) system comprising;
a messaging facility configured to establish a secure connection with a computer system of a health care provider, wherein the messaging facility is adapted to query the computer system for electronic health care data of a plurality of patients using master patient identifications that are unique to each respective patient across multiple health care providers, wherein the master patient identifications are not social security numbers;
a PHR population engine configured to assemble a plurality of personal/individual/electronic health records for the plurality of patients based, at least in part, on the electronic health care data received from the messaging facility;
a web-based portal configured to provide a patient with access to his personal/individual/electronic health record over the Internet; and
a data analysis module configured to analyze patterns in the plurality of personal/individual/electronic health records across multiple patients.
