Abstract

An absorbable screw fastener and a method of firing with an applicator capable of applying a surgical fastener to tissue in order to form tissue connection to secure objects to tissue, the fastener including a body portion having a helical thread, a head portion disposed at the proximal end of the body portion. The head portion includes a driver receiving configuration on its outer surface. The screw fastener further includes a cannulated center lumen with an opening extending from the head portion through the longitudinal length of the body portion.

33. An absorbable screw fastener, comprising:

a body portion defining a longitudinal axis and having a proximal end and a distal end, the body portion having a helical thread formed thereon, the helical thread having a major diameter and a minor diameter; and

a head portion disposed at the proximal end of the body portion, the head portion having a driver receiving slot formed in an outer radial surface of the head portion, the helical thread having a helical thread slot corresponding to and in alignment with the driver receiving slot, wherein a radially-innermost surface of the helical thread slot is between the major diameter and the minor diameter of the helical thread.

34. The screw fastener of claim 33, wherein the head portion has an outer diameter substantially equal to the major diameter of the helical thread.

35. The screw fastener of claim 33, wherein the head portion has a plurality of driver receiving slots formed in its outer radial section, and the helical thread has a plurality of helical thread slots, each one of the plurality of helical thread slots corresponding to and in alignment with one of the plurality of driver receiving slots.

36. The screw fastener of claim 33, wherein the body portion includes a center shaft that defines the minor diameter of the helical thread.

37. The screw fastener of claim 33, wherein the major diameter of the helical thread varies between the distal end and the proximal end of the body portion.

38. The screw fastener of claim 33, wherein the minor diameter of the helical thread varies between the distal end and the proximal end of the body portion.

39. The screw fastener of claim 33, wherein the body portion and the head portion define a lumen extending longitudinally through the screw fastener.

40. The screw fastener of claim 33, wherein the distal end of the body portion is angled relative to the longitudinal axis of the body portion.

41. The screw fastener of claim 33, wherein the distal end of the body portion defines an atraumatic blunt tip.

42. The screw fastener of claim 35, wherein the plurality of driver receiving slots are formed so as to be diametrically opposed on the outer radial surface of the head portion, and the plurality of helical thread slots are formed so as to be diametrically opposed on the body portion.

43. The screw fastener of claim 33, wherein said driver receiving slot is a flat.

44. The screw fastener of claim 33, wherein said helical thread slot is a flat.

45. The screw fastener of claim 33 and a fastener applier for applying the screw fastener, wherein the fastener applier has a driver configured to be received in the helical thread slot and driver receiving slot of the screw fastener.

46. The screw fastener and fastener applier of claim 45, wherein the fastener applier is configured to apply a plurality of screw fasteners, the plurality of screw fasteners arranged longitudinally in series within the fastener applier.

47. The screw fastener and fastener applier of claim 45, wherein the driver is received in the driver receiving slot of each fastener.

48. The screw fastener and fastener applier of claim 45, wherein the driver is received in the helical thread slot of each fastener.

49. An absorbable screw fastener, comprising:

a body portion defining a longitudinal axis and having a proximal end and a distal end, the body portion including a center shaft on which is formed a helical thread, the helical thread defining at its outer radius an outer distance and defining an inner distance where the helical thread meets the center shaft, the helical thread including a surface between the inner and outer distances that defines a helical thread slot; and

a head portion disposed at the proximal end of the body portion, the head portion having an outer radial surface in which is formed a driver receiving slot, the helical thread slot and the driver receiving slot being longitudinally aligned relative to each other.

50. The screw fastener of claim 49, wherein the helical thread includes a surface that defines the helical thread slot.

51. The screw fastener of claim 49, wherein the surface that defines the driver receiving slot is one of planar and flattened.

52. The screw fastener of claim 49, wherein the head portion includes a surface that defines the driver receiving slot.

53. The screw fastener of claim 52, wherein the surface that defines the driver receiving slot is one of planar and flattened.

54. The screw fastener of claim 49, wherein the screw fastener is formed of a bioabsorbable material.

55. The screw fastener of claim 54, wherein the bioabsorbable material is formed from a polylactide.

56. The screw fastener of claim 54, wherein the bioabsorbable material is selected from the group consisting of polyglycolic acid, poly-Glycolide (PGA), polylactic acid (PLA), L1 (18/82 poly-Glycolide-co-L-lactide), L4 (42/58 poly-Glycolide-co-L-lactide), and PGB (63/37 poly-Glycolide-co-Trimethylene Carbonate).

57. The screw fastener of claim 52, wherein the surface that defines the driver receiving slot includes a shoulder.

58. The screw fastener of claim 49, wherein the outer radial surface of the head portion has a diameter substantially equal to the outer distance of the helical thread.

59. The screw fastener of claim 49, wherein the head portion has a plurality of driver receiving slots formed in its outer radial surface, and the helical thread has a plurality of helical thread slots, each one of the plurality of helical thread slots corresponding to and in alignment with one of the plurality of driver receiving slots.

60. The screw fastener of claim 49, wherein the outer distance of the helical thread varies between the distal end and the proximal end of the body portion.

61. The screw fastener of claim 49, wherein the inner distance of the helical thread varies between the distal end and the proximal end of the body portion.

