Abstract

A system and method for generating and/or updating a personal/individual health record. Inputs of data to the system may come from diverse sources including, but not limited to, patient questionnaires, insurance company (or other payor) claims data, hospitals, clinics and other institutional providers, and individual physicians and physicians'"'"' offices.

Claims

1. A personal/individual/electronic health record (“

PHR”

) system comprising;

a messaging facility configured to establish a secure connection with a computer system of a health care provider, wherein the messaging facility is adapted to query the computer system for electronic health care data of a plurality of patients using master patient identifications that are unique to each respective patient across multiple health care providers, wherein the master patient identifications are not social security numbers;

a PHR population engine configured to assemble a plurality of personal/individual/electronic health records for the plurality of patients based, at least in part, on the electronic health care data received from the messaging facility;

a web-based portal configured to provide a patient with access to his personal/individual/electronic health record over the Internet; and

a data analysis module configured to analyze patterns in the plurality of personal/individual/electronic health records across multiple patients.

The PHR system of claim 1, wherein the data analysis module is configured to profile quality of care for a plurality of health care providers by analyzing patient outcomes associated with the health care providers in the plurality of personal/individual/electronic health records across multiple patients.

3. The PHR system of claim 2, wherein the data analysis module is configured to rank quality of care for the health care providers based on the analysis of the plurality of personal/individual/electronic health records.

The PHR system of claim 1, wherein the data analysis module is configured to profile cost of care of a plurality of health care providers by analyzing the cost charged by the health care providers for a plurality of medical procedures in the plurality of personal/individual/electronic health records across multiple patients.

5. The PHR system of claim 4, wherein the data analysis module is configured to rank cost of care of the health care providers for the plurality of medical procedures based on the analysis of the plurality of personal/individual/electronic health records.

6. The PHR system of claim 1, wherein the data analysis module is configured to establish a list of preferred health care providers based on objective measures for quality and cost of care determined by an analysis of the plurality of personal/individual/electronic health records across multiple patients.

7. The PHR system of claim 1, wherein the data analysis module is configured to perform population studies by analyzing the plurality of personal/individual/electronic health records across multiple patients.

8. The PHR system of claim 7, wherein the population studies include an analysis of one or more of progress of condition, impact of co-morbidities on an underlying condition, effectiveness of procedures on a condition, and effectiveness of medications on a condition.

9. The PHR system of claim 7, wherein the data analysis module is configured to determine an outcomes measurement based on the population studies of the plurality of personal/individual/electronic health records.

10. PHR system of claim 9, wherein the data analysis module is configured to analyze the plurality of personal/individual/electronic health records to identify an effectiveness of a medication for a condition.

11. The PHR system of claim 9, wherein the data analysis module is configured to analyze the plurality of personal/individual/electronic health records to identify an effectiveness of a surgery for a condition.

12. The PHR system of claim 11, wherein the data analysis module is configured to analyze the plurality of personal/individual/electronic health records to measure complications of types of surgeries.

13. The PHR system of claim 9, wherein the data analysis module is configured to analyze the plurality of personal/individual/electronic health records to determine the effectiveness of one or more wellness programs.

14. The PHR system of claim 1, where the health care provider includes a hospital, a physician, a research associate, a pharmaceutical provider, a laboratory, or a pharmacy benefits manager.
