system is disclosed. This activation or inhibition can be accomplished by stimulating the greater splanchnic nerve or other portion of the sympathetic nervous system using an electrode. This nerve activation can result in reduced food intake and increased energy expenditure.

Claims

1. A method of reducing fat in a liver of a patient, comprising:

providing a pulse generator that emits electrical pulses in a programmable stimulation pattern;

wherein the stimulation pattern is programmable to reduce fat in the liver from electrical activation of a sympathetic nerve of the patient, and wherein the stimulation pattern is further programmable to keep the patient'"'"'s blood pressure within safe limits during the electrical activation of the splanchnic nerve; and

electrically activating the sympathetic nerve of the patient with said pulse generator using said stimulation pattern so as to reduce fat in the liver and keep the patient'"'"'s blood pressure within safe limits.

2. The method of claim 1, wherein the splanchnic nerve is selected from the greater splanchnic nerve, lesser splanchnic nerve and least splanchnic nerve.

3. The method according to claim 1, wherein the pulse generator is electrically connected to electrodes.

4. The method according to claim 3, wherein the electrodes are electrically connected through vascular structures in proximity to the liver.

5. The method according to claim 1, wherein the programmable stimulation pattern includes an “

off”

time and an “

on”

time.

6. The method according to claim 5, wherein the off time is greater than the on time.

7. The method according to claim 1, wherein fatty acids are liberated.

8. The method according to claim 1, wherein the electrical activation comprises a frequency of from about 0.5 Hz to about 300 Hz.

9. The method according to claim 1, wherein the electrical activation comprises a pulse width of from about 10 microseconds to about 1,000 microseconds.

10. The method according to claim 1, wherein glucose or glycogen is released from the liver.

11. The method according to claim 1, wherein the patient is diabetic.

12. A method of reducing fat in the liver of a patient comprising:

electrically stimulating a postganglionic branch of the splanchnic nerve with a stimulation pattern, wherein the postganglionic branch of the splanchnic nerve stimulates the liver and wherein fat is reduced in the liver.

13. The method of claim 12, wherein the postganglionic branch is derived from a celiac ganglion.

14. The method according to claim 12, wherein the pulse generator is electrically connected to electrodes.

15. The method according to claim 14, wherein the electrodes are electrically connected through vascular structures in proximity to the liver.

16. The method according to claim 12, wherein the programmable stimulation pattern includes an “

off”

time and an “

on”

time.

17. The method according to claim 16, wherein the “

off”

time is greater than the “

on”

time.

18. The method according to claim 17, wherein fatty acids are liberated.

19. The method according to claim 12, wherein the electrical stimulation comprises a frequency of from about 0.5 Hz to about 300 Hz.

20. The method according to claim 12, wherein the electrical stimulation comprises a pulse width of from about 10 microseconds to about 1,000 microseconds.

21. The method according to claim 12, wherein glucose or glycogen is released from the liver.

22. The method according to claim 12, wherein the patient is diabetic.

23. A method for reducing fat in a liver of a patient comprising:

electrically coupling an electrode to a splanchnic nerve in a mammal at a location above the diaphragm of the mammal; and

electrically activating the splanchnic nerve, wherein the electrically activating is performed in a stimulation pattern for a first time period within a period of about 24 hours and ceasing the electrical activation of the splanchnic nerve for a second time period within the period of about 24 hours, wherein the stimulation pattern includes a stimulation intensity pattern adapted and configured to result in net reduction in fat in the liver over a period of about 4 to 8 weeks.

24. The method of claim 23, further comprising repeating the steps of electrically activating and ceasing the electrical activation.

25. The method of claim 23, wherein the first time period plus the second time period equals about 24 hours.

26. The method of claim 23, further comprising varying the stimulation intensity over time.

27. The method of claim 23, further comprising increasing the stimulation intensity daily.

28. The method of claim 23, further comprising creating a unidirectional action potential in the splanchnic nerve.

29. The method of claim 23, further comprising creating an anodal block in the splanchnic nerve.

30. The method of claim 23, further comprising administering an alpha sympathetic receptor blocker.

31. The method of claim 23, further comprising titrating the electrical activating to the plasma level of a catecholamine achieved during therapy.

32. The method of claim 23, wherein the electrically activating is performed about every three hours, for durations of between about 2 to 30 minutes.

33. The method of claim 23, wherein the electrically activating is delivered at intervals coinciding with meal times.

34. The method of claim 23, wherein the electrically activating lasts from between 1 and 3 hours.

35. The method of claim 23, wherein the electrically activating starts just prior to the meal.

36. The method of claim 23, wherein the electrically activating stimulation intensity is increased in a stepwise manner.

37. The method of claim 23, in which the stimulation pattern includes increasing the stimulation intensity in increments which allow for habituation to the increased intensity.

38. The method of claim 37, in which the stimulation intensity is incremented by about 0.5 mA daily.