62. The screw fastener of claim 49, wherein the body portion and the head portion define a lumen extending longitudinally through the screw fastener.

63. The screw fastener of claim 62, wherein the lumen is defined by the center shaft.

64. The screw fastener of claim 49, wherein the distal end of the body portion is angled relative to the longitudinal axis of the body portion.

65. The screw fastener of claim 59, wherein the plurality of driver receiving slots are formed so as to be diametrically opposed on the outer radial surface of the head portion, and the plurality of helical thread slots are formed so as to be diametrically opposed on the body portion.

66. The screw fastener of claim 49, wherein the driver receiving slot includes structure configured to transmit torque.

67. The screw fastener of claim 66, wherein the structure configured to transmit torque is a shoulder.

68. The screw fastener of claim 49, wherein said driver receiving slot is a flat.

69. The screw fastener of claim 49, wherein said helical thread slot is a flat.

70. An absorbable screw fastening system, comprising:

a fastener applier having a driver; and

a plurality of absorbable screw fasteners arranged longitudinally in series within the fastener applier, each one of the plurality of absorbable screw fasteners comprising;

a body portion defining a longitudinal axis and having a proximal end and a distal end, the body portion including a center shaft on which is formed a helical thread, the helical thread defining at its outer radius an outer distance and defining an inner distance where the helical thread meets the center shaft, the helical thread including a surface between the inner and outer distances that defines a helical thread slot; and

a head portion disposed at the proximal end of the body portion, the head portion having an outer radial surface in which is formed a head slot, the helical thread slot and the head slot being longitudinally aligned relative to each other so as to receive the driver.

71. The screw fastening system of claim 70, wherein the helical thread includes a surface that defines the helical thread slot.

72. The screw fastening system of claim 71, wherein the surface that defines the driver receiving slot is one of planar and flattened.

73. The screw fastening system of claim 70, wherein the head portion includes a surface that defines the head slot.

74. The screw fastening system of claim 73, wherein the surface that defines the head slot is one of planar and flattened.

75. The screw fastening system of claim 73, wherein the surface that defines the head slot includes a shoulder.

76. The screw fastening system of claim 70, wherein the outer radial surface of the head portion has a diameter substantially equal to the outer distance of the helical thread.

77. The screw fastening system of claim 70, wherein the head portion has a plurality of head slots formed in its outer radial surface, and the helical thread has a plurality of helical thread slots, each one of the plurality of helical thread slots corresponding to and in alignment with one of the plurality of head slots.

78. The screw fastening system of claim 70, wherein the outer distance of the helical thread varies between the distal end and the proximal end of the body portion.

79. The screw fastening system of claim 70, wherein the inner distance of the helical thread varies between the distal end and the proximal end of the body portion.

80. The screw fastening system of claim 70, wherein the body portion and the head portion define a lumen extending longitudinally through the screw fastener.

81. The screw fastening system of claim 70, wherein the distal end of the body portion is angled relative to the longitudinal axis of the body portion.

82. The screw fastening system of claim 77, wherein the plurality of head slots are formed so as to be diametrically opposed on the outer radial surface of the head portion, and the plurality of helical thread slots are formed so as to be diametrically opposed on the body portion.

83. The screw fastening system of claim 70, wherein the head slot includes structure configured to transmit torque.

84. The screw fastening system of claim 83, wherein the structure configured to transmit torque is a shoulder.

85. The screw fastener system of claim 70, wherein the driver is received in the driver receiving slot of each fastener.

86. The screw fastener system of claim 85, wherein the driver is received in the helical thread slot of each fastener.

87. The screw fastener system of claim 70, wherein the fastener applier includes a pilot extending through the plurality of screw fasteners.

88. The screw fastener system of claim 87, wherein the pilot extends through a lumen defined in each of the plurality of fasteners.

89. The screw fastener system of claim 87, wherein the pilot defines a tip portion for piloting the fasteners through mesh and tissue.

90. The screw fastener system of claim 89, wherein the tip portion of the pilot is sharpened.

91. A method of performing a surgical procedure, the method comprising the steps of:

providing an absorbable screw fastening system, including;

a fastener applier having a driver; and

a plurality of absorbable screw fasteners arranged longitudinally in series within the fastener applier, each one of the plurality of absorbable screw fasteners including;

a body portion defining a longitudinal axis and having a proximal end and a distal end, the body portion having a helical thread formed thereon, the helical thread having a major diameter and a minor diameter; and

a head portion disposed at the proximal end of the body portion, the head portion having a driver receiving slot formed in an outer radial surface of the head portion, the helical thread having a helical thread slot corresponding to and in alignment with the driver receiving slot, wherein a radially-innermost surface of the helical thread slot is between the major diameter and the minor diameter of the helical thread;

applying a tip of the fastener driver to a tissue site; and

actuating the fastener driver to advance a pilot thereof into the tissue site and to then drive a fastener into the tissue.

92. The method of claim 91, further comprising the step of advancing the pilot and driving the fastener into tissue upon a single stroke of the fastener driver.

93. The method of claim 91, wherein the step of advancing the pilot into the tissue site includes a tip portion of the pilot defining an engagement space in the tissue site.
