Abstract

A method and apparatus are disclosed for treating obesity includes an artificial fistula created between gastrointestinal organs such as between the stomach and the colon. The method includes selecting an implant comprising a passageway having an internal lumen with an inlet end and an outlet end. The passageway is positioned passing through a first wall of first gastrointestinal organ (for example, passing through the wall of the stomach) and a second wall of a second gastrointestinal organ (for example, passing through the wall of the large intestine) with the inlet end disposed within an interior of the first gastrointestinal organ and with the outlet disposed within an interior of the second gastrointestinal organ.

Claims

1. A method for creating an artificial fistula between gastrointestinal organs to treat a gastrointestinal disorder, said method comprising:

selecting an implant comprising a passageway having an internal lumen with an inlet end and an outlet end;

positioning said passageway passing through a first wall of first gastrointestinal organ and a second wall of a second gastrointestinal organ with said inlet end disposed within an interior of said first gastrointestinal organ and with said outlet disposed within an interior of said second gastrointestinal organ.

2. A method according to claim 1 further comprising sealing an external surface of said passageway at each of said first wall and said second wall.

3. A method according to claim 1 wherein said first gastrointestinal organ is a stomach.

4. A method according to claim 3 wherein said second gastrointestinal organ is an intestine.

5. A method according to claim 1 further comprising restricting a flow through a pylorus of said patient.

6. An apparatus for creating a fistula between a stomach and an intestine of a patient, said apparatus comprising at least a first flow controller for creating a fistula from said stomach to said intestine of said patient.

7. An apparatus according to claim 6 further comprising a second flow controller for controlling a flow of contents from a stomach to a duodenum through a pylorus of said patient

8. An apparatus according to claim 7 wherein said second flow controller includes:

a passageway defining a lumen having an inlet and an outlet, said passageway having a longitudinal axis extending through said inlet and said outlet;

said passageway sized to be passed through a pylorus of a patient with said passageway having a length between said inlet and said outlet dimensioned for said outlet to reside in a duodenum of said patient when said inlet resides in a stomach of said patient;

said lumen dimensioned to restrict a flow of contents to an amount less than a flow of stomach contents through an untreated pylorus.

9. An apparatus according to claim 6 wherein said first flow controller includes:

a passageway having an internal lumen with an inlet end and an outlet end, said passageway sized to extend from said stomach to said intestine with said inlet residing in said stomach and with said outlet residing in said intestine;

an anchor for sealing an external surface of said passageway.

10. An apparatus according to claim 9 wherein said anchor comprises:

a. at least a first expandable chamber attached to the passageway proximate said inlet;

b. at least a second expandable chamber attached to the passageway proximate said outlet; and

c. said first and second chambers adapted to seal against opposing tissue of at least one of said stomach and said intestine upon expansion of said chambers.

11. An apparatus according to claim 10 wherein said anchor further comprises:

a. a third expandable chamber attached to the passageway proximate said inlet;

b. a fourth expandable chamber attached to the passageway proximate said outlet; and

c. said first and third chambers and said second and fourth chambers adapted to form first and second cooperating pairs upon expansion for a stomach wall to be captured between said first pair of chambers, and with an intestine wall captured between said second pair of chambers.

12. An apparatus according to claim 9 comprising a valve in said lumen.

13. An apparatus for treating a gastrointestinal disorder, said apparatus comprising:

a passageway defining a lumen having an inlet and an outlet, said passageway having a longitudinal axis extending through said inlet and said outlet;

said passageway sized to be passed through a pylorus of a patient with said passageway having a length between said inlet and said outlet dimensioned for said outlet to reside in a duodenum of said patient when said inlet resides in a stomach of said patient;

said lumen dimensioned to restrict a flow of contents to an amount less than a flow of stomach contents through an untreated pylorus.

14. An apparatus according to claim 13 wherein said anchoring system includes:

a first expandable chamber attached to the passageway proximate said inlet;

a second expandable chamber attached to the passageway proximate said outlet, wherein the first expandable chamber is spaced from the second expandable chamber along the longitudinal axis; and

each of said chambers sized to resist passage through said pylorus when expanded.

15. An apparatus according to claim 14 wherein said first expandable chamber is sized to urge against a wall of an antrum of said patient with a force sufficient to induce a sensation of satiety.

16. An apparatus according to claim 13 wherein the passageway is collapsible in response to compressive forces of said pylorus.

17. An apparatus according to claim 13 comprising a lumen adjuster to alter a size of said lumen.

18. An apparatus according to claim 13 further comprising a sleeve connected to said outlet and sized to direct nutrient flow distal within said duodenum.

21. An apparatus according to claim 13 wherein said first expandable chamber is sized to urge against a wall of an antrum of said patient with a force sufficient to induce a sensation of satiety.

19. An apparatus according to claim 24 wherein said sleeve is sized to extend past a papilla of said patient.

20. An apparatus according to claim 24 wherein said sleeve is at least partially permeable.
