Abstract

Cardiac devices and methods that select pacing rates for automatic threshold tests based on a patient'"'"'s hemodynamic need. A sensor-indicated pacing rate corresponding to a patient'"'"'s hemodynamic need is determined. A test pacing rate is selected from either the sensor-indicated rate or another rate. Capture threshold testing is performed using the selected pacing rate.

Claims

1. A cardiac rhythm management system, comprising:

a sensing system configured to sense cardiac signals;

a pacing device operable at least at a sensor-indicated pacing rate and a non-sensor-indicated pacing rate; and

a processor coupled to the sensing system and the pacing device, the processor configured to operate the pacing device in a current pacing mode before a capture threshold test and determine a sensor-indicated pacing rate corresponding to the patient'"'"'s sensed hemodynamic need, the processor configured to select either the sensor-indicated pacing rate or the non-sensor-indicated pacing rate as a test pacing rate, switch operation of the pacing device from the current pacing mode to the capture threshold test, and perform the capture threshold test using the test pacing rate.

The system of claim 1, wherein the processor is configured to select the sensor-indicated pacing rate if the sensor-indicated pacing rate is greater than the non-sensor-indicated pacing rate, and select the non-sensor-indicated pacing rate if the non-sensor-indicated pacing rate is greater than the sensor-indicated pacing rate.

3. The system of claim 1, wherein the processor is configured to use a shortened AV-delay if the current pacing mode comprises a tracking mode.

4. The system of claim 1, wherein the processor is configured to select the test pacing rate to reduce fusion beats during the capture threshold test.

5. The system of claim 1, wherein the processor is configured to select the test pacing rate to increase a likelihood of captured beats during the capture threshold test.

6. The system of claim 1, wherein the processor is configured to select the test pacing rate to reduce a pacing rate change when the capture threshold test is initiated.

7. The system of claim 1, wherein the current pacing mode is a rate-adaptive mode, and the processor is configured to select the test pacing rate to reduce a pacing rate change when switching from the current pacing mode to the capture threshold test.

8. The system of claim 1, wherein the processor is configured to select the test pacing rate to reduce a pacing rate change when the capture threshold test is terminated.

9. The system of claim 1, wherein the processor is configured to adapt the test pacing rate to meet the patient'"'"'s hemodynamic need when performing the capture threshold test.

10. A cardiac rhythm management system operable for performing a capture threshold test, comprising:

a sensing system configured to sense cardiac signals;

a pacing device configured to provide cardiac pacing at least at a sensor-indicated pacing rate, a predetermined rate, and a pacing rate determined based on a heart rate of a patient before the capture threshold test; and

a processor coupled to the sensing system and the pacing device, the processor configured to operate the pacing device in a current pacing mode before the capture threshold test and determine a sensor-indicated pacing rate corresponding to the patient'"'"'s sensed hemodynamic need, the processor configured to;

select, as a test pacing rate, one of the sensor-indicated pacing rate and the predetermined pacing rate if the current pacing mode comprises a tracking mode;

select, as the test pacing rate, one of the sensor-indicated pacing rate and the pacing rate determined based on the patient'"'"'s heart rate before performance of the capture threshold test if the current pacing mode comprises a non-tracking mode;

switch operation of the pacing device from the current pacing mode to the capture threshold test; and

perform the capture threshold test using the test pacing rate.

11. The system of claim 10, wherein the pacing rate determined based on the patient'"'"'s heart rate before performance of the capture threshold test comprises a pseudo lower rate limit.

12. The system of claim 10, wherein the pacing rate determined based on the patient'"'"'s heart rate before performance of the capture threshold test is based on a measured heart rate to which a predetermined offset is added.

13. The system of claim 12, wherein the measured heart rate is determined prior to the capture threshold test based on a predetermined number of sensed and paced beats.

14. The system of claim 10, wherein the predetermined rate comprises a programmable pacing rate.

15. The system of claim 10, wherein the predetermined rate comprises a lower rate limit.

16. The system of claim 10, wherein the sensor-indicated pacing rate corresponding to the patient'"'"'s sensed hemodynamic need is based on patient activity.

17. The system of claim 10, wherein determining the sensor-indicated pacing rate corresponding to the patient'"'"'s sensed hemodynamic need is based on patient respiration.

18. The system of claim 10, wherein the processor is configured to adapt the test pacing rate to meet the patient'"'"'s hemodynamic need when performing the capture threshold test.

19. A cardiac rhythm management (CRM) system, comprising:

means for operating the CRM system at least at a sensor-indicated pacing rate and a non-sensor-indicated pacing rate;

means for operating the CRM system in a current pacing mode before a capture threshold test is performed;

means for determining a sensor-indicated pacing rate corresponding to a patient'"'"'s sensed hemodynamic need;

means for selecting a test pacing rate from one of the sensor-indicated pacing rate and a non-sensor-indicated pacing rate;

means for switching operation of the CRM system from the current pacing mode to the capture threshold test; and

means for performing the capture threshold test using the test pacing rate.

20. The system of claim 19, wherein the selecting means comprises means for selecting the test pacing rate to reduce a pacing rate change when the capture threshold test is initiated or terminated.
