Abstract

A method of implanting a prosthetic valve in a stenosed aortic valve via a catheterization technique. A balloon catheter is advanced into a patient'"'"'s vasculature and a balloon is expanded within the stenosed aortic valve to push aside the calcified leaflets. The prosthetic valve is then introduced into a patient'"'"'s vasculature through an 18 French arterial introducer. After advancement through the femoral artery and aorta, the prosthetic valve is radially expanded for implantation in the dilated aortic valve. The prosthetic valve includes a compressible and expandable frame and a valvular structure made with pericardial tissue. The prosthetic valve further includes an internal cover fastened to an internal surface of the frame between an inlet end of the frame and the valvular structure.

Claims

1. A method of implanting a prosthetic valve in a stenosed aortic valve without surgery, comprising:

advancing a balloon catheter through a femoral artery and aorta until a distal end portion of the balloon catheter is positioned within the stenosed aortic valve;

inflating a balloon to push aside valvular leaflets of the stenosed aortic valve, the balloon mounted on the distal end portion of the balloon catheter;

deflating the balloon and withdrawing the distal end portion of the balloon catheter from the stenosed aortic valve;

introducing a prosthetic valve into a patient'"'"'s vasculature through an 18 F (5.7 mm) arterial introducer, the prosthetic valve comprising a radially compressible and expandable frame and a flexible valvular structure made with pericardium, the valvular structure positioned entirely between upper and lower extremities of the frame when the valvular structure is in a closed state, the prosthetic valve having an internal cover fixed on an inner wall of the frame between the lower extremity of the frame and the valvular structure;

advancing the prosthetic valve through the femoral artery and aorta by a catheterization technique until the prosthetic valve is positioned within the stenosed aortic valve; and

radially expanding the prosthetic valve within the stenosed aortic valve;

wherein, after radial expansion, the prosthetic valve replaces the function of the stenosed aortic valve and wherein the entire method is performed without extracorporeal circulation.

2. The method of claim 1, wherein the frame comprises intercrossing bars having diameters in the range of about 0.1 to 0.6 mm.

3. The method of claim 2, wherein the internal cover is made with pericardium and is adapted for preventing the passage of blood through spaces between the intercrossing bars of the frame.

4. The method of claim 3, wherein the intercrossing bars are rounded and smooth linear bars.

5. The method of claim 1, wherein the valvular structure forms a continuous surface and is provided with guiding means.

6. The method of claim 5, wherein the guiding means create stiffened zones which induce the valvular structure to follow a patterned movement when moving from the closed state to an open state.

7. The method of claim 6, wherein the guiding means are further configured to prevent eversion of the valvular structure.

8. The method of claim 1, wherein the second catheter is a balloon catheter and the frame is balloon expandable.

9. The method of claim 1, wherein a substance with anti-thrombotic properties is incorporated into the valvular structure.

10. The method of claim 1, wherein the frame constitutes a grate type structure.

11. The method of claim 1, wherein the upper and lower extremities of the frame are rounded and smooth.

12. The method of claim 1, wherein the frame comprises an expanded external diameter of about 20 to 25 mm.

13. The method of claim 1, wherein the frame comprises a concave shape profile.

14. The method of claim 1, wherein the frame is metallic and is viewed under fluoroscopy while advancing the prosthetic valve.

15. The method of claim 1, wherein the prosthetic valve is positioned such that valvular leaflets provide a stable base for fixing the frame within the native aortic valve.

16. The method of claim 1, wherein the frame has a height of at least 10 mm after expansion.

17. The method of claim 1, wherein the lower extremity of the frame is located above the plane of the mitral valve in the human heart after the prosthetic valve is radially expanded such that the mitral valve remains free of obstruction from the prosthetic valve.

18. The method of claim 1, wherein the step of inflating a balloon to push aside valvular leaflets of the stenosed aortic valve comprises dilating the stenosed aortic valve to a diameter of at least 20 mm.

19. The method of claim 1, wherein the lower extremity of the frame forms a zig-zag shape and the internal cover has an inlet end sewn to the lower extremity of the frame along a zig-zag line.

20. A method of implanting a prosthetic valve in a stenosed aortic valve without extracorporeal circulation, comprising:

advancing a balloon catheter over a guide wire and through a femoral artery and aorta until a distal end portion of the balloon catheter is positioned within the stenosed aortic valve;

inflating a balloon to push aside valvular leaflets of the stenosed aortic valve, the balloon mounted on the distal end portion of the balloon catheter;

deflating the balloon and withdrawing the balloon catheter from the patient;

introducing a prosthetic valve into the patient'"'"'s vasculature through an 18 F (5.7 mm) arterial introducer, the prosthetic valve comprising a radially compressible and expandable metallic frame and a flexible valvular structure made with pericardium, the valvular structure positioned entirely between upper and lower extremities of the frame when the valvular structure is in a closed state, the prosthetic valve having an internal cover made with pericardium and fixed on an inner wall of the frame, the internal cover positioned only between the lower extremity of the frame and the valvular structure;

advancing the prosthetic valve through the femoral artery and aorta by a catheterization technique until the prosthetic valve is positioned within the stenosed aortic valve; and

radially expanding the prosthetic valve within the stenosed aortic valve.

21. The method of claim 20, wherein the compressible and expandable metallic frame is formed with bars having intersections, the bars having diameters in the range of about 0.1 to 0.6 mm.

22. The method of claim 21, wherein the upper extremity of the frame is formed with a zig-zag shape and the lower extremity of the frame is formed with a zig-zag shape.

23. The method of claim 22, wherein a lower end of the internal cover is fastened to the lower extremity of the frame along a zig-zag line and wherein an upper end of the internal cover is sewn to the valvular structure.

24. The method of claim 23, wherein the frame comprises a concave shape profile.

25. The method of claim 24, wherein the upper and lower extremities of the frame are rounded and smooth to avoid trauma to the aorta or myocardium.

26. The method of claim 20, wherein the intercrossing bars are rounded and smooth linear bars.
