Abstract

Devices, methods and kits for forming and/or advancing one or more knots in a tether (e.g., during a tissue modification procedure) are described. In some variations, a tether may be used to gather or compress tissue by bringing two pieces or sections of the tissue together. The tether, which may be under tension, may then be locked to maintain the tension (e.g., by forming one or more knots in the tether). In some variations, an open knot may be advanced along a tether before being converted into a closed knot. The devices, methods and/or kits may be used, for example, in minimally invasive procedures.

Claims

1. A knot pusher for advancing a knot in a tether, the knot pusher comprising:

an elongated member;

a first coupling member slidably coupled to the elongated member;

a second coupling member fixedly coupled to the elongated member, wherein at least a portion of the second coupling member extends distally of the elongated member;

a first bearing; and

a second bearing distal of the first bearing,

wherein the first and second bearings are fixedly coupled to the first coupling member and slidably coupled to the second coupling member.

2. The knot pusher of claim 1, wherein at least one of the first and second coupling members comprises a wire.

3. The knot pusher of claim 1, wherein the elongated member comprises a first lumen, and the first coupling member is slidably disposed within at least a portion of the first lumen.

4. The knot pusher of claim 3, wherein the elongated member comprises a second lumen, and the second coupling member is fixedly disposed within a portion of the second lumen.

5. The knot pusher of claim 1, wherein the first bearing comprises a first tapered portion and the second bearing comprises a second tapered portion.

6. The knot pusher of claim 5, wherein the first and second bearings are slidably coupled to the second coupling member at the first and second tapered portions.

7. The knot pusher of claim 1, wherein the first tapered portion of the first bearing has a first aperture therethrough and the second tapered portion of the second bearing has a second aperture therethrough, and wherein the first coupling member passes through the first and second apertures.

8. The knot pusher of claim 1, further comprising a tubular member having a proximal end, a distal end, and a lumen therethrough, wherein the elongated member is disposed within the tubular member.

9. The knot pusher of claim 8, wherein the tubular member comprises a wall portion having a first aperture therethrough.

10. The knot pusher of claim 9, wherein the first and second bearings are disposed within the tubular member, and the first aperture is positioned adjacent the first bearing.

11. The knot pusher of claim 9, further comprising a hypotube having a second aperture therethrough, wherein the hypotube is disposed within the tubular member such that the second aperture is aligned with the first aperture.

12. The knot pusher of claim 1, further comprising a cutting member.

13. A method of tying a knot in a tether coupled to body tissue using a knot pusher comprising first and second bearings and a coupling member slidably coupled to the first and second bearings, the method comprising:

winding a portion of the tether around the first and second bearings to form an open knot in the tether;

advancing the knot pusher distally to advance the open knot distally;

withdrawing the coupling member from the first and second bearings; and

forming a closed knot from the open knot after the coupling member has been withdrawn from the first and second bearings.

14. The method of claim 13, wherein the method comprises pulling the tether to form the closed knot from the open knot.

15. The method of claim 13, wherein the first and second bearings are disposed in a lumen of a sheath comprising a wall portion having an aperture therethrough, and wherein the method further comprises passing the tether through the aperture.

16. The method of claim 13, wherein the body tissue comprises cardiac tissue.

17. The method of claim 16, wherein the cardiac tissue comprises heart valve tissue.

18. The method of claim 17, wherein the heart valve tissue comprises mitral valve tissue.

19. The method of claim 13, wherein the tether is coupled to a plurality of anchors that are coupled to the body tissue.

20. The method of claim 13, wherein the method comprises using a lasso to wind the portion of the tether around the first and second bearings to form the open knot in the tether.
